Tips for successful and safe anesthesia for hair transplantation surgery

Objectives: The demand for hair transplantation surgery is increasing worldwide, so proper anesthesia will make operation more acceptable. For hair transplantation surgeons it is important to know how to avoid anesthesia related complications and how to manage them.

Introduction: It was soundly said about aesthetic patients : if you pain them you will loose them. This is absolutely correct in hair transplantation surgery. The impact of properly anesthetizing hair transplantation patients is great. Not only this but knowing the medical history of the candidates and proper dosing and methodology of drug administration will be of great value for a successful hair transplantation surgery outcome.

Materials / method: The techniques , dosing, and different ways of hair transplantation anesthesia will be explained in details with illustrative videos > A special care will be given to how to avoid potential complications and how to manage them.

Results: The expected results of the presentation will be better and safer hair transplantation surgery

Conclusion: The better practice of hair transplantation surgery will be reflected on both candidates and physicians . Properly administered anesthesia will positively improve the outcome of hair transplantation surgery.

Unshaven hair transplant

Objectives: Unshaven hair transplant is a beautiful advancement in the field of hair transplant wherein patients can avoid the sometimes uncomfortable phase of going completely bald before the surgery thereby increasing the patient's compliance towards hair transplant on the whole.Using a manual sharp punch with stabbing motion during graft harvesting can be the key element in performing this technique

Introduction: In today's scenario when lot of patients are not comfortable disclosing that they have gotten transplant done or they may feel inconvenience in going totally bald, unshaven sessions are the need of the hour.
In unshaven hair transplant technique described here, the patient does not need to shave his scalp. The extraction is done by using a manual punch by the stabbing method. Due to absence of any oscillatory motion of the punch, there is no entanglement of hair shafts and transaction of hair is also reduced. Implantation is done by using implanters

Materials / method: The technique involves usage of a manual punch to extract grafts through the FUE procedure without shaving the scalp of the patient.First step will be local anesthesia followed by extraction of grafts with a sharp manual punch with stabbing method. To and fro motion of the punch will be avoided. Implantation will be done using implanters in between the existing hair in the recipient area, in a single step without creating reception holes.

Results: Neither the implanted area nor the extracted area was shaved. 1500 grafts were extracted and implanted.

Conclusion: The methodology of unshaven FUE is not dissimilar to Micro FUE wherein grafts are extracted one by one from the donor and then implanted. The difference lies in the way extraction and implantation takes place due to existence of hair both in donor and implanted area.

Debate difficult cases in surgical hair loss management

Objectives: The Objective of this presentation is to understand that in every specialty there are always challenging cases. In Hair restoration, some are because of severe limitation of the donor area, others are because of the underlying progressive conditions. Yet others are due to the complications of the previous treatments.

Introduction: In this paper, we will be discussing the options of the treatment plans to address the challenging issues and, will also discuss how to reduce the complications of a procedure. The understanding of the co-relation that exist between the density, caliber, length and the styling of hair to address such difficult cases.

Materials / method: The main method of addressing the difficult cases are based on using the judgement of underlying medical problem, the understanding of the donor area, the tools of surgery, and other options of camouflage like SMP.

Results: Once we as physicians understand the basic concepts of problems we are facing, our limitation, what to do and what not to do. We can almost address all the problems in difficult cases.

Conclusion: In every specialty, there are always difficult cases that we face. The key is to understand the underlying medical cause, the limitation of a procedure, to learn about patient selection and also the selection of the procedure that you will choose. In many cases, one need to understand that best option could be simple camouflage or acceptance of the conditions, rather surgery.

Hair transplant for definitive post-chemotherapy female alopecia

Objectives: The aim of this study was to assess the efficacy and safety of hair transplantation in the treatment of endocrine-therapy induced alopecia.

Introduction: Alopecia is one of the most common adverse events caused by numerous anticancer agents. Although alopecia in cancer patients is not a life-threatening event, it has a significantly negative psychological and social impact. Although it has been well documented in chemotherapy and radiotherapy, the incidence of alopecia in patients treated with targeted therapies or endocrine therapy is not anecdotal. Some cases were reported assessing the efficacy of Minoxidil in endocrine-therapy induced alopecia. However, the long term efficacy in this indication is unknown.

Materials / method: Women with breast cancer, suffering from endocrine-therapy induced alopecia, have been grafted with “follicular unit long hair” technique. With this technique a strip of non-shaved hair is harvested from the donor area, segmented into follicular units under stereomicroscope and transplanted into the recipient area.
The efficacy on global hair coverage was assessed by both investigator and patient themselves. In addition, hair density, thickness and growth rate were assessed with a digitalized phototrichogram on the donor area (FotoFinder™).

Results: 10 patients, ranged from 42 to 72 years old, presented male pattern hair loss, emphasizing the hormonal role of endocrine therapy in this type of alopecia.
Hair coverage assessed either by the investigator or by the patient was improved by 50 to 70%.
The long term follow-up, up to 3 years, showed the lasting efficacy of hair transplantation.
Neither transplantation failure nor side effect, except frontal oedema during the 3-4 days after surgery, has been reported.

Conclusion: Endocrine-therapy induced alopecia is not rare. Its clinical pattern is similar to androgenetic alopecia. Hair transplantation has demonstrated its efficacy for the long term treatment of alopecia induced by endocrine therapy. Since no shaving is needed with “follicular unit long hair” technique, this is particularly recommended in women. The advantages of hair transplantation are the long-lasting effect, the poor side effects and the absence of compliance issue.
