LISWT for erectile rejuvenation

Objectives: Low Intensity Shockwave Therapy has been FDA cleared for the treatment of cellulite. Because the mechanism of action is one of inducing micro-trauma, new vessels are formed post-treatment. This talk will focus on mechanism and a literature review of this novel new approach.

Introduction: This treatment has now been shown to be highly effective for vascular induced erectile dysfunction irrespective to the response to PDE5i oral agents.

Materials / method: Both a literature review and a day to day approach within a clinical setting will be reviewed.

Results: Low Intensity Shockwave Therapy has been shown to be effective for improving erectile dysfunction in me with vascular induced issues. A variety of different treatment protocols have been used.

Conclusion: This approach represents a radical new, simple, approach to promote erectile rejuvenation and enhanced male sexual wellness.

PRP for erectile dysfunction

Objectives: Objectives : PRP (Platelets Rich Plasma) for Erectile Dysfunction (ED) is widely used in some Men’s health centers. Because the mechanism is well-known in different areas as sports medicine, rheumatology and others medical conditions, this lecture will focus on the scientific literature in Erectile dysfunction and the possible extension of its use in urology ans specifically in ED.

Introduction: Introduction : evidence-based-medecine for PRP is just emerging in the field of erectile dysfunction despite the current use for ED.

Materials / method: Materials/Methods : Both a literature review and the current practice with a pragmatic and medical point of view is reviewed.

Results: Results : PRP has not yet completely proved efficiency in ED at the present time. Nevertheless, some theoretical and preliminary studies may suggest a potential efficiency, and we need urgently robust and well-designed studies to better understand and stratify the indication of PRP for ED in the future.

Conclusion: PRP may become in the future a new treatment for ED, eventually with a synergistic effect with others treatments as PDEI and/or Low Intensity Shock Wave.

Botulinum toxin and erectile dysfunction: new concept

Objectives: The prevalance of ED , underlying causes and comorbidities , why diagnosis of ED is important , ED could be the first sign of cardiovascular diseases , major risk factors , diagnostic tools and evaluation , recent management guidelines for ED , future and experimental treatments and rule of botox in ED

Introduction: Erectile dysfunction a major Male sexual Disorder , prevalence in some studies 52% of male above 40 years old , causes underlying could be psychogenic or organic mainly mixed , may be signal of underlying diseases , or cardiovascular disease , EHS used for diagnosis , recent and future management guidelines are available with significant result including intra corporal injection of botox

Materials / method: review of update researches in causes , recent management , future management , theories for erectile dysfunction management including Botox , P shot , Stem cells .
48 candidas to évalue the effective rule of Botox injection intra corporal injection , group A 24 control received IC 1 ml saline , group B 24 received IC single injection BTX-A 50 units

Results: significant results in the Sexual Encounter Profile , Global Assessment Questions ( GAQ) , and the erection hardness score (EHS) of the group B received IC BTX-A-50 units compared by the controlled group A received IC 1ml saline

Conclusion: The results suggest the possibility of intra- cavernosal BTX-A in treatment of patients with sever erectile dysfunction , and may lead to a reduction in the number of patients requiring penile implant surgery , results more significant if combined with immediate pumping ,

The new era with shockwaves in erectile dysfunctions (unrestricted educational grant from Zimmer MedizinSysteme)

Objectives: Prospective study to evaluate the safety and preliminary effectiveness of the use of radial Shockwaves device in erectile disfunction

Introduction: Erectile dysfunction (ED) is defined as persistent difficulty achieving and maintaining an erection sufficient to have sex. Causes are usually medical but can also be psychological. Organic causes are usually the result of an underlying medical condition affecting the blood vessels or nerves supplying the penis. With shockwave treatment, it is possible to significantly promote increased perfusion and stimulate the formation of new vascular structures at the same time. The radial shockwaves, which are emitted on the shaft of the penis as well as on the area around the genitals, support the natural stimulation of the area to be treated, particularly in the case of erectile dysfunction due to vascular causes. Even patients with severe erectile dysfunction who are taking so-called PDE-5 inhibitors achieve significantly better therapeutic success.
Depending on the degree of erectile dysfunction, an average of about 5 to 8 treatments are needed to achieve a lasting improvement in the erection. Shockwave therapy takes up to 25 minutes and should be performed 2-3 times per week.

Materials / method: Thirty-two patients with erectile disfunction were recruited to perform 6 sessions of radial Shockwaves Device during three weeks of initial treatment. Follow-up after three months. The patients received sessions lasting 10 minutes, completing the different treatment protocols. Initially the patients underwent a quality of life test before and after treatment, evaluation with advanced ultrasound using elastography to measure the initial tissue's elasticity and vascularization and be able to compare after treatment, clinical functional evaluation and urodynamic test , flowdoppler and disfunction erectile test.

Results: No adverse reactions were observed. All the patients finished the treatment sessions. The treatment was highly satisfactory in 89,7 % of the patients. After the first three months the improvement was maintained in 85 % of the patients. No Tissue injuries were observed. Elastographic changes and improvement of sexual health were detected by advanced ultrasound (elastography) in 100% of patients.

Conclusion: The radial Shockwaves is safe, well tolerated and effective for the treatment of erectile disfunction. The observed elastographic changes demonstrate the improvement degree of satisfaction after treatment. We have demonstrated by an advanced ultrasound an increase in angiogenesis and in the increase in penile vascular perfusion.
Recommendations: Continue increasing the number of cases for research and increase the variables that we have decided to incorporate in the next research section such as MRI and pressure calculation.
