Abstract

Objectives: To comparatively evaluate the lifting efficacy, volumizing capacity, and skin quality improvement of high G′ HA fillers versus polycaprolactone in superficial facial lifting. Introduction: High G′ hyaluronic acid (HA) fillers provide immediate mechanical lifting through structural projection and tissue support. In contrast, polycaprolactone (PCL) acts as a biostimulatory agent, offering both initial volumization and progressive collagen remodeling. While both modalities are widely used for superficial and lateral facial lifting, their short- and long-term lifting profiles may differ significantly due to distinct mechanisms of action. Materials / method: Subject: Age 30-40,Age 40-50Follow up: 2-weeks, 1 month, 3 months.This prospective comparative study assesses patients treated with either high G′ HA filler or PCL for superficial facial lifting. Evaluations are performed at baseline, immediately post-treatment and during followup. Primary endpoints include lifting magnitude and vector analysis measured by 3-dimensional imaging and Global Aesthetic Improvement Scale (GAIS). Secondary endpoints include volumetric maintenance and dermalsubcutaneous thickness measured by highfrequency ultrasound, as well as as clinical assement of skin quality Results: Immediate lifting outcomes are expected to be comparable, with HA fillers potentially demonstrating stronger early projection due to higher elastic modulus. However, PCL is hypothesized to provide superior long-term durability of lift through progressive neocollagenesis, reflected by sustained elevation on 3D imaging, increased tissue thickness on ultrasound, and continued GAIS improvement. Conclusion: High G′ HA fillers offer effective mechanical elevation and may be preferable in cases requiring isolated lifting without the need for biostimulator tissue response or collagen induction. In contrast, PCL may provide a dual-phase lifting effect initial structural support followed by biologically driven tissue remodeling resulting in more sustained and multidimensional rejuvenation.
