Reduction mammaplasty by the Fish-Tail-Technique

Objectives: Present a fast mammoplasty technique indicated when flattened upper breast pole, moderate ptosis and good skin quality.

Introduction: Many patients consult with the desire for youthful, firm breasts without implants.The task is to transform a flat breast into a conical one with the shortest possible scars.

Materials / method: N=92 patients have been included in the study. Principles, technique and outcomes are presented.

Conclusion: Mammaplasty by the fishtail-technique is an excellent alternative and is a real re-sculpture of the gland with an inferior gland flap - the fishtail - which projects the areola.

Breast lipofilling: a two-in-one treatment

Objectives: The author presents his experience of more than 100 cases on breast Lipofilling: personnal technique and treaments of complications

Introduction: Lipofilling is an old technique that was taught by one of France’s pioneering aesthetic doctors, Doctor Fournier, back in the 1980s. A few years ago, the lipofilling technique was only used for post-cancer breast reconstruction. Now, fat injections can be performed for purely aesthetic purposes.

Materials / method: Fat transfer into the breast is an approved and highly effective technique. The results of breast lipo lling are always very natural-looking, because the breasts are made up of fat and glands. The visual appearance and feeling of the breasts give this procedure excellent aesthetic results. Furthermore, the figure is also corrected by liposuction in the areas where the fat is harvested (hips, abdomen, saddlebags, knees). The patient gets double the results!

Results: In cosmetic surgery, it is a growing and almost systematic demand by patients with lipoaspiration. Fat is an autologous product, meaning that lipofilling is a natural (organic) intervention that does not involve any foreign bodies, and is therefore suitable for patients who do not want breast implants. Breast lipofiling can also be coupled with breast implants, enabling us to achieve more natural results in patients who are particularly thin, by covering the implant with a layer of fatty tissue. Conclusion: The author presents his re-injection aspiration protocol using a simple method of fat freezing then washing and reinjection in a short time of less than one hour. This personal technique improved for more than six years gives lasting results with less than 30% fat melting. The author presents the technique in more detail step by step but also the good and bad indications, the disadvantages and possible complications as well as their mode of treatment.

High-definition fat grafting breast augmentation using anchor threads

Objectives: Modern fat grafting breast augmentation allows to successfully enhance a breast. However, there is no fine control on breast footprint and IMF, as well as on breast projection and shape. In this paper, the author introduces a novel scarless technique to enhance breast shape in fat grafting breast augmentation (FGBA) and evaluate its beneficial effect in improving the cosmetic outcomes.

Introduction: When compared to breast augmentation with implants, FGBA is a closed procedure based on “liquid” augmentation, which does not allow to provide a high defined volume as for “solid” (i.e. implant) augmentation where the implant dictates shapes, volume and volume distribution. Thus, FGBA suffers three main technical limitations: no fine control on final breast footprint, no fine control on position of the new inframammary fold (IMF) and no fine control on breast projection and shape.

Materials / method: Fifty-one consecutive patients undergoing cosmetic breast augmentation with fat only between September 2012 and August 2016 were retrospectively reviewed. In the first 29 cases(56%) we did not use dual anchor cog threads(Group A-control group) whereas in the remaining 22 cases(44%), we used dual anchor cog threads (Group B, case group).Breast shape analysis was performed using a standardized evaluation method and the BREAST-Q was used to study patients' satisfaction. Mann-Whitney U test and Chi-square tests were for categorical variables, independent-samples t-test for continuous variables.

Results: No significative difference in graft take was found (mean Group-A 71.2% vs Group-B 71.6%, p<0.05). Group B showed significantly higher rating than Group A, for lower pole profile, IMF and lateral footprint definition. No major postoperative complications were experienced in both groups. In Group B no thread-related complications were experienced.

Conclusion: The dual anchor thread suture is a novel, effective, simple, reliable, safe and scarless method to improve breast shape in fat grafting breast augmentation, allowing to achieve high-defined results.

Ecological impact of plastics used in breast augmentation surgery

Objectives: We wanted to evaluate the impact of breast implant augmentation and revision on plastic use and thus, after quantification, to become aware of the amount of plastic used for surgery in order to consider solutions to reduce their use.

Introduction: With more than 329 914 breast augmentations performed according to ASAPS statistics in the United States and 49,834 revisions in 2018, breast augmentation represents a significant part of the plastic surgeon's activity.
In recent years, there has been a real awareness of the use of plastics in the industry. Indeed, the use of plastic has generally resulted in global over-consumption and pollution.

Materials / method: As part of a change of implants for a 42-year-old patient due to shelling, we collected, photographed, weighed and evaluated the volume of all plastic waste used by both the surgeon and the anesthesiologist.
We did the same for a simple bilateral breast augmentation in a 25-year-old patient.

Results: The study makes us aware that plastic is the main tool used in the operating room in surgery, particularly for breast augmentation.

Breast augmentation has a specificity because silicone waste is present.
In proportion to the number of breast augmentations performed in the United States according to ISAPS figures, the waste represents more than 1,200 tons of plastic. This quantity, extrapolated worldwide, but also for all surgeries and over the last 30 years, represents billions of tons of plastic burned or deposited in the form of waste. Conclusion: The use of plastic during a breast augmentation represents thousands of tons of plastic used each year and probably millions since the rise of breast augmentation. There is an ecological urgency for surgeons and clinics to raise awareness among manufacturers to find solutions to improve the ecological impact of surgical procedures.
