Novel minimally invasive subcision technique for acne scars

Objectives: This case series highlight a novel minimally invasive method to treat acne scar using a combination of blunt cannula and hyaluronic acid fillers.

Introduction: Acne scar is a permanent disfiguring condition resulting from severe inflammation and injury caused by acne vulgaris. One of the procedures used to treat acne scar is subcision. The principle is to break the fibrotic attachments that tether the scar to the underlying subcutaneous tissue. A typical subcision requires the use of a Nokor needle with sharp, blade-like tip inserted adjacent to each scar with the bevel facing upwards parallel to the skin surface. This needle will then be moved side-to-side in a fan-like motion. Although effective, this method is highly traumatic.

Materials / method: 20 patients (12 males and 8 females) presented with multiple atrophic acne scars mainly on bilateral cheeks. All patients reported previous history of severe acne vulgaris with manipulation of lesions. None of the patients have had any prior treatment for acne scars.
Clinical examination on all patients showed multiple different types of atrophic scars on bilateral cheeks.
Each patient underwent subcision by using a blunt cannula inserted into the subdermal area followed by injection of hyaluronic acid fillers. Three monthly sessions were performed with a follow up three months later.

Results: After three sessions, 15 patients (nine males and six females) reported very significant improvement. The other five patients (three males and two females) reported significant improvement. All patients reported tolerable mild post-procedural pain. There were no complications reported.

Conclusion: This novel minimally invasive subcision technique to treat acne scars using a combination of blunt cannula and hyaluronic acid may be a safer, less traumatic yet effective alternative to regular subcision.

Technique of tissue reset by fiber folding and its clinical application

Objectives: To discuss the technique of tissue reset by fiber folding with thread embedded and its clinical effect on facial rejuvenation.This method achieves the reduction of displaced tissues by shortening the fibers between the loosened tissues. Its advantage is to replace "line" with "surface" in compact and elevated. Its effect maintained not only by the biting force of wire sawtooth to tissues, but also by the winding force between wire and tissues and the ability of collagen tissue and tissue repair produced between the winding tissues. Therefore, it is more durable and has fewer complications.

Introduction: Folding does not depend excessively on anchor points,but replaces linear pull with the resilience of the dislocation plane.
Because of the reduction of the dislocation plane,the dislocation heals more sufficiently,the curative effect will maintain longer, and the complications are fewer.

Materials / method: From June 2016 to June 2018, 500 cases with thread embedded by fiber folding for facial rejuvenation were observed. According to the characteristics of facial aging of beauty seekers, thread embedded by fiber folding were undergone and the changes of nasolabial sulcus, oral bursa, oral and mandibular sulcus and mandibular margin after operation were observed, and immediate postoperative satisfaction of cases were calculated.

Results: In the 50 cases group, 490 cases (98.0%) of nasolabial sulcus were obviously shallower, and there were 450 cases (90.0%) with disappearance of oral capsule bag, 460 cases (92.0%) with shallow oral and mandibular sulcus, 480 cases (96.0%) with clear mandibular margin. Then 470 cases (94%) were satisfied with immediate postoperative satisfaction.

Conclusion: The technique of tissue reset by fiber folding with thread embedded for facial rejuvenation could effectively improve the aging characteristics of nasolabial sulcus, oral bursa, oral and mandibular sulcus and mandibular margin. It has the advantages of good facial rejuvenation and high satisfaction, and is worthy of clinical application and popularization.

The possibility of botulinum toxin treatment for jawline contouring

Objectives: To describe the possibility of botulinum toxin treatment for Jawline contouring

Introduction: It is broadly accepted to use botulinum toxin for typical aging signs on frontalis, glabellar, orbicularis oculi and massetor muscle.
Also variety of the usase of botulinum toxin are reported these days.
The experience of the treatment for depressor anguli oris and platisma lead us understand that we can extend the usase of botulinum toxin for jawline contouring.

Materials / method: We used 20 units of type A botulinum toxin for depressor anguli oli and 50 units for platisma.
20 patients are treated.

Results: We found the improvement of the corner of the mouth position.
And we see the improvement of jawline especially in the movement of smiling.
80% patients repeated the same treatment.

Conclusion: This treatment have a potential
to prevent typical aging of perioral area and jawline.
This experience help us understand the importance of the knowledge of anatomy and diagnosis for each parents' movement.

The MUST LIGHT thread lift with non-surgical atraumatic undermining in jawline and neck contouring

Objectives: In contrast to traditional surgical SMAS and platysma undermining the MUST LIGHT approach makes direct dissection of the soft tissues and subsequent suturing unnecessary, because it allows to perform by using a special plane-tip blunt cannula through several bilateral skin punctures from the each side. Moreover, the area of subcutaneous undermining might be wider than in a surgical face and neck lift, in case when it is required.

Introduction: In contrast to traditional surgical SMAS and platysma undermining the MUST LIGHT approach makes direct dissection of the soft tissues and subsequent suturing unnecessary, because it allows to perform by using a special plane-tip blunt cannula through several bilateral skin punctures from the each side. Moreover, the area of subcutaneous undermining might be wider than in a surgical face and neck lift, in case when it is required.

Materials / method: Threads are anchored on the Lori’s fascia opposite the tragus by engaging the fascia with a curved surgical needle and pulling threads through to their central barbless part. Thread placement with the help of blunt guide needles is simple and straight, the thread barbs ensure reliable and safe soft tissue fixation and support. Implantation of each thread ensures simultaneous use of two different implantation planes: the lateral SMAS plication and the medial subcutaneous placement. Results: Having obtained good results with the MUST LIFT approach (Minimally invasive Undermining with Suspension using Threads) in rejuvenating face surgeries, I decided to adapt this technique into cosmetologically practice for using in less invasive way. The MUST LIGHT procedure benefits significantly from the MUST LIFT surgical treatment in terms of protocol simplification and invasiveness reduction.

Conclusion: So the MUST LIGHT technique becomes much safer, simpler to perform and substantially less invasive; the procedure requires only and local anesthesia or light sedation, the rehabilitation period is shorter and less morbid for patients.

Smoothing the lid-cheek junction: personal technique

Objectives: The audience will learn the technical nuances of creating a smooth, impressive result using hyaluronic acid whilst avoiding complications such as irregularities and bumps. More difficult cases will be reviewed including those patients who would also be candidates for lower lid blepharoplasty.

Introduction: A smooth lid-cheek junction is a hallmark of youth. With aging, midface volume loss and herniation of orbital fat against an attenuated orbital septum results in a prominent shadow at the medial and lateral palpebromalar groove. Correcting this with non-surgical manoeuvres using hyaluronic acid fillers is an important skill in aesthetic practice. Certain key considerations are important in this unforgiving area to avoid an unfavourable result.

Materials / method: Periorbital anatomy is discussed in order to appreciate the changes that occur with normal aging. Various images of patients are presented including a commentary on treatment options for each case. Videos are shown to portray changes that occur with movement. The technique of effacing the lid-cheek junction medially and laterally is shown.

Results: Results of treatment using advanced placement of non-permanent fillers are presented with clear images of real patient results. Younger and older patients are shown and discussed, illustrating the range of patients and indications using the technique of infraorbital rejuvenation.

Conclusion: Smoothing the lid-cheek junction is a powerful technique that be reproduced if certain key factors are considered. The technique provides rejuvenation in young and older patients, obviating the need for blepharoplasty in some cases.
