Genoskin - Recycling discarded human skin samples from surgical resections to accelerate drug development

Objectives: To quantify the amount of botulinum toxin-A (BoNT-A) protein in each vial of commercial BoNT-A product and compare the light chain activity of 150 kDa BoNT-A in different products.

Introduction: BoNT-A effects are mediated by the 150 kDa BoNT-A neurotoxin. Each product available has a unique manufacturing process, different excipients and noninterchangeable potency units. For example, in glabellar lines, total doses are abobotulinumtoxinA (AboBoNT-A) 50U, onabotulinumtoxinA (OnaBoNT-A) 20U, and incobotulinumtoxinA (IncoBoNT-A) 20U. Light chain activity of 150 kDa BoNT-A in different products is unknown.

Materials / method: We used sandwich ELISA with antibodies specific to 150 kDa BoNT-A to quantify the amount (pg) of 150 kDa BoNT-A in each product; each of the commercial products were quantitated versus a calibration curve of recombinant, pure BoNT-A. Light chain activity was assessed using EndoPep assay, where the concentration of the cleaved target was measured and quantity of 150 kDa BoNT-A determined relative to quantity of recombinant BoNT-A required for equivalent light chain activity.

Results: The amount of neurotoxin per product unit was: AboBoNT-A (5.38pg), OnaBoNT-A (9.04pg) and IncoBoNT-A (4.03pg). There were no significant differences between BoNT products in light-chain activity. Thus, when total recommended doses for glabellar lines are injected, there is more neurotoxin injected in AboBoNT-A (50U x 5.38pg = 0.269ng) than in OnaBoNT-A (20U x 9.04pg = 0.181ng) and IncoBoNT-A (20U x 4.03 = 0.081ng).

Conclusion: No differences in light chain activity were seen between BoNT-A products. However, there were greater amounts of neurotoxin in AboBoNT-A at the recommended dose for glabellar lines than either OnaBoNT-A or IncoBoNT-A. At the recommended dose, all BoNT-A products have a similar safety profile. At approved doses, the amount of active neurotoxin in AboBoNT-A is likely to be a major factor to explain the long-lasting symptomatic relief observed in clinical studies within a well-characterised safety and tolerability profile.

Objectives: To evaluate the safety and pharmacodynamic (PD) profiles of the first recombinant BoNT serotype E (rBoNT-E) versus abobotulinumtoxinA (AboBoNT-A).

Introduction: Different botulinum toxin (BoNT) serotypes have different pharmacological features, which can be exploited for clinical benefit. An interesting profile could be a BoNT with a fast onset of action and limited duration of effect.

Materials / method: In this double-blind, placebo-controlled ascending dose study, 28 healthy males were randomised (3:1) to administered to injections of rBoNT-E and placebo in the right extensor digitorum brevis (EDB) muscle (sequential cohorts; up to 3.6ng rBoNT-E). A further 24 subjects were randomised (6/treatment arm) to receive a double-blind injection of aboBoNT-A 20, 40 or 70U, or placebo. Pharmacodynamics was assessed by recording compound muscle action potential (CMAP) of the injected EDB following supramaximal stimulation. Data from active treatment groups only (rBoNT-E and AboBoNT-A) are described.

Results: All rBoNT-E doses were well tolerated. Most treatment-emergent adverse events were evaluated as unrelated to study treatment. No unexpected treatment-related toxicities were identified with rBoNT-E vs. aboBoNT-A. rBoNT-E had a faster onset of effect (15% inhibition of CMAP), quicker peak effect and shorter duration of action vs. AboBoNT-A. For rBoNT-E, the onset of effect was on Day 1-2 with peak effect by Week 1 compared with an onset of AboBoNT-A effect on Day 1-7 with peak effect by Weeks 2-6. The duration of effect was ~7 weeks for 0.9 and 3.6ng of rBoNT-E vs. >26 weeks for AboBoNT-A.

