Design lip elegance algorithm

Objectives: Understanding a visual algorithm for lip filler treatment. Learning Step by step all the passages and techniques for creating natural looking lips with HA fillers.

Introduction: Lip filler injection is growing as one of the top face treatment, thanks to social media boom. Performing elegant and natural results is mandatory for modern aesthetic standard. Lips are unique and women wishes are completely individual, so operator has to follow a useful and simple algorithm for creating the best shape for each face. Following this algorithm allows the operator to avoid bumps, unnatural or duck shape and above all to enhance the best individual topics of each lip.

Materials / method: The algorithm is divided in two steps.
FIRST STEP: the overall lip shape on face.
Operator has to identify 1) the individual dental class of patient (on lateral view) and 2) the individual ratio between upper/lower Lip (on frontal view).
By this simple identification operator will decide which part on lateral and frontal has to enhanced and which is not to be filled.
SECOND STEP: the shape ideal building.
Lip shape is divided in five parts. Three part on upper lip (two lateral sides, one central part), two parts on lower lip. Each lip part has to be constructed by following specific geometric

Results: Response to treatment after 6 and 12 months were observed in 90% and 80% of patients, respectively. The mean value of IGA score in month 6 and 12 were 4.4 ± 0.60 and 4.2 ± 0.67, respectively. The study subjects were all satisfied with their lip improvement lip shape is correct in frontal and lateral view in each case. Reported adverse effects were temporary and mostly mild in severity.

Conclusion: By using a visual algorithm to construct the best version of each individual lip shape, operators, despite of his/her experience and individual patient requests, could create correct shape with no bumps, duck shadow or incorrect proportion.

Non-surgical correction of the lower eyelid using fillers

Objectives: The report describes the author's classification of the lower eyelid zone and original technique of correction of tear trough and palpebromalar groove, depending on the presence and prominence of orbital fat.

Introduction: Injection correction of tear trough with hyaluronic acid fillers usually gives a good aesthetic result. But in some cases, after correcting tear trough with a good immediate result, adverse effects may appear after a while. There are accumulation of the filler below the tear trough, enlargement in the hernia of lower eyelids, and periorbital puffiness. To prevent these adverse effects, it is very important to consider the anatomical features of each patient.

Materials / method: This study was performed in 118 female patients. Patients were divided into 3 groups, depending on the presence and prominence of orbital fat. The injection correction of the lower eyelid zone was performed using a hyaluronic acid filler.

Results: The result was determined by the patient through a visual assessment of photographs based on the criteria for the presence of tear trough and palpebromalar groove. Patients noted good satisfaction with the results of the correction of the lower eyelid zone. Only six patients were dissatisfied with their results.

Conclusion: Selection of patients and the correction of the lower eyelid zone, taking into account the principles outlined in this report, gives a good aesthetic result with a low risk of adverse effects.

Orbital complications of dermal fillers

Objectives: Orbital complications may occur long after the filler injections and since the site is distant from the injection, patients will not immediately make the connection. Furthermore, this can cause ordering unnecessary examinations and performing surgery in attempt to identify the etiology. Fillers should be considered in the differential diagnosis of patients presenting with a new onset orbital process.

Introduction: Filler injections continue to grow in popularity as a method of facial rejuvenation. We report a series of cases demonstrating a new complication of fillers in the orbit. Treatment methods and possible mechanisms are discussed.

Materials / method: Retrospective, multicenter review of six patients who underwent dermal filler injection to the face with subsequent orbital complications

Results: #1:61 yrs female underwent excisional biopsy d/t swelling lid which revealed a massive FB reaction.
#2:63 yrs female underwent orbitotomy d/t orbital lesion. biopsy revealed FB reaction with acellular bluish material.
#3:67 yrs female underwent orbitotomy d/t orbital lesion that revealed liquid mass and HA filler.
#4:47 yrs female had inability to adduct the eye post injection. Neuro consultant presumed as filler was related.
#5:42 yrs female underwent exploration d/t orbital mass which was a filler.
#6:45 yrs male, underwent lacrimal surgery as the filler obstructed his lacrimal sac.

Conclusion: We present six cases of orbital complications secondary to dermal filler injection. Possible mechanisms include inadvertent direct injection or filler migration. Filler migration implies that the filler is present at a location other than the injection site. This is a possible mechanism as the orbit symptoms did not appear directly after the injection, in some cases occurring years after injection. However, unrecognised intra-orbital injection could explain how the material crossed the septum. In all cases, however, surgical intervention was required.

Harmonization of facial proportions

Objectives: - Analyze the proportions of the European face type;
- Choose the key areas for filler injecting in order to obtain a visible result while maintaining individuality;
- Learn how not to aggravate imbalances and not to make similar looking faces by analyzing the main categories of “unsuccessful” corrections;

Introduction: Dermal filler injection is one of the most popular procedures, but the result does not always work aesthetically. According to my observation, there are the following categories of “unsuccessful” corrections :
- “Moon-shaped face” - round, extended; there are no wrinkles and folds, but the person does not look younger;
- “Duck-lips” - the lips themselves look perfect in the photo separately, but on the face as a whole - neither naturally nor aesthetically
- “Carrot Face” - an excessively elongated chin
- “Asymmetry”, which arose or worsened after the filler injection;

Materials / method: Literature review, facebook / instagram browsing, 10 years of my own experience

Results: An effective approach to the analysis of the proportions of a European face type without special equipment;
The algorithm for determining key correction zones to obtain a visible result while maintaining individuality, as well as the exclusion of zones in order to avoid “unsuccessful" corrections.

