Introduction: Clean beauty products dominate the market, offering promises of safety and purity. However, their claims often lack robust scientific validation. This research examines the potential risks of ingredients flagged by the clean beauty movement, addressing concerns over allergens, hormonal disruptors, and carcinogens, to uncover the reality behind the marketing.

Materials / method: This study investigates clean beauty ingredient safety through a comprehensive literature review. Key databases such as PubMed, MEDLINE, and Embase were systematically searched using targeted Medical Subject Headings (MeSH) and keywords. Peer-reviewed articles, clinical trials, and reviews were meticulously selected for their relevance and recent publication dates. Data extraction and synthesis focused on evaluating the scientific evidence behind commonly scrutinized ingredients, ensuring a rigorous analysis of their safety profiles.

Results: Irritants: MI, fragrance, and formaldehyde are confirmed allergens linked to dermatitis, validating their exclusion in clean beauty.
Endocrine Disruptors: The link between chemicals like Triclosan and hormonal disruption remains uncertain, with most studies conducted on animals.
Carcinogens: Formaldehyde and 1,4-dioxane are associated with cancer risk, though evidence is still emerging.
Overall: Clean beauty promotes scrutiny of ingredients, but more research is needed to fully validate the safety of many claims.

Conclusion: The clean beauty movement has highlighted the importance of scrutinizing cosmetic ingredients, revealing that some commonly avoided chemicals, like MI, fragrance, and formaldehyde, are linked to known health issues. However, the safety of many other "clean" ingredients remains inconclusive, with limited human studies. While eliminating certain harmful ingredients is prudent, more research is needed to fully understand the effects of many "clean" cosmetics components on human health.

Results: Exosomes in dermatology show promise for skin regeneration, wound healing, and managing inflammatory and autoimmune conditions. They enhance scar treatment, promote hair growth, and aid in wound recovery. Derived from sources like stem cells and dermal papillae, they improve skin conditions and reduce scarring. However, challenges in isolation standardization and long-term safety need addressing. Despite these hurdles, ongoing research suggests that exosomes could revolutionize dermatological treatments with their unique therapeutic capabilities.

Conclusion: Exosomes are revolutionizing dermatology, bridging the gap between potential and application. They play a significant role in skin regeneration, wound healing, and treating inflammatory and autoimmune skin diseases. Stem cell-derived exosomes offer innovative solutions with reduced risk compared to direct stem cell therapies. However, challenges in purification, standardization, and regulation remain. Future research must address these issues to fully realize exosomes' therapeutic potential and ensure their safe clinical use.

Dihydrotestosterone (DHT) drives follicular miniaturization, with links to cardiovascular risks in men and polycystic ovarian syndrome (PCOS) in women. Despite its ubiquity, therapeutic advancements have lagged, with limited FDA-approved options. This presentation examines the evolving therapeutic paradigm, spotlighting innovative and synergistic approaches to elevate clinical outcomes in AGA management.

Materials / method: A thorough review of scientific literature was conducted across databases including Medline, PubMed, Embase, and EBSCO, supplemented by authoritative dermatology texts. The findings were further enriched by the clinical experience of seasoned practitioners and key insights into AGA management trends. This comprehensive analysis underpins the strategies detailed in this study.

Results: - FDA-Approved Essentials: Minoxidil and finasteride lead AGA management, complemented by the LaserMax HairComb as the sole FDA-approved device.
- Innovative Frontiers: Emerging therapies like LLLT, microneedling, PRP, oral and topical finasteride, spironolactone, botulinum toxin, and stem cell treatments offer transformative potential.

Efficacy and Safety of Bimekizumab in Moderate to Severe Hidradenitis Suppurativa: A Systematic Review and Meta Analysis of RCTs

Objectives: This systematic review and meta-analysis aimed to evaluate the efficacy and safety of bimekizumab, a monoclonal antibody selectively inhibiting both IL-17A and IL-17F, in patients with moderate-to-severe HS based on evidence from randomized controlled trials (RCTs).

Introduction: Bimekizumab represents a novel therapeutic approach targeting key cytokines implicated in HS. Following the completion of recent pivotal phase 3 clinical trials, a comprehensive synthesis of the evidence is needed to understand its clinical value in HS management.

