Objectives: The concept of Fractional Photothermolysis (FP) was published way in 2004 and till now it is still widely used in our modern lasers. In fact, the concept has been used in other energy based devices such as radiofrequency-based device. I'll discuss about the historical background and advancements in Ablative Fractional Laser (AFL), Non Ablative Fractional Laser (NAFL) and Hybrid lasers. Additionally, it will discuss strategies for optimizing outcomes for patients undergoing skin rejuvenation treatments.

Objectives: How to get optimal result by using SUPERB technology?

Introduction: Recently, treatment for rejuvenation by using various kinds of energy-based devices have been performed in our esthetic medical field. The mechanism of almost devices is giving thermal damage to tissue and regenerate tissue by collagen synthesis.
A new generation ultrasound device has been developed to resolve these kinds of issues. Intense ultrasound beam technology, synchronous ultrasound parallel beam (SUPERB ) have come into our aesthetic medical field.

Materials / method: The high frequency, parallel beams enable the majority of the thermal effect to remain localized between 0.5 and 2mm within the dermis, with the center of the treatment effect at a depth of 1.5mm, while avoiding injury to deeper anatomic structures. As the parallel beams propagate the tissue, an array of volumetric cylindrical shaped thermal zones are created. Since all transducers operate simultaneously, relatively high energy is delivered at once into the mid-dermis, causing the tissue temperature to increase to 60-70˚C, leading to inflammatory response and eventually to collagen remodeling.

Conclusion: Importantly, the elongated thermal zones lie in parallel to the direction of the collagen
fibers. Collagen contraction creates vector lines of tightening along the direction of facial
lines and wrinkles leading to restoration of the natural skin appearance. Understanding about this vector line is important to get optimal result.
Nowadays, we doctors have been focusing filler injection treatment. However, to get optimal result, we should treat both atrophy and sagging. By using SUPERB technology, we can resolve a solution for over-filled face or unnatural appearance.

Objectives: Determine the efficiency of using the combination of yellow (589nm) and infrared (1319nm) lasers for the improvement of skin roughness and thickness.

Introduction: Infrared and yellow laser treatments for improving skin tone and rejuvenation have become increasingly popular due to short downtime and fewer side effects compared to ablative treatments.

Materials / method: The cheek area of 15 patients was treated for 3 sessions, once a month using 2 passes of infrared laser (1319nm) with 10x10mm square spot, 20ms pulse duration and 30J. In the same treatment session the cheek area was treated with 2 passes of yellow laser (589nm), 10x10mm square spot, 20ms pulse duration and 25J. Measurements of skin roughness and maximum skin depth were collected using an imaging system and echography analysis prior to treatment and 4 weeks after completion of the last treatment session.

Results: All 15 patients showed significant improvement in all measured parameters (P<0.001) with no significant side effects.

Conclusion: Combination of monthly treatments with a yellow (589nm) and infrared (1319nm) lasers are safe and effective.

Objectives: To offer a literature review on the short term and long term effects of fillers.
To examine the possibility that fillers genuinely produce neocollagenesis that gives a long lasting rejuvenation effect on the skin texture and appearance
To examine side effects and their frequency and severity following injections with different fillers
Specifically compare different fillers and enumerate the benefits and/or side effects they present

Introduction: For many patients, injectable filling agents offer the promise of facial rejuvenation while offering reduced risks compared with more invasive procedures. While hyaluronic acid fillers are generally accepted as ones that fill in a space, recent clinical observation has documented a persistence of the filling effect that is longer than the biological availability of the filler. On the basis of this clinical observation, it has been assumed that fillers may have a biostimulatory effect on tissue by neocollagenesis.

Materials / method: A literature review of dermal filles from 1995 to present reveals that different dermal fillers depend on the procedural technique, and the agent injected. All dermal fillers have the potential to cause complications as a result of volume and technique, product formulation and concentration, and insufficient knowledge of facial anatomy. The majority of adverse reactions are mild and transient, such as bruising and trauma-related edema. Serious adverse events are rare but present.

Results: One of the underreported side effects of cosmetic injections is vascular occlusion. This condition can be identified by the pain on the injection spot, as well as in areas distant from the injection site. Blanching, white, dusky or pale skin may also be signs of a reduction in blood supply as a result of the cosmetic injection. Other side effects of fillers include allergic reactions, erythema, infections, chronic inflammation and granulomas. Rare side effects include intense necrosis and Keratoacanthoma-like reactions after hyaluronic acid injections

Conclusion: When evaluating anti-aging skin rejuvenation it is important to understand the exact role that Collagen and Elastin are playing in delaying aging, These two proteins are vital for skin firmness and texture but, from a molecular biology perspective, they cannot give an overall youthful look. Youthfulness depends on the interactions and signalling communications of around 100,000 proteins, as well as in the consistent reduction of toxicity, free radical formation and inflammation.