Conclusion: A comparatively good safety profile of single intramuscular doses of rBoNT-E was demonstrated up to 3.6ng. rBoNT-E has faster onset of effect, greater and quicker peak effect and shorter duration of action versus AboBoNT-A, when injected in EDB muscles of healthy males. The PD profile of rBoNT-E addresses new and different patient needs both in therapeutics and in aesthetics.

Objectives: To understand factors impacting patient satisfaction with BoNT-A treatment.

Introduction: Positive self-image and self-esteem are key drivers for choosing non-surgical treatments. While patient expectations for aesthetic treatment are high, patients generally report a positive experience provided that they are given good quality information, have a good clinician rapport and adequate post-treatment follow-up. Positive experiences are reinforced by satisfaction with results of the treatment and its duration

Materials / method: Two international, web-based patient surveys were conducted to understand participant opinions of aesthetic products and provide physicians with insights that might improve patient satisfaction and clinical practice. Survey 1 was conducted in Colombia, Lebanon, Malaysia, Russia and Turkey and Survey 2 was conducted in Colombia, Russia, Thailand, Turkey and the United Arab Emirates.

Results: In the first survey, patient-ratings of satisfaction with previous aesthetic medical procedures (7 point Likert scale where 0 =very dissatisfied and 6 =very satisfied) ranged from 4.8 to 5.4. In the second survey, patient-ratings of satisfaction with BoNT-A treatment outcome (10 point Likert scale where 1 =not satisfied at all and 10 =completely satisfied) ranged from 7.0 to 8.7. Top reasons for dissatisfaction with treatment outcome were: did not meet expectations (56%), did not look natural (24%), the physician did not set realistic expectations (22%).

Conclusion: Some patients remain dissatisfied with BoNT-A treatment, most commonly by treatment not meeting expectations. It is therefore highly important that factors affecting treatment satisfaction (such as treatment onset, duration of response, realistic outcomes), be discussed with patients when recommending treatments to match their individual expectations.

Viol - Microneedle pulse RF: a new approach in managing pigmentary disorders

Objectives: To look into the importance of adding micro needle pulse radiofrequency into the management of melasma.

Introduction: Pigmentary disorder, especially Melasma, can be difficult to be fully treated with current available technologies. Most of the time, managing them with combination of medications, creams, and lasers help to contain the hyperpigmentation from coming back. However, we also understand the limitation due to increase in abnormal blood vessels and disruption of the basement membrane.

Materials / method: We added micro needle pulse radiofrequency to our treatments of melasma, especially the recalcitrant melasma cases, and saw drastic improvement with the treatment outcome.

Results: Microneedle Pulse Radiofrequency seems to work to improve the melasma treatment outcome perhaps through: restoration of basement membrane, reduction in abnormal blood vessels, and rejuvenating senile fibroblasts.

Conclusion: Microneedle pulse radiofrequency might be a very strong tool to be used in combination to fight the war of melasma.

Objectives: The audience will learn how acoustic wave therapy (AWT) is acting on hyperactive muscles based on the extensive evidence derived in muscular hyperactivity in spasticity in neurology. AWT is able to reduce muscular tension. A number of wrinkles are caused by muscular hyperactivity and/or volume depletion. As such, muscular hyperactivity can be significantly reduced.

Introduction: Acoustic wave therapy (AWT) as acoustic waves act via extracorporeal shockwaves (ESWT) on biological tissues. Besides stem cell activation, a number of additional effects of AWT have been discovered over the past decade. As such antifibrotic actions of AWT via TGF-ß-pathways have been elucidated. Facial wrinkles might be the result of muscular hyperactivity, where BTX is frequently used and/or by volume depletion. In neurology, a vast number of clinical studies have been published on AWT in muscular hyperactivity/spasticity.

Materials / method: In September 2018 electronic databases as well as reference lists of the available studies were evaluated on AWT/ESWT on muscles in a systematic evidence-based review.

