Male , 56 yo, healthy , one year and 4 months post upper blepharoplasty

Dear colleagues,
I would like to ask for your opinion on a case that has been puzzling me.
This is a 56-year-old male patient whom I operated on in April last year for upper blepharoplasty. The post-operative course was uneventful, and he was initially satisfied with the result.
However, for the past 2–3 months, he has been complaining of a persistent “heaviness” in the upper eyelids. He reports that the sensation improves temporarily when he massages the upper eyelid and eyebrow area, gently lifting the brow and eyelid in the process.
He also mentions that his eyes are naturally small and his eyebrows have always been low-set, which adds to his frustration. People frequently ask him if he's tired, and this bothers him.
I’m attaching a short video so you can better observe the dynamic and what he does to relieve the sensation.
I was thinking in removing the excess skin he has, and do maybe something to lift the brows.
What would you suggest in this case? What options would you consider for improving both function and aesthetics?
