Challenging Case of PIH Following Vbeam Treatment in Skin Type IV

Patient description
24-year-old male patient, Fitzpatrick skin type IV, with a capillary vascular malformation
Case description
He previously underwent Vbeam laser treatments at another clinic, which resulted in a greyish discoloration followed by crusting (I suspect the settings were too aggressive). As a result, he developed post-inflammatory hyperpigmentation (PIH) that has proven very difficult to treat.
A photo is attached, showing a clear contrast between:
An untreated area, where more visible vessels are present
A treated area, where fewer vessels are visible but there is prominent PIH, which has not improved despite several treatments
Treatments attempted for PIH:
Topical hydroquinone
Picoway laser
RedTouch laser – 8 sessions completed, with no significant improvement in pigmentation
