Objectives: Techniques of hair transplantation are evolving with time both in terms of use of better methods of graft harvestment and implantation. The result of the procedure ultimately depends on the tenderness with which grafts are handled, but also on the type of the implanter used.
Introduction: This study aims to explain the basic rules of implantation (preparation, types of implanters, placement technique) and to showcase the advantages and disadvantages of Direct Implantation technique. Also, it aims to show the use of direct implantation in other areas and cases, such as beard, eyebrow, and scar restoration.
Materials / method: This technique has been applied in 1750 patients during the last year, both men and women, who were willing to undergo a hair transplantation by follicular unit excision (FUE) and one-step implantation method. After the administration of local anesthesia, the extraction of the hair follicles from the donor area was performed, using a motorized punch. Then, hairs were classified according to the number of follicles and the recipient area was designed. Lastly, the direct implantation of grafts without reception holes was performed.
Results: Direct hair implantation without reception holes with sharp implanters, in the hands of the experienced FUE hair surgeon may be a reliable alternative technique which combines excellent, natural, dense results with significantly less trauma and postoperative pain and side effects, even in the most demanding and difficult cases e.g. scar correction or eyebrows.
