Objectives: To analyze the cosmetic outcome in patients submitted to upper lid blepharoplasty and ptosis correction at the same surgical time.
Introduction: Both Müller's muscle-conjunctiva resection (MMCR) and levator aponeurosis advancement (LA) are effective in managing ptosis in patients with good levator function. The aim of this talk is to evaluate the efficacy and cosmetic outcome of these two procedures when performed together with upper lid blepharoplasty.
Materials / method: The outcome of surgery was analyzed in patient with upper lid ptosis and dermatochalasis with good levator function. All patients were accurately tested for eye dominance, Hering's phenomenon, orbital volume deficit, brow asymmetry.
Results: MMCR is more predictable, does not need intra-operative adjustment, and recovery following surgery is very rapid.
LA allows intra-operative adjustment, however is less predictable and post-operative swelling may be remarkable.
Conclusion: Both Müller's muscle-conjunctiva resection and aponeurosis advancement are effective for mild to moderate ptosis in patients with good levator function. Patients submitted to MMCR have a lower incidence of overcorrection than patients undergoing to LA when the operation is performed together with upper lid blepharoplasty, but the surgical option should be custom-made for each patient in order to achieve the best cosmetic result.