Results: This meta-analysis included 888 patients from 10 randomized-controlled trials (RCTs, n=488) and 14 non-RCTs (n=400) on AWT in muscles. Regarding the ESWT technology used, four RCTs used radial ESWT and five RCTs focused, while one RCT compared radial and focused ESWT on spastic gastrocnemius muscles. Both, radial and focused ESWT improved function and reduced spasticity significantly. On average 3-5 AWT sessions (1500 shots, 4-5Hz) were applied, while two studies did up to 12 AWT sessions.

Conclusion: Both, focused and radial AWT improve wrinkles caused by muscular hyperactivity. In neurology, AWT in spasticity has been extensively studied with significant beneficial effects up to 3 months after a single AWT session. Combination therapy with other physical modalities is an intriguing option.

T-Lab regenerative medicine - Stromal vascular fraction: how is non-enzymatic digestion possible?

Objectives: Due to strong issues and restraints have come up against enzymatic isolation methods, many groups and companies have turned their face to develop non-enzymatic, mainly mechanical methods to dissociate the cells from adipose tissue. An alternative to enzyme use, we demonstrated here a protocol of an in-vitro study of cells dissociated using Microlyzer from manually harvested lipoaspirate.

Introduction: The adipose tissue is known as one of the most important sources generically for “stem cells”. ASCs are isolated at the point of care from the lipoaspirate tissue as the stromal vascular fraction. The most common isolation method requires the use of collagenase enzyme thus adipose stem cells are able to be isolated using various techniques however some of the methods are being considered more than “minimally manipulated” by current good manufacturing practice requirements. Alternatively, mechanical isolation methods are set to demonstrate whether the possibility of isolation of ADSCs.

Materials / method: Manually harvested lipoaspirate is used in this study. The design of Microlyzer offers 2-way connectors of Microlyzer catridges with luer-connectors for syringes. One side syringe with lipoaspirate is affixed and the other side there is a vacant same volume syringe is attached. The Microlyzer cartidge is a nest fort he blade-filters. The blade-filters are 2400 to 600 microns. The device requires 3 steps of cutting of lipoaspirate for mechanical separation of the cells from the ECM. We used Luna Stem (Logos Bio, S. Korea) which can identify the population of nucleated and non-nucleated cells.

Results: Mechanical methods from literature offers wide range of nucleated cell counts/ml with different viability rates. Markarian et al (2014) demonstrated 25.000 (RBC lysis and centri.) and 10.000 (centrifuge) nucleated cells/ml with 65 – 70% viability. Conde-Green et al (2014) demonstrated 11.500 (high speed centri.) – 23.000 (vortexing and centri.) nucleated cells/ml with 80% - 90% viability. Millan et al (2014) demonstrated 140.000 nucleated cells/ml with 87,3% viability. Our protocol bladed out the lipoaspirate, then centrifuged at 1300G for 3 mins. 3ml analyzed. Resulted 157.000 nucleated cells

Conclusion: The standard enzymatic SVF isolation methods have been demonstrated with clinical benefits. However the enzymatic method requires longer protocol durations, higher costs and risks associated to presence of enzyme residual activities in the frame of regulation issues. Our method and device may be an alternative to enzymatic isolation methods due to the factors of regulation restaints, costs and duration of the isolation processes. Yet, still compared to enzymatic isolation cell numbers seem to be less. Further studies are required to increase the nucleated cell population in the final product.

SVF-stromal vascular fraction, HFSc-hair follicle stem cells) is injected in affected areas with deep mesotherapy technique as an autologous micrografts.

Results: The procedure is simple, fast and efficient with no downtime. First results in most cases occur after 30-60 days. This metod is considered to be effective in reversing hair miniaturization in AGA and as a result clinically improving hair density. Clinical response rate to this treatment is about 80-90% with proper qualification of the patient (clinical examination and trichoscopy).

Results: The results have show that PRP is not as effective as it has been claimed. PRP has failed to deliver on its promise. We examined many applications of PRP and no standard protocol or dosage has been make available by any of the companies or studies.

Conclusion: We concluded that PRP is not favorable in many aspects of aesthetics and new better and safer alternatives are now available.

2. ASC-exosomes can be a novel promising cell-free therapeutic modality for AD treatment.