Materials / method: Following PRISMA guidelines (PROSPERO: CRD420251025763), we systematically searched multiple databases for RCTs comparing bimekizumab vs. placebo in moderate-to-severe HS. Two reviewers independently extracted data and assessed risk of bias (RoB 2). Primary outcome was HiSCR50 achievement; key secondary outcomes were pain reduction (HSSDD) and serious adverse events (SAEs). Data were pooled using random-effects models to calculate Risk Ratios (RRs) and 95% CIs.

Results: Three RCTs (N=1220) met inclusion criteria. Compared to placebo, bimekizumab significantly increased HiSCR50 achievement (RR=1.64, 95% CI [1.37, 1.96], P<0.00001) and meaningful pain reduction (RR=2.21, 95% CI [1.48, 3.29], P<0.0001), with no heterogeneity (I²=0%). No significant difference was found in SAE risk (RR=1.82, 95% CI [0.58, 5.72], P=0.31), though CIs were wide due to low event counts. All studies had a low risk of bias.

Conclusion: Based on this meta-analysis of high-quality RCTs, bimekizumab demonstrates significant efficacy in improving clinical response (HiSCR50) and reducing pain in patients with moderate-to-severe HS compared to placebo. The short-term safety profile appears acceptable, though further data are needed for rare events. Bimekizumab is a promising therapeutic advancement for HS.

Investigating the Skin Wound Healing Potential of Biodegradable Polymers PLLA and PDLLA in Zebrafish

Objectives: Investigating the efficacy of biodegradable polymers, PLLA and PDLLA, in promoting skin wound healing in zebrafish, while understanding their mechanisms of action at the cellular and molecular levels.

Introduction: The field of wound healing has seen significant advancements with the introduction of biodegradable polymers, particularly in addressing the complex process of skin repair, which is crucial for maintaining the body’s integrity and protection. Skin serves as the largest organ, playing an essential role in barrier function, temperature regulation, and defense against pathogens. Therefore, studying skin wound healing is critical for developing treatments, especially for chronic or non-healing wounds.

Materials / method: Using a laser ablation method, which provides precision and reproducibility, skin wounds are created in zebrafish, and treated with these polymers. The healing process is monitored through advanced imaging techniques to capture the dynamics of wound closure, and further analyzed through histological and immunohistochemistry techniques to evaluate key markers like collagen deposition, angiogenesis, and inflammation.

Results: Results show that both PLLA and PDLLA significantly enhance wound healing compared to controls. PLLA-treated wounds exhibit accelerated re-epithelialization and increased collagen deposition, reinforcing structural integrity, while PDLLA shows a pronounced effect on angiogenesis, with a higher density of newly formed blood vessels in treated wounds. Both polymers also demonstrate anti-inflammatory properties, reducing inflammatory cell infiltration and lowering pro-inflammatory cytokine expression.

Conclusion: Overall, this study highlights the promising therapeutic potential of PLLA and PDLLA in enhancing skin wound healing, providing valuable insights into their mechanisms and paving the way for future research and clinical applications in human wound care

USE OF PICOSECOND LASER (755 NM) IN THE REMOVAL OF EYEBROW MICROPIGMENTATION: A REPORT OF THREE CASES

Objectives: To evaluate the effectiveness and safety of the 755 nm picosecond laser in removing dark eyebrow micropigmentation through the analysis of three clinical cases, contributing to the expansion of scientific literature on aesthetic laser treatments and supporting the development of more effective and personalized protocols.

Introduction: Eyebrow micropigmentation is a popular but permanent cosmetic procedure. Many patients seek laser removal due to dissatisfaction. The 755 nm picosecond laser shows promise, with rapid and safe action that fragments pigment without damaging surrounding tissue. This study reports three cases treated with this laser to contribute to scientific literature on the topic.

Materials / method: Three women (26–36 years old) dissatisfied with eyebrow micropigmentation were treated with picosecond laser (755 nm). Two to three sessions were performed, with parameter adjustments and different anesthetic techniques. Treatment used various tips and energies from 0.7 J to 1.3 J. A barrier ointment was applied after each session to support healing.

