Objectives: understanding the special anatomy of the nasal pyramid
understading why we are using which flap for closing a local defect on the nose
tipps and tricks to consider in reconstructive surgery of the nose

Introduction: The complex three dimensional anatomy of the nasal pyramid is always a challenge to reconstruct, specially in skin cancer surgery with local flaps after excision.

Materials / method: In a retrospective study of 112 patients who have been operated between 2013 and 2019 we analysed the different localization of the cancer and discuss the chosen flap with the aesthetic and functional result . Complications and special difficulties in this type of surgery are exposed.

Results: The glabellar flap for defects of the upper third of the nasal pyramid , Rieger/ Marchac flap for the lower third of the nasal pyramid / nasal tip, the bilobed flap for defects of the lateral portion of the nasal pyramid and nasolabial fold advancement flaps for the close to alar defects are our prefered and mostly used flaps.

Conclusion: Respecting the aesthetic units and taking consideration of the complex anatomy of the external nose
we give some guidelines to get optimal form and function in reconstructive surgery of the nose.

Objectives: Appreciate the utility of an island pedicle propeller flap as a viable option for reconstructing nasal defects involving the nasal dorsum and nasal sidewall.

Introduction: Propeller flaps have classically been described in the plastic surgery literature. A propeller flap is a type of island pedicle flap that reaches the recipient site through axial rotation. We demonstrate the use of a procerus-based propeller flap as an excellent, single-stage reconstructive option for patients with large defects of the nasal dorsum and sidewall. The flap affords good patient satisfaction and cosmetic result.

LEARNING OBJECTIVES
Following this course, the attendee should be able to :
• Select defects appropriate for transposition flaps on the nasal ala.
• Design and execute single and multi-lobed local transposition flaps.
• Design and execute regional transposition flaps.

Results: DESCRIPTION
This practical session will use case examples to illustrate key principles to execute single and multi-lobed transposition flaps for nasal defects. Topics will include: selection of appropriate defects and tissue reservoirs, principles of transposition flap design, and anatomy relevant to lifting and tacking these flaps to produce reliable results and ideal contour.

Conclusion: This session is directed toward dermatologic surgeons of all levels.
