My experience with growth factors in hair regeneration and rejuvenation: let’s think beyond PRP

Objectives: 1. To diagnose different types of alopecia and other hair conditions

Introduction: Hair disorders are amongst the most prevalent and distressing conditions affecting both males and females worldwide. The most common conditions include Male pattern hairloss (MPHL) or Androgenetic alopecia (AGA) affecting both men and women, Female pattern hairloss (FPHL), Alopecia areata, and Cicatricial alopecias. Dandruff and Premature Greying of hair also impacts the aesthetic value. Apart from medical management, Platelet rich plasma (PRP) has been used extensively but not successfully in hair loss treatment. Growth factor therapy has been a promising new intervention superior to PRP.

Materials / method: Patients aged 18 years and above, both males and females, with different hair disorders are assessed and diagnosed. After a detailed history and examination, patients are advised PRP therapy for the initial 3 sessions at an interval of 3-4 weeks. They are then called for further sessions alternating PRP therapy with Growth factor therapy. The results are evaluated after a total of 6-8 sessions. The patients were assessed for a period of one year.

Results: Patients showed a faster and superior response in hair regeneration when conventional PRP therapy was combined with the newer, innovative Growth factor cocktail. The quality of the scalp also improved with decreased dandruff, and controlled premature greying. 

Conclusion: PRP treatment alone is not successful in most hair loss conditions due to lack of standardised preparation methods and dosing protocols which are required but are confusing. Growth factor therapy is a much simpler, yet a powerful promising treatment that ultimately results in hair growth, hair thickening, scalp rejuvenation and control of premature greying when performed alone or in combination with PRP.

New near-infrared light emitting diode (nNIR LED) lighting improves skin and promotes hair growth

Objectives: Wrinkled skin and hair loss are external reflections of morphological and physiological aging processes. Phototherapy using lasers and light emitting diodes (LEDs) is a non-invasive procedure that uses light energy to regenerate and repair aged skin. However, it has limitations owing to their dot-shaped emission characteristics and narrow spectral bandwidth. Because the action spectrum on tissue regeneration consists of more than one wavelength, applying a spectrum covering a wider spectral region may be advantageous.

Introduction: In this study, we developed a new near-infrared LED (nNIR LED) with a broader spectrum than the existing NIR chip LED lighting and applied it to human skin cells and animal models to investigate its effects on skin improvement and hair growth. We investigated the effects of nNIR LEDs, compared to those of white LEDs, on cytotoxicity, adenosine triphosphate (ATP) synthesis, and reactive oxygen species (ROS) reduction in human skin cells. We also evaluated changes in skin thickness, collagen synthesis, and collagenase expression in a UV-induced photoaging mouse model.

Materials / method: Additionally, the effect of NIR LED irradiation on hair follicle growth was observed by analyzing skin thickness, anagen induction, and follicle number.

Results: Additionally, the effect of NIR LED irradiation on hair follicle growth was observed by analyzing skin thickness, anagen induction, and follicle number. nNIR and white LED did not show cytotoxicity in human skin cells, increased ATP and collagen synthesis, and decreased ROS levels. In a UV-induced photoaging mouse model, irradiation with nNIR LED and white LED decreased skin wrinkles and skin thickness, increased collagen synthesis, and downregulated collagenase expression. Moreover, these promoted hair growth, skin thickness, and hair follicle number.

Conclusion: Our study findings could be used to develop novel therapy for skin and hair growth improvement using LED light.

Female Pattern Hair Loss: a clinical and pathophysiological review

The studies detailed identify methods of diagnosis, the benefits and pitfalls of treatments , and the objective results.

Investigating the Safety and Efficacy of Platelet-Rich Plasma (PRP) Treatment for Female Androgenetic Alopecia: Review of the Literature

The data from the review indicates high levels of satisfaction, better responses in terms of hair growth and thickness with use of PRP compared to placebo or topical minoxidil.

In our lab we use calcium carbonate or calcium chloride to activate the PRP. Thrombin is effective at releasing growth factors, but the tissues may not be receptive to too many G Factors at a time

Conclusion: - Tried and tested treatment for hair
- Technology re: hair restoration is advancing
- Future avenues include more concentrated solutions, use of stem cells
- Cloned hair follicles? Research from Japan looking promising
- Improving hair strength and appearance may be the No.1 most effective intervention for our female clients in aesthetics

PRP for treatment of pattern hair loss: Which clinical effects can we expect and how satisfied are male/female patients and clinicians?