Results: Patients B and C had total pigment removal after two sessions. Patient A showed partial lightening. Response varied based on pigment density and depth. No serious complications were observed. All patients reported aesthetic improvement and satisfaction with the results; one highlighted restored self-esteem.

Conclusion: The 755 nm picosecond laser is a safe and effective option for removing dark eyebrow pigmentation. At least three sessions may be needed for optimal results. Barrier ointment supports healing. Further studies with larger samples are recommended to standardize treatment parameters and improve result predictability.

Trichoscopy-guided diagnosis and non-ablative Er:YAG laser monotherapy in androgenetic alopecia: A clinically innovative approach

Objectives: A recent review by Gupta et al. (2025) highlights LLLT and fractional lasers but does not mention non-ablative Er:YAG laser systems, indicating a current gap in literature.
Er:YAG lasers delivering non-ablative thermal pulses can induce thermal stimulation. This has been associated with WNT/β-catenin pathway activation, angiogenesis, inflammation reduction, and the release of growth factors and extracellular matrix proteins. We present two cases of AGA treated exclusively with this laser modality, aiming to evaluate outcomes and patient satisfaction using a trichoscopy- AI guided diagnosis.

Introduction: Androgenetic alopecia (AGA) is the most common cause of hair loss in both men and women and is associated with significant psychological distress and reduced quality of life, particularly in women. Conventional treatments include minoxidil, finasteride, PRP, and low-level laser therapy (LLLT), with variable response rates and adherence. Laser-based regenerative strategies are increasingly explored. A recent review by Gupta et al. (2025) highlights LLLT and fractional lasers but does not mention non-ablative Er:YAG laser systems, indicating a current gap in literature.

Materials / method: Two patients with clinically and trichoscopically confirmed AGA were treated using an Er:YAG laser delivering non-ablative thermal pulses (~0.5 mm depth). Case 1: male, 22 y/o, Norwood-Hamilton 3V with central thinning. Case 2: female, 48 y/o, Ludwig 1–4, with coexisting alopecia areata. Both underwent trichoscopy and AI-based analysis at baseline. Laser sessions were biweekly, then monthly or quarterly. Follow-up was based on macro photography, clinical evolution, and patient-reported outcomes.

Results: Case 1 showed progressive vertex and frontal density increase by month 6, confirmed by external feedback and macro photography. Maintenance continues quarterly.
Case 2 reported rapid reduction in shedding, visible regrowth, and normalization of daily hair loss. A coexisting patch of alopecia areata resolved during therapy. Trichoscopy and AI analysis supported accurate diagnosis. No adverse events occurred. Both patients reported high satisfaction and improved confidence over time.

Conclusion: Non-ablative Er:YAG laser monotherapy appears to be a safe and effective regenerative treatment for AGA in both men and women. It offers visible clinical improvement without systemic therapy or injectable procedures such as PRP. This makes it an attractive, non-invasive alternative for patients seeking biological treatment. In regions where PRP is restricted or unavailable, it may provide a safe option for improving hair density and patient satisfaction through thermal follicular stimulation. Further studies are needed to confirm biological mechanisms and optimal protocols

Non-ablative Er:YAG laser in alopecia areata: Trichoscopy as a tool for treatment monitoring

Objectives: To demonstrate the potential of non-ablative Er:YAG laser monotherapy in chronic severe alopecia areata, monitored through AI based trichoscopy. The case highlights objective follow-up from baseline to month 8 without systemic or injectable therapy.

Introduction: Alopecia areata is an autoimmune condition with often incomplete or temporary results from systemic or local therapy. Non-ablative Er:YAG laser may induce follicular activity via biological stimulation and inflammation modulation. Trichoscopy guided by AI provides essential support in therapeutic evaluation and follow-up.

Materials / method: A 45-year-old woman developed extensive alopecia areata in 2019. She received both systemic and intralesional corticosteroids and methotrexate without significant improvement. In August 2022, she began non-ablative Er:YAG laser therapy: 8 sessions over 4 months and tapered to 1 monthly session for 8 more months. AI-based trichoscopy was performed at baseline and after 8 months.

