Telehealth as a growth engine

Objectives: Attendees will be able to: (1) describe the current scale and growth of Japan's online self-pay aesthetic medicine market; (2) understand the regulatory framework, including the 2022 permanent legalization of initial online consultations and the 2025 Medical Care Act amendment; and (3) apply evidence-informed digital marketing strategies (content creation, channel selection, and patient-funnel design) to build a sustainable, digitally integrated aesthetic practice. Introduction: Japan's self-pay aesthetic medicine market reached approximately JPY 594 billion in 2023, growing 8.8% year-on-year. In April 2022, initial-visit online consultations were permanently legalized, removing a key barrier to telemedicine in self-pay practice. Japan's telemedicine market, valued at about USD 5.2 billion in 2024, is projected to reach USD 20 billion by 2033 (CAGR 16.2%). Digital-first patient acquisition has become a critical competency for aesthetic clinics, yet evidence-based guidance remains scarce. Materials / method: This presentation synthesizes government statistics (Ministry of Health, Labour and Welfare), commercial market research, and a structured narrative review of marketing practices in Japan's online aesthetic medicine ecosystem. The regulatory analysis covers the 2022 permanent legalization of initial online consultations and the 2025 Medical Care Act amendment, which codifies telemedicine as a recognized medical act and introduces safety-reporting obligations for aesthetic clinics. Results: Four findings emerged. (1) Same-day online prescription of topical agents (e.g., tretinoin, tranexamic acid, hydroquinone) is a primary entry-point service, with demand concentrated among patients in their 20s-30s. (2) Short-form video on social media platforms outperforms traditional digital advertising in reach and conversion. (3) Physician-authored educational content and transparent, publicly listed pricing are the strongest drivers of patient trust. (4) Only about 3.5% of patients report having used online consultation, indicating substantial unmet potential. Conclusion: Japan's online self-pay aesthetic medicine sector is among Asia's most rapidly evolving healthcare business models. The convergence of favorable regulation, a digitally active younger demographic, and scalable prescription-based services creates a distinctive, replicable framework. As comparable regulatory liberalization advances across China and Southeast Asia, the Japanese experience offers actionable strategic lessons for practitioners building sustainable, digitally integrated aesthetic practices.
