Objectives: To demonstrate the nature of Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) in Australia since the index case was first reported in 2007.

Introduction: BIA-ALCL has been an emerging cancer for over a decade that has been linked to the use of textured devices. Recent increase in number and frequency of cases has led to worldwide regulatory action.

Materials / method: Confirmed historical cases were collected and then prospectively analyzed from October 2015 to May 2019. Clinical and implant exposure data was determined and compared with company sales data for four devices to generate implant-specific risk.

Results: 104 cases of BIA-ALCL were diagnosed in Australia. The mean age of patients was 48.2 years. Average time from implantation to diagnosis of 6.8 years. 51.7% of implants utilized were Allergan Biocell devices. The indication for implant usage was for primary cosmetic augmentation in 70%, post breast cancer reconstruction in 23% and following weight loss/pregnancy in 7%. The majority of women presented with early (stage 1) disease (87.5%). The risk for developing BIA-ALCL ranged from 1 in 1947 sales for Silimed Polyurethane devices to 1 in 36730 for Siltex imprinted textured devices.

Conclusion: Implants with higher surface area/texture seem to be more associated with BIA-ALCL in Australia. Recent regulatory action to suspend, cancel, or recall some of these higher risk devices is supported by these findings.

Objectives: To review the USA perspective on BIA-ALCL and Breast Implant Illness

Introduction: Anaplastic large cell lymphoma and breast implant illness have been responsible for a global re-evaluation of the types of devices we choose for implant based augmentation and reconstruction

Materials / method: Changes in the United States have focused on the dialogue with patients in that we now discuss the risk of ALCL and BII with all patients.

Results: Patients are informed that the risks are very low and that they need to make an informed decision and in some cases, sign a patient decision checklist. Another aspect in the United States is the type of implant used. The majority of plastic surgeons have used smooth round devices in the vast majority of cases. The use of textured surface devices has diminished even more from 3-5% to now less than 2% of devices used. Many patients now present and demand explantation and “en-bloc” capsulectomy. In the USA, we explain that “en-bloc” capsulectomy is reserved to patients with active disease.

Conclusion: Breast implant illness remains a vague condition with a constellation of symptoms affecting a relatively small group of patients. Ongoing research is being conducted; however, to date there has been no definitive study definitively linking these symptoms to breast implants despite overwhelming evidence to support the safety of breast implants. Despite this, social media continues to expand and fuel negativism related to breast implants.

Objectives: Between the end of 2018 and the beginning of 2019, BIA-ALCL and BII completely overturned the market for breast augmentations due to the high media resonance worldwide.

Introduction: BIA-ALCL and BII in Italy have led to a turnaround in the market for breast implants, but they have certainly stimulated research to better understand the pathologies.

Materials / method: the analysis of the world and local Italian case studies have led to the creation of study and consultancy groups for the Italian Ministry of Health.

Results: From the patients' point of view, the fear of these pathologies has greatly diminished, also thanks to the media, which quickly reduced their alarmism. as far as the Italian Ministry of Health is concerned, guidelines have been created for doctors and patients to ensure rapid diagnosis and correct treatment.

Conclusion: Thanks to worldwide cooperation and similar guidelines from scientific societies and the various ministries of health, BIA-ALCL and BII are no longer scary for patients and doctors alike.

Objectives: Breast implant–associated anaplastic large cell lymphoma (BIA-ALCL) is an uncommon neoplasia occurring in women with either cosmetic or reconstructive breast implants. Our idea in this Congress would be to know the opinión of the leaders in the field of plastic surgery from all over the world , invited to express and discuss their opinion about some key questions on BIA-ALCL, trying to reach a consensus about BIA-ALCL cause, pathogenesis, diagnosis, and treatment in light of the actual best evidence.

Introduction: The increasing incidence and associated mortality of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) has become alarming. However, many patients remain unaware of their risk for BIA-ALCL and may overlook early warning signs of the cancer

Materials / method: We do not have enough informations and data concerning the BII or also called Asian Sindrome

Results: Just a few cases of ALCL around the world ( less tan 600 ) in more than 15 millions of women.

Conclusion: Informing patients with breast implants at risk for BIA-ALCL is an important endeavor. Patients educated on the disease will likely be diagnosed and treated earlier.

Conclusion: Continuar con una evaluacion y control cercano y alejado de todas las pacientes que se hallan realizados Mastoplastias con Implantes y /o Reconstruccion Mamaria a los efectos de diagnosticar precozmente BIA-ALCL, y su correspondiente estadificacion y tratamiento oportuno.-