Results: Hair shedding decreased after 2 months, followed by visible regrowth. Methotrexate was discontinued during ER:YAG laser treatment. Trichoscopy guided by AI at month 8 showed improved density, increased number of follicular units in the temporal area and increased shaft thickness. After 16 months, the patient reports continued improvement; treatment is ongoing.

Conclusion: This case supports the use of non-ablative Er:YAG laser as a safe and effective monotherapy in chronic severe alopecia areata, particularly when systemic or local treatments yield insufficient results. The objective monitoring by trichoscopy confirms significant benefit in a previously treatment-resistant case.

Large Volume Fat Transfer for the Face: The Technique and Outcomes

Objectives: Large-volume facial fat transfer offers effective rejuvenation but faces challenges with graft retention. This case series reviews outcomes in two patients receiving over 240 ml of fat. One-year follow-up showed sustained volume and no complications. Findings emphasise the role of fat compartmentalisation, technique refinement, and post-op care in optimising results.

Introduction: Autologous fat transfer is key in facial rejuvenation, restoring volume lost with age. ISAPS 2023 reports facial fat grafting makes up 4.7% of global plastic surgeries. Grafting is volume-based: small (<100 ml), large (100–300 ml), and mega (>300 ml). Ageing affects fat compartments unevenly, causing volume loss and imbalance. Large-volume grafting targets atrophic areas for natural correction. This study explores outcomes, fat survival, and technique refinement for lasting results.

Materials / method: Patients with facial volume loss underwent large-volume fat grafting. Pre-op prep included photos, labs, and cessation of smoking/alcohol. Fat was harvested under GA with a 10-ml syringe after tumescent infiltration, decanted, and injected across different planes ,via 1-ml syringes using 18G blunt cannulas in a radial, multi-plane pattern. Post-op care included antibiotics, facial massage, no compression, supine sleep for a week, and continued smoking/alcohol avoidance for four weeks.

Results: Two patients received large-volume facial fat grafting (257 ml and 247 ml), targeting multiple facial areas. One-year follow-up showed sustained volume, natural rejuvenation, and no complications. Radial, multi-layered injection ensured even distribution and minimised risk. Post-op photos confirmed improved contour, and both patients reported high satisfaction with long-term results

Conclusion: Large-volume facial fat transfer is effective for natural rejuvenation. Success relies on precise anatomy, careful fat handling, and multi-plane, radial injection to boost graft survival and prevent ischemia. Post-op care, including avoiding compression and lifestyle changes, supports retention. This case series shows safety, efficacy, and sustained results at one year.

Title: Knowledge, Attitude, And Practice Towards the Adjunctive Perioperative Use of Phenylethyl Resorcinol with Aesthetic Treatments

Objectives: This study evaluates the Knowledge, Attitudes, and Practices (KAP) of Chinese dermatologists regarding the use of adjunctive perioperative skincare in conjunction with aesthetic treatments, focusing on phenylethyl resorcinol, a skin brightening agent for hyperpigmentation. The study findings explore clinical perceptions and offer experts’ insight on integrating adjunctive skincare with aesthetic procedures to enhance treatment outcomes.

Introduction: Hyperpigmentation, including solar lentigo, post-inflammatory hyperpigmentation (PIH), and melasma remains a major concern, especially among Asian population, due to higher melanin content. Topical skin brightening therapies, in combination with aesthetic treatment modalities play a key role in the treatment of hyperpigmentation. This survey explores dermatologists’ perception and acceptance of the integration of phenylethyl resorcinol (PR), a potent antioxidant and tyrosinase inhibitor, as part of comprehensive perioperative aesthetic regimen for skin hyperpigmentation.

Materials / method: A structured online survey via UMER Doctor network platform (dermatology digital learning network in China) recruiting 844 Chinese dermatologists from private and tertiary care public healthcare institutions was conducted. The survey examined the perceived importance of perioperative adjunctive skincare, the efficacy, outcome expectancy, and practice feasibility of PR with common non-invasive or minimally invasive aesthetic procedures. Spearman's rank correlation test was used for the analysis, and P value of <0.05 was considered statistically significant.

