Toward Optimal Injection Volume in Autologous Fat Grafting: A Volumetric Strategy for Lean Asian Women

Objectives: To determine the optimal injection volume in autologous breast fat grafting by analyzing the relationship between injected volume and 3-month volume gain, and to identify efficient injection-to-capacity ratios for maximizing outcomes while minimizing fat waste.

Introduction: In lean Asian women, limited donor fat and small breast capacity complicate volume planning in fat grafting. Overharvesting leads to unnecessary fat waste, while overinjection may cause fat necrosis, nodules, or distortion due to tissue pressure overload. Despite its clinical importance, objective guidelines for optimal injection volume remain lacking, particularly in low-BMI patients. We aimed to model efficiency and safety using 3D volumetry.

Materials / method: Twelve female patients undergoing fat grafting (mostly with the Condense method) were analyzed using 3D volumetric imaging pre-op and at 3 months post-op. Volume efficiency was defined as:
(3M volume − pre-op volume) ÷ injected volume.
Injection ratio was defined as:
injected volume ÷ pre-op breast volume.
Quadratic regression and stratified analysis were used to identify optimal injection ranges.

Results: Efficiency peaked at an injection ratio of ~1.3. Ratios beyond 1.5 achieved greater volume gain but at diminishing efficiency. No benefit was observed above 1.8. In patients with BMI 18.5–21 and pre-op volume <150 ml, injecting 240–280 ml per side balanced high efficiency and significant volume gain without waste.

Conclusion: Modeling the injection-to-capacity ratio enables a balanced approach to maximize volume gain while minimizing risks associated with overinjection, such as fat necrosis or nodularity. In lean Asian patients, a ratio around 1.3 offers high efficiency, while 1.5 serves as a safe upper threshold.
While the sample size was limited, these findings offer a foundation for future studies to refine personalized grafting strategies.

Cultural and Somatic Influences on Breast Implant Preferences in the APAC Region: A Comparative Observational Study

Objectives: To explore how cultural norms and somatic traits influence preferences for breast implant size and surgical technique across APAC countries. This study aims to inform surgeons about culturally aligned treatment planning that respects diverse aesthetic ideals beyond anatomy alone.

Introduction: Implant choices are shaped not only by physical proportions but also by cultural perceptions of beauty, modesty, and identity. The APAC region presents a unique diversity where breast aesthetics vary dramatically by country. This study evaluates how body data and cultural values correlate with national preferences in implant volume and procedure.

Materials / method: We analyzed >10,000 implant sales records from GCAesthetics across 7 countries. Distribution patterns were compared against average height, BMI, chest width, and bra size (WHO/OECD data). Preferences for implant vs. fat grafting vs. fillers were estimated from clinical and regional sources to contextualize patient decision-making.

Results: Japan and Korea favored smaller sizes (250–300cc) with natural results; fat grafting is popular. China averaged around 300cc. Taiwan, India, and Thailand preferred larger sizes (≥350cc). Australia selected 300–400cc for balance. Technique preference and size reflect each culture’s concept of femininity, proportion, and visual impact.

Conclusion: Breast augmentation trends across APAC are not dictated by anatomy alone. Cultural values deeply influence surgical goals. Recognizing these patterns allows surgeons to plan procedures that align with patient identity, achieving satisfaction through both physical harmony and cultural relevance.

A cross cultural study of Western versus Asian patient preferences for ideal breasts and Inter-racial trends of Breast Augmentation via Fat Grafting

Objectives: Objective 1: To study differential standards of inter-racial breast beauty preferences pertaining to Western and Asian women.
Objective 2: We aim to study the cross cultural variance existing in techniques for Autologous fat grafting for breast augmentation by a comprehensive overview and recapitulation of literature available.

Introduction: Breast augmentation is most frequently performed aesthetic surgery procedure. Fat grafting is a common technique to augment breasts that is practiced by plastic surgeons worldwide for volume enhancement and contour correction. Breast beauty is a dynamic entity that is subjective and ever changing through generations. Little knowledge is available if breast size is a cue to Darwinian fitness or its modules. Evolutionarily larger breasts are considered more attractive, possessing better reproductive health and nurturing ability. However, perception of ideal breasts is evolving with changing time

Materials / method: Online search engines comprising of The PubMed, Google Scholar, Cochrane Central Register of Controlled Trials and clinical key/Elsevier, were searched for relevant studies from January of 2019 to March of 2025. Articles describing ideal female breasts and Autologous Fat Grafting for Breast Augmentation were selected based on pre-determined inclusion and exclusion criteria. A total of six and five studies respectively were included after removing the duplicates. Inclusion criteria included females aged 18-60 years and follow up of 1 year. Females with co-morbidities were excluded.

Results: In maximum studies, donor area was abdomen followed by thigh then buttocks. Manual fat aspiration was preferred using 2-4mm cannula attached to 10 cc syringe. Centrifugation included 3000 rates per minute for 4-5 minutes but 1 study did 1200g for 3 minutes. Most common complications include induration, persistent pain and hematoma in maximum studies. While Caucasian preferred skyward pointing nipples with concave upper pole, tight lower pole convexity, Asians preferred front nipple position, straight upper pole and half hemispherical lower pole.

Conclusion: Defining an ideal female breast size is the most fundamental part of surgery. With evolving trends, cross culturally women desire natural looking youthful breasts that are in balance with the body proportions. However, the perception of breast beauty by men continues to remain inclined towards larger breasts. Autologous fat grafting is a promising method that provides satisfactory volume retention and long lasting results. Future studies should throw light on cancer occurrence and detection to further substantiate Autologous fat grafting safety.

Body Contouring Re-defined: PURE (Power-Assisted, Ultrasound-Assisted, and Radiofrequency-Enhanced) Sculpting for Safe and Consistent Results

Objectives: 1. Understand the fundamentals of PURE technologies and their application to provide the best aesthetic outcomes

Introduction: Body contouring has evolved with technological advancements, improving fat removal and skin tightening. Traditional methods like Suction-Assisted Liposuction (SAL) have limitations, leading to innovations such as Power-Assisted Liposuction (PAL), Ultrasound-Assisted Liposuction (UAL), and Radiofrequency (RF)-Enhanced Contouring. This study evaluates the efficacy of PURE Contouring, a novel approach integrating these three techniques.

Materials / method: A retrospective analysis was performed on 1,968 patients who underwent PURE Contouring from September 2021 to December 2024 at a private hospital. After a thorough evaluation, they were subjected to anaesthesia. The procedure included tumescent infiltration followed by UAL initially for fat emulsification and eventually PAL for efficient fat removal using the Separation Aspiration Fat Equalization (SAFE) technique. Finally, RF was done to enhance skin tightening. Patient demographics, procedure details, and satisfaction levels were evaluated over six months, and statistical analysis conducted.

Results: Most patients were female (53.20%), and 360-degree liposuction was the most commonly performed procedure (38.61%). Most patients were 35–45 years old (36.03%). Satisfaction level was measured on a 5 point Likert scale. Overall, 91.51% of patients reported being highly satisfied with their outcomes. This strong positive response suggests that most patients achieved their desired aesthetic results and had a smooth surgical and recovery experience.

Conclusion: PURE Contouring effectively enhances body contouring outcomes, offering superior fat removal and skin tightening. High satisfaction rates support its efficacy, highlighting its potential as a preferred technique in aesthetic surgery.
