Abstract

Objectives: This study aims to share the authors' experience using a dual wavelength laser combined with PRP (LADD) for treating post-acne scars. Introduction: Post-acne scars present a long-standing challenge in dermatology and aesthetic medicine. However, recent technological advancements have provided various treatment options for improving both the appearance of acne scars and the overall skin texture and tone. Among these, laser treatments—especially fractional ablative and non-ablative lasers—have proven effective in scar reduction. When combined with Platelet-Rich Plasma (PRP), delivered through LADD via microchannels created by the laser, the treatment not only accelerates recovery but also enhances overall results. Materials / method: This retrospective study was conducted in two clinics in Albania, involving 38 patients with post-acne scarring. Patients were evaluated before treatment and 3 months after the final session. The treatment consisted of a hybrid approach combining fractional ablative CO2 laser and fractional non-ablative 1570 nm wavelength laser, immediately after the laser treatment, PRP was applied using Laser Assisted Drug Delivery (LADD). Scar improvement was assessed using two standardized qualitative tools: the Vancouver Scar Scale (VSS) and the Patient and Observer Scar Assessment Scale (POSAS). Results: Among the 38 patients, a significant reduction in both VSS and POSAS scores was observed, with a reduction of 71.9% ± 14.4% in VSS and 67.1% ± 8.4% in POSAS. The follow-up period ranged from 3 to 6 months, and no serious adverse events were reported. Conclusion: This study demonstrates that the combination of a hybrid fractional ablative and non-ablative laser approach with PRP delivered through LADD is a safe and effective modality for the treatment of post-acne scars. The results indicate significant improvement in scar appearance, high patient satisfaction, and a favorable safety profile.