Results: 99% of dermatologists (HCPs) advise adjunctive skincare to patients. 99% of patients are aware of and demand perioperative adjunctive skincare. 83% of HCPs agree that topical PR can reduce and inhibit melanin synthesis, improve hyperpigmentation. More than 75% of HCPs agree that PR is beneficial in enhancing and maintaining efficacy of aesthetic treatment. 66% and 55% of HCPs agree PR monotherapy is effective for acne-related PIH and solar lentigo, respectively, while 71% of HCPs agree that multimodality of PR with aesthetic therapy can yield good treatment outcome in recalcitrant melasma

Conclusion: Majority of the surveyed dermatologists recognize the adjunctive role of phenylethyl resorcinol (PR) in synergistically enhancing and maintaining the efficacy of skin brightening aesthetic treatments while reducing the risk of PIH. These insights highlight the growing importance and high acceptance of PR as adjunctive perioperative skincare in combination with aesthetic therapy for the management of hyperpigmentation.

Retention of Broken Cannula Tip Following Debulking of Lower Limb Free Flap Reconstruction: Rare Case Report and Literature Review

Objectives: Liposuction has become a a highly common procedure performed by plastic surgeons since its advent in 1979. However, it is not without risk. The authors report their experience with breakage and retention of the tip of a liposuction infiltration cannula. They also discuss their implemented interventions to prevent recurrence of such complications.

Introduction: Liposuction is a common cosmetic and functional procedure. It is generally considered safe. The most common complications are seroma, haematoma, and bruising. Breakage of the tip of the cannula has rarely been reported in literature. The authors report their experience with retention of a broken tip of an infiltration cannula, their subsequent management, and learning points.

Materials / method: A 66-year-old man with previous anterolateral thigh (ALT) free flap reconstruction for his right foot underwent liposuction-assisted and open surgical debulking of the flap. Over the next 2 weeks, the proximal third of the flap developed full thickness necrosis, wound dehiscence, and purulent discharge. Bedside debridement revealed an 18-mm cannula tip lodged in the subcutaneous tissue. This led to full disclosure, intravenous antibiotics, and formal surgical debridement in the operating theatre. He eventually received skin graft coverage over the new wound.

Results: Late recontouring of bulky cutaneous flaps using liposuction is common, particularly in lower limb reconstruction, for achieving well-contoured coverage. In our case, an infiltration cannula tip broke intra-operatively, marking the first reported instance during lower limb flap debulking. Factors such as instrument wear, suboptimal technique, and the tip's small size may have contributed to this. Interventions include purchase of a new liposuction set, replacement of the broken cannula, phasing out cannula calibre < 2-mm, and new protocols for measurement of cannulas before and after use.

Conclusion: The authors’ experience, together with the cited case reports, demonstrate that cannula tip breakage (be it during infiltration or liposuction) is rare but possible. Surgeons should be cognisant of these risks and counsel patients appropriately. They should also select appropriate instruments for their task and ensure good technique. OT staff may also put in place stricter protocols for inspection and maintenance of instruments. All team members handling the instruments should carefully inspect them before and after use, particularly the fragile points.

Anti-aging mechanism of skin cells through macrophage activation by brown rice extract

Objectives: In this study, we investigated whether the mature extract of brown rice (KR-ex.), which is made by extracting only the nutritious parts of brown rice (sub-aleurone layer, scutellum of germ, and crushed cell group), exerts an anti-aging effect on skin fibroblasts through the activation of human macrophages (MΦ). That is, we aimed to pursue the anti-aging effect of skin through inner care by gene expression of fibroblast proliferation-promoting growth factor 2 (FGF2) and STAB1, which is necessary for phagocytosis of aged fibroblasts, when cultured human MΦ are stimulated with KR-ex.

Introduction: In Japan, KR-ex. has been commercialized as a functional food. It has been reported that it has health benefits such as reduced susceptibility to colds and a decrease in the prevalence of skin problems. We have clarified the immune-stimulating effect of KR-ex. by activating human MΦ in an in vitro system and shifting intracellular energy metabolism from mitochondria to glycolysis. However, the effects of KR-ex. on the skin remain unclear. In recent years, the relationship between the M1/M2 balance and the dysregulation of collagen homeostasis in photoaged skin has been demonstrated.