Conclusion: Each face is individual and inimitable and our task is to obtain a visible aesthetic result, but at the same time preserve its natural beauty.

Gender specifics in aesthetic medicine

Objectives: analysis of gender characteristics of male and female faces

Introduction: Few years ago, aesthetic medicine was a service mostly for female, but year by year there are more and more male patients.
There are some psychologic specifics of male and female patients, which should be taken into account.

Materials / method: analysis of gender features of male and female faces, which have an impact on the approaches to esthetic correction.

Results: Usually male patients want to get quick results. They don't want severe rehabilitation, they don't want to feel pain, usually they don't read about methods of treatment. They don't have certain complains, they want to became more handsome and young. Usually they follow doctors recommendations exactly. They are more loyal to the doctor.
Female patients usually ready for long, step by step treatment, they are ready for rehabilitation and painful treatment. Women usually read a lot about deferent methods in aesthetic medicine and know what and how they want to treat. They look a lot on themselves and know every wrinkle in their faces. They are looking for the miracle, and they can try different doctors, which were seen in different social networks.
It’s very important to take into account gender specifics, planning male or female correction in order not to get caricatural result.
The animators of the Walt Disney Company underlined gender features of princes and princesses faces to prettify them. Analyzing beautiful princes and princesses faces we can see gender specifics for male and female faces.
Specific for male faces are: straight line of eyebrows, straight line of upper eyelid, mandibular emphatic corner, round or square chin, longer upper lip, plane infraorbital area, prominent cheekbones.
Specific for female faces are: triangular face shape, eyebrow’s line with elevated corner and eyebrow’s tail, round shape of the upper eyelid, volume in the area of cheekbones and in the midface, expressive lips.

Conclusion: The correction of masculine face is contemporary and difficult esthetic procedure. It’s necessary to take into account gender features, which enhearten face, and individual features of facial skeletal pattern and soft tissues.

"Not a girl" - The main tips of male volumization

Objectives: According to growing popularity of male aesthetic correction and a lot of practical "mistakes" there are some clinical tips to keep male features after injections in men.

Introduction: injections in male patients demonstrate a lot of different peculiar features. The main problem is a risk of feminization which usually occurs after using of HA - fillers. Severity of edema provokes feminized face features. In our practice we have developed a special protocol using CaHa - filler in different techniques.

Materials / method: Using of CaHa - filler in male patients allows us to inject in special "masculine" technique. This complex procedure consists of jaw line correction, chin, temple and cheek augmentation. It helps to exaggerate standard male features and make our patients more handsome.

Results: According to 5 - year experience, using of CaHa - filler in male patients helps to avoid blurring of masculine face features and to emphasize them very attractively

Conclusion: This technique is recommended for male patients treatment and for emphasizing masculine face features

A dimple on the chin, the devil within

Objectives: CHIN is the key for fascial treatments mostly overlooked by many of us as astheticians and patients and as sculptors it is important to bring it in their notice .Direct improvement of lower face is indirect correction of upper face and double chins hence its a target area for face proportion.3 dimensional approach to chin augmentation which includes treating the surrounding areas as prejoul sulci ,paragonial and gonial angles by combining the botulinum and stimulatory injectables with lasting neocollegenesis brings exceptional proportions and looks.

Introduction: Customizing chin length, width,projection and shape depends on injectors diagnostic approach ,face does look beautiful in divine proportions of phi but the proportion is just a reference and doesnt adress some fine crinkles that are produced in an attempt to create JUST chin.as davinci proposed

however irony in doing chin is injector produce the prejoulsulci which were not there before so 'path to hell is poved by good intentions; the potential might start noticing these sulci in selfies

Materials / method: In all patients with (migrogenia or microgenthia) the product selected was one with good graph of expansion over 4-6 months post injection chin itself was approachable with needles in deeper plane with 3 boluses technique. Total of 3 ml Ha filler after 45 angle for projection and elongation. Additional 2 mil of PCL was injected in variable planes in paragonial area and Prejoul sulci with cannula to replace losses in variable planes. Ideally procedure was split into 2 steps to avoid vascular and lymphatic compromise along with several units of Botox were injected in mentalis

Results: CHIN is perhaps the most eminent aspect of facsial asthethics as it completes the frame of face and axis of communication between different features,this is an advance combination of HA fillers and non HA biostimulators to increase the longevity of results and reduce overall cost ,techniques targetting the multimodality approach to create perfect chin n surrounding area In both animation and statis repose brings harmonius and natural results without leaving an artifact

Conclusion: Doing chin properly decrease the stigma of ageing our method of making the intelligent choice for their use depends on systematic approach ,patients can understand the changes in a more precise manner with measurable outcome. Hence what matters in the end is assessment and evaluation of face, appropriate choice of injectable ,recognition, avoidance and treating complications of not doing the faces properly and safely. Core of this lecture is precision in technique with 5 point analysis to decide on appropriate treatment plans.
