Objectives: The objective of this study is to evaluate the efficacy, safety, and patient satisfaction of using a dual-wavelength laser system that combines the ablative power of CO2 lasers (10,600 nm) with the deep-penetrating, non-ablative effects of 1570 nm lasers. Specifically, the study aims to determine whether this dual-wavelength approach provides superior improvements in skin texture, reduces pore size, and diminishes wrinkles, particularly in patients with Fitzpatrick skin types III–V, while minimizing downtime and adverse effects.
Introduction: Fractional CO2 lasers have long been the gold standard for skin rejuvenation due to their powerful ablative results, while non-ablative 1570 nm lasers are valued for their gentler, safer approach and minimal downtime. Combining the ablative precision of CO2 (10,600 nm) with the deep-penetrative, non-ablative effects of 1570 nm lasers, a dual-wavelength system offers a promising solution for improving skin texture, reducing pore size, and diminishing wrinkles. This study evaluates its efficacy, safety, and patient satisfaction, particularly for Fitzpatrick skin types III–V.
Materials / method: This clinical trial (October 2023–October 2024) enrolled 25 individuals aged 30+ (mean age: 43.6 ± 15.7) with mild-to-moderate facial aging (Glogau I–II). Participants underwent three dual-wavelength laser treatments at three-week intervals. CO2 laser power was set at 15–25 W with 0.6–1 ms pulses, and 1570 nm laser power at 8–12 W with 3–4 ms pulses. Outcomes were assessed via VISIA Imaging System and scoring systems such as MFWS, GAIS, and sGAIS, with pain measured by VAS. Adverse events were documented, and results were evaluated by five blinded dermatologists.
Results: The results highlighted the superior potential of dual-wavelength treatment over single-wavelength systems. Patients showed significant improvements across all parameters, with wrinkles reduced by an average of 0.33 points on the MFWS (p < 0.001), and 92% reporting noticeable improvements. VISIA analysis confirmed enhanced skin texture and pore refinement (p = 0.001). High satisfaction was reflected in GAIS (2.4) and sGAIS (2.08) scores. Pain was mild (VAS 3.12), with 76% resuming activities within seven days and only 4% needing over two weeks for recovery.
Conclusion: The dual-wavelength laser system marks a breakthrough in skin rejuvenation by combining the ablative precision of CO2 with the non-ablative depth of the 1570 nm laser. This approach allows for the safe delivery of more energy, enabling superior results even in Asian skin, which has traditionally posed challenges in laser treatments. By improving wrinkles, skin texture, and pore size with minimal downtime, this system is an effective and versatile solution across diverse patient populations