Materials / method: KR-ex. was added to MΦ-like cells differentiated from human monocyte cell line (U937 cells) with phorbol myristate acetate and allowed to act for 24 hours. In addition, a control (Cntrl) was prepared by adding sterilized purified water, and a positive control was prepared by adding lipopolysaccharide (LPS) (1 μg/mL). Macrophage activation was measured by gene (mRNA) expression of interleukin (IL)-1β (M1 type) and STAB1 (M2 type), which are produced during MΦ activation. The mRNA expression of growth factor (FGF2), which directly activates fibroblasts, was also measured.

Results: The expression of STAB1 in MΦ treated with KR-ex. decreased in a KR-ex. concentration-dependent manner and showed a negative correlation with IL-1β. Meanwhile, FGF2 was generally expressed at a low level, but was significantly higher than that of Cntrl at 10 μg/mL and 100 μg/mL or more. Overall, changes in the behavior of gene expression were observed at 33.3 μg/mL and 100 μg/mL, which are the intermediate concentrations of the six KR-ex. concentrations. At low concentrations, only M2Φ-related genes were expressed, while at high concentrations, both M1Φ- and M2Φ-related genes were expressed.

Conclusion: It was suggested that KR-ex. may promote the proliferation of skin fibroblasts through growth factors such as FGF2 via the activation of MΦ, and that the enhanced phagocytic ability due to STAB1 expression may exert an anti-aging effect on skin tissue by eliminating senescent fibroblasts. KR-ex. is a functional food that may have an effect on improving skin, and is also thought to expand the possibilities for cosmetics that can contribute to skin aging.

Bridging the divide- A hybrid breast augmentation strategy for minimising Cleavage Gap

Objectives: To evaluate outcomes of hybrid breast augmentation

Introduction: According to international global survey, second most commonly performed aesthetic procedure is breast augmentation. Breast implants are widely regarded as one of most pivotal devices in the history of aesthetic surgery. In recent years, the perception of attractive breast has shifted towards 3D analysis with particular attention to shape and preferred proportion patterns. A major drawback of breast implants is the inability to provide a narrow clevage as desired by most females. Thus, we combined breast implants with fat grafting to achieve the desired results.

Materials / method: A retrospective analysis was conducted from January 2021 to September 2024, on 131 female patients ranging from 30 to 55 years who underwent hybrid breast augmentation. The procedure involved subfascial silicone implant placement and fat grafting to the superomedial breast quadrant. Fat was harvested from the thigh or lower abdominal region. Outcomes included cleavage distance measurements and patient satisfaction assessed via Breast- Q score.

Results: There was a significant reduction in postoperative cleavage distance ranging from 9.5 ± 1.23 cm to 4.74 ± 0.48cm with a p-value of <0.0001. Implant size ranged from 230cc to 320cc (mean 271.64 ± 24.22cc). Fat grafting volume ranged from 50 to 100cc (mean 63.11 ± 11cc). Minor complication in the form of wide scars was noted in 3.1% of cases. The highest satisfaction of “very satisfied” was reported in 93.9% of cases.

Conclusion: Fat grafting enhances key aesthetic features softening the intermammary cleft, creating fullness in the upper and medial portions ensuring symmetry between the breasts.
By ingeniously combining the voluminising power of implants and natural enhancement of the grafting, we acheived harmonious balance.

A Kaleidoscope of Skin: Incontinentia Pigmenti in a Newborn

Objectives: To describe a rare case of Incontinentia Pigmenti (IP) in a newborn Filipino female patient.
To highlight the clinical manifestations and diagnostic challenges associated with IP in early infancy.
To emphasize the importance of early recognition and management of IP to prevent potential complications.
To contribute to the limited literature on IP in the Filipino population.

Introduction: Incontinentia Pigmenti (IP) is a rare X-linked dominant genodermatosis characterized by a distinctive pattern of skin lesions that progress through four stages. This case report presents a rare case of IP in a newborn Filipino female patient, highlighting the importance of early recognition and management of this complex condition.

Materials / method: We reported a case of a 1-day-old female neonate with multiple vesicles, pustules, and desquamated patches on erythematous plaques on her scalp, trunk, buttocks, genitals, and extremities. Dermoscopic examination for new vesicles revealed a yellow center and erythematous halo, while older lesions had yellowish serocrusts surrounded by scaling.