Objectives: 1: Diagnose pattern hair loss.
2: Consider PRP as potential treatement option for both genders.
3: Formulate a tailored approach to each individual including 3-4 injections of PRP.
4: Ensure patient compliance, analyze clinical effects and patient satisfaction by using a short questionnaire and taking global photographs.

Introduction: Hair loss is a widespread condition in both genders. Over the past decade PRP has become a common treatment for hair loss. Our goal was to analyze patient and clinician satisfaction and to investigate all potential clinical effects of PRP on male and female pattern hair loss in a simple and practical way instead of counting the number of hairs per cm², which requires advanced technologies such as trichoscopic imaging and is usually not available daily practice.

Materials / method: Over a period of 12 months, we treated a total of 56 patients for androgenetic alopecia with PRP. 49 patients were treated with a series of three injections at monthly intervals, 3 patients with a series of four injections, 2 patients with a series of 5 injections, 1 patient was treated with a series of 7 injections, and 1 patient with a series of 8 injections. Follow-ups were conducted 1 month after the last treatment. Global photographs were taken and a self-drawn questionnaire was used to assess the satisfaction and clinical results from the patient's and the clinician's perspectives.

Results: 32 patients were male and 24 female. The average patient satisfaction score was 7.29 (scale: 0-10). The clinician's rating was similar (6.46). The recommendation rate was high (8.0). Female patients' ratings were slightly but not significantly better. The probablity of occurence of clinical effects was reported as follows: improvement in hair density: patient‘s rating: 64%; clinician's rating: 46%, thickness: 38%; 45%, quality: 46%; 54%, sheen/lustre: 27%; 21%, new hair growth: 57%; 68%, less hair loss: 48%; 20%, other positive effects: 5%, 2%, no effects: 4%; 4%, negative effects: 0%; 0%.

Conclusion: Using a short questionnaire in daily practice is easy and may be also helpful to increase compliance and satisfaction. Our study revealed encouraging results for the treatment of male and female pattern hair loss with PRP. In 96% of all patients positive clinical effects were found. The autologous treatment was rated with high satisfaction scores and can be considered a safe and effective treatment modality for both genders. As patient satisfaction scores are subjective to some extent, we added clinician satisfaction scores and found out that they were consistent with the patients' scores.

Evaluation of efficacy of derma roller sizes vs topical application for administration of a patented hair regrowth formulation

Objectives: 1) To evaluate the efficacy of the derma roller technique for the administration of a patented hair regrowth solution in male and female patients with androgenetic alopecia
2) To compare the 1.5- and 0.5-mm derma roller groups to the topical application group.

Introduction: Micro-needling with derma roller has emerged as a recent management strategy in androgenetic alopecia (AGA) cases. It is a method that creates transdermal microchannels across the stratum corneum barrier layer of skin to increase the skin permeability of small molecules, drugs, proteins, etc. Dermaroller is an effective solution to the difficulty faced in providing injectable treatment therapy when there is a lack of trained personnel, time boundaries and geographical boundaries. We need to assess its application to non-surgical hair restorations such as a patented hair regrowth therapy.

Materials / method: Male patients with Norwood Hamilton grades II-IV and female patients with Ludwig's types I-III were included. Patients were randomly divided into three groups of 25 patients each (Group A—1.5-mm derma roller group, Group B—0.5-mm derma roller group, and Group C—topical application group). Hair pull test and global photographic assessment were used for evaluation.

Results: Reduction in hair fall during hair pull test was noted in the patients of all three groups by the end of 6 months, whereas the hair fall was reduced in just 70% in Group B compared with 60% of participants in Group C. Marked improvement was seen in Group A (mean-7), whereas the mean score in Group B was 5.5 at 6 months. Group C showed a mean score of 6.5 at baseline, which improved to 7.2 at year 1.

Conclusion: According to this study, a depth of 1.5 mm of microneedle tended to be more effective than that of 0.5 mm and the topical application in terms of improving terminal hair count and hair thickness. However, there is not much difference observed between the val- ues. Group B showed improvement in vellus hair count as compared to Group A.
