Objectives: Even though treatment of solar lentigines seems to be the easiest to treat, there are patients that can be quite challenging.

Introduction: In Portugal, a sunny country all year long, benign pigmented lesions are a common cosmetic concern for many patients. Over the counter and prescription topical creams, liquid nitrogen, chemical peels and light-based modalities are well known treatment modalities.

Materials / method: There are a numerous of LASERs that can specifically target pigmented lesions, including red-light LASERs (694-nm ruby, 755-nm alexandrite), green-light LASERs (510-nm pulsed dye, 532-nm frequency-doubled Nd:YAG), and near-infrared LASERs (1064 nm Nd:YAG which perfectly illustrates the broad absorption spectrum of melanin. There are other LASERs not so target-specific (argon, krypton, copper, carbon dioxide and erbium-doped yttrium aluminium garnet) that can be effective when associated with adequate cooling devices.

Results: We have to take into consideration the LASER settings, the initial clinical response and the patient tolerance to treatment.

Conclusion: I will present a clinical case of a patient with solar lentigines treated with a picosecond LASER.

Objectives: - the causes and the risk factors for pigmented lesions,
- the management of these lesions, including sun protection, local treatment, and energy-based devices approaches,
- the practical approach for the pigmented lesions.

Introduction: Skin represents both the largest organ of the body and the first line of defense against aggression. Skin health and skin influence directly self-esteem and mental status, social interactions, and quality of life.

The main factors with adverse effects on the skin are exposed to sunlight or UV, smoking and pollution, toxins from various substances used in the environment, chemicals in skincare products, profession, and various diseases.

The face is the primary source of information in social perception and serves a key role in social perception.

Materials / method: The distribution of pigment color (blood and melanin) in the skin can affect human faces' apparent health, age, and attractiveness. Facial appearance, or perceived age, comprises different facial features, including skin wrinkling, lip size, pigmented spots, skin texture, and sun damage.

Hyperpigmentation (postinflammatory dark marks, melasma, or solar lentigines) increases the perception of an aged appearance to a greater degree than wrinkles.

Results: Skin pigmentation is an essential defense process against UV light that involves key cellular players: MELANOCYTES and KERATINOCYTES in the epidermis and FIBROBLASTS in the dermis.

We must think in 3D because melanin is not horizontal and vertical; melanin is BOTH horizontal and vertical. Also, Melanosomes (stage I-IV) are horizontal and vertical, and synchronous and asynchronous! So, we don’t have the best treatment, and we have the best approach.

Conclusion: The pigmented lesions represent the first sign of skin aging, with a direct impact on first impression, perceived age, attractiveness, perception of health and beauty,

Also, pigmented lesions impact many areas like social, personal, professional, leisure, etc. We must have a PRECISE diagnosis before treatment because we want to exclude the risk for melanoma.
Treatment is more THAN laser and includes sun protection, lifestyle, skincare, local, and general treatment!