Results: Tzanck smear showed multiple eosinophils, and punch biopsy of one of the lesions revealed eosinophilic spongiotic dermatitis, consistent with IP. The patient's eye exam was normal, and she had occasional seizures that were managed with medication. Upon discharge, her eye exam was still normal, and she was seizure-free. She was treated with emollients and topical steroids to help manage her intermittent skin flares with favorable results.

Conclusion: This case report highlights the importance of early diagnosis and treatment of IP. The skin lesions can be very distinctive, but they can also be less obvious, especially to non-dermatologists. Raising awareness of IP will help to prevent unnecessary medical interventions and ensure that patients receive the care they need. Patient and family education about the genetic implications of IP and its other potential complications is also imperative.

Five component composite augmentation spreader grafts in Rhinoplasty

Objectives: To study the aesthetic outcomes of dorsal augmentation by five components composite L - strut Autogenous rib cartilagenous framework fixed to nasal bone by grooving.

Introduction: Aesthetic and functional problems are encountered due to collapse of nasal dorsum that can be due to infection, post traumatic, congenital or iatrogenic causes. Dorsal augmentation via costal cartilage is a valuable means of improving nasal appearance by correcting low and/or concave nasal dorsum.
Various options for autologous augmentation of nasal dorsum include - Diced Cartilage Fascia (DCF), solid rib cartilage, sliced counter opposed rib cartilage etc. Costal cartilage provides reliable structural support with sufficient strength, resistance to resorption and is present in good quantity.

Materials / method: The framework is reconstructed into five components: The strips are cut 1.5-3mm thick, 3.5- 4 cm long, and 6-7mm wide for spreader/septal augmentation. Caudal strips which are 1.5-2mm thick, 20-25 mm long and 4-5mm wide and at its posterior end, wedge is made for the Anterior Nasal Spine. The caudal and dorsal strips are sutured tongue in a groove fashion at the desired angle. A Gap graft is placed. DCF- For dorsal onlay placed. A groove is made in the nasal bone on either side up to the Bony Radix. The groove is made just of adequate width to fit the cartilage snugly.

Results: Out of 15 cases (9 females and 6 males), 7 were Binder’s syndrome and 8 were of severe saddle nose deformity. No major complications were seen like graft warping, skin necrosis, infection, except in one patient who had slight hump on one side of nose because of DCF mal-alignment. All patients were satisfied with the aesthetic outcome with mean follow up of 11 months. None of the patients complained of airway problems postoperatively.

Conclusion: Our study shows that it is an instrumental technique for Indian patients with thick nasal skin. Our technique uses the advantage of both the solid structural augmentation and soft dorsum for good contour and no graft show. Internal nasal valve collapse is prevented and there is significant improvement in patient’s functionality.

Clinical Experience of Primary Subcutaneous Mycoses in Shanghai: A Retrospective Analysis

Objectives: The nonspecific clinical symptoms of primary subcutaneous mycoses pose significant challenges for clinicians. Increased awareness of these infections can help prevent adverse outcomes associated with delayed diagnosis and treatment. Therefore, a study that focuses on primary subcutaneous mycoses in Shanghai and summarizes the clinical experience is warranted.We summarized the clinical features, histopathological findings, etiological characteristics, drug sensitivity test results, therapeutic regimens, and follow-up data to optimize the management of subcutaneous mycoses in Shanghai.

Introduction: Primary subcutaneous mycoses are a heterogeneous group of fungal infections caused by pathogenic or opportunistic organisms. In recent years, cases have steadily increased in Shanghai, an area where the disease was previously uncommon. This study aimed to summarize clinical experiences with primary subcutaneous mycoses in Shanghai, to optimize their management.

Materials / method: 33 patients with confirmed primary subcutaneous mycoses were enrolled . Their medical histories, clinical features, histopathological findings, etiological characteristics, drug sensitivity tests, therapeutic regimens, and follow-up data were recorded.

Results: Identification of pathogens from skin tissue cultures revealed distinct colonial morphologies across diverse mycological species. The isolates included yeast (45.5%), mold (30.3%), and dimorphic fungi (24.2%). The most common species were C. parapsilosis (n = 8, 24.2%), T. rubrum (n = 5, 15.2%), and S. schenckii (n = 8, 24.2%). Thirty-two patients received systemic antifungal treatment based on the results of the drug sensitivity test, whereas one patient was treated with complete surgical resection, owing to a single plaque.

Conclusion: This study highlights the considerable diversity among fungal species implicated in primary cutaneous mycoses and underscores the complexities involved in their accurate diagnosis and management. Correcting unhealthy lifestyles, boosting immunity, and completely removing and avoiding re-exposure to the pathogenic fungi can effectively reduce the risk of relapse in primary subcutaneous mycoses. Our findings provide valuable insights into primary subcutaneous mycoses and may contribute to improved patient prognoses.

A rare case of skin infection caused by Corynebacterium striatum in an immunocompromised adult

Objectives: C. striatum has significantly challenged the healthcare of adults, especially those with weakened immune systems. However, skin infections are occasionally associated with C. striatum and such cases were rarely described. This report presents a rare case of skin infection caused by multidrugresistant C. striatum in a patient with prior medical history of rheumatoid.

Introduction: Corynebacterium striatum (C. striatum) is commonly viewed as a contaminant once isolated from clinical specimens as it often colonizes human skin and mucous membranes. As an emerging causative agent of hospital-acquired infections, C. striatum has increasingly associated with various infections in immunocompromised and hospitalized patients, particularly in rising cases of drug-resistant and multidrug-resistant infections. However, skin infections are occasionally associated with C. striatum and such cases have been infrequently documented.

Materials / method: This is a Case report.Here we report a rare case of a 62-year-old female with prior medical history of rheumatoid arthritis and breast cancer, who experienced a non-healing skin abscess on her right hand for one year following an accidental cut.

Results: Cultures obtained from both pus and deep subcutaneous tissue produced non-hemolytic and milky-white colonies when grown on 5% sheep’s blood agar. The isolate was promptly identified as C. striatum using matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS). Based on the in vitro antimicrobial susceptibility testing results, the lesion was effectively treated with a combination of rifampicin and clarithromycin over a period of 3 months.

Conclusion: This case highlights the risk factors for C. striatum and the potential of C. striatum as a possible pathogen causing skin infection, rather than merely a clinical contaminant.We recommend that all pure cultures of diphtheroids be identified to the species level, and their antimicrobial susceptibility testing be conducted to facilitate the initiation of prompt and appropriate treatment for a successful outcome.

Comparative Effectiveness and Safety of Fractional Laser and Fractional Radiofrequency for Atrophic Acne Scars A Propensity Score Analysis

Objectives: To compare the clinical effectiveness and safety of fL and fRF in the treatment of atrophic acne scars.

Introduction: Atrophic acne scars are a common dermatologic concern, particularly in Asian populations where the risk of post-inflammatory hyperpigmentation (PIH) is elevated. Fractional laser (fL) and fractional radiofrequency (fRF) are widely used energy-based devices (EBDs), but comparative evidence in Fitzpatrick skin types III–IV remains limited.

Materials / method: A retrospective cohort study was conducted among patients aged 18–60 years who underwent at least two sessions of fL or fRF between 2012 and 2023. Propensity score (PS) stratification adjusted for baseline confounders. Clinical improvement was assessed at 1, 3, and 6 months. Missing data were handled via multiple imputation. The primary outcome was the proportion of patients achieving ≥25% improvement. Equivalence testing was performed with a predefined margin of 20%.

Results: A total of 397 patients were included (254 fL; 143 fRF). After PS stratification, baseline characteristics were balanced. At 6 months, ≥25% improvement was achieved in 88.1% of fRF-treated and 71.9% of fL-treated patients. While fRF consistently showed greater mean improvement, differences were not statistically significant. fL was not inferior to fRF. Pain scores were significantly higher with fRF (p < 0.001), but adverse events, including PIH, were infrequent and comparable.

Conclusion: Both fL and fRF are effective and safe for treating atrophic acne scars in Fitzpatrick skin types III–IV. fL was not inferior to fRF while fRF showed more discomfort but comparable safety.
