New methodologies for rejuvenation, skin disorders, and hair growth

Objectives: This lecture has two objectives.
1. Evaluate different procedures for hair loss.
2. Examine different therapies for rejuvenation, hair growth and skin disorders which are the most difficult to treat presenting high levels of reoccurrence and a substantial deterioration with ageing

Introduction: We explored a variety of methods such as RF, laser, monochromatic near-infrared treatment, TENS, acupuncture, and pulsed electromagnetic fields applied to hair loss, facial rejuvenation and skin disorders. Most current technologies demonstrated inconclusive, limited, or temporary results with minor or short-term improvements. Hair growth was one of the most challenging areas of rejuvenation since even hair transplants do not really produce the attractive results one sees in the ads.

Materials / method: We explored low-energy technologies that seemed to offer long-lasting results with no re-occurrence of skin lesions and substantial rejuvenation as well as reasonable hair growth. Research conducted by ultra-low energy technologies also reports a reduction of inflammation because of antioxidant electron donation transforming free radicals into stable molecules. We report the results of clinical studies on rejuvenation, hair growth, and a randomized double blind one-year-long longitudinal clinical study on rejuvenation, hair growth and repair of skin lesions.

Results: After 8-10 treatments patients experienced fast relief from their symptomatology with no side effects and no symptom rebound. The statistically significant results on wound repair were not age contingent. This contradicted past research postulating that age-accumulated inflammation and endothelial dysfunction can further exacerbate skin disorders.

Conclusion: Importantly, a method offering age-independent, cost-effective, long-term substantial skin healing with no symptoms rebound has major implications in reducing expensive dermatological treatments or hospitalization time and overall expenses by offering a solution that enhances the quality of life. Rejuvenation and hair growth is also substantially important in enhancing self confidence and indirectly affecting interpersonal relationships and self esteem.

532 nm KTP laser vs. 595 nm PDL in the treatment of port-wine stains: a prospective, randomized, split-side study

Objectives: 1: Diagnose port-wine stains.
2: Understand the difference in efficacy and safety between KTP laser and PDL treatment for port-wine stains.
3: Manage and plan treatment approaches for port-wine stains with laser modalities.

Introduction: For port-wine stains (PWS), the pulsed dye laser (PDL) is currently the first-line treatment. However, due to the considerable maintenance expenses and technological instability associated with PDL systems, there is a need for alternatives. A recent addition to this field is the large-spot 532 nm KTP laser to address various vascular lesions, including PWS. This study aims to assess the effectiveness and safety of the 532 nm KTP laser in comparison to the 595 nm PDL for treating PWS.

Materials / method: A prospective, randomized, split-side study. Divided lesions were randomly assigned to the KTP laser or PDL, with 1 to 5 treatment sessions at intervals of 6 to 8 weeks. A control visit was scheduled 6 weeks after final session. Efficacy was evaluated by using computer-aided colorimetry, measurement of surface area changes, and clinical evaluation by two blinded board-certified dermatologists. Safety assessment involved pain intensity during treatment (numeric rating scale), tolerability, and patient satisfaction (5-point-scale).

Results: 35 patients were included in the study. Evaluation revealed significant lightening in erythema (?E = 8.25 after KTP laser vs ?E = 7.43 after PDL) and reduction of area (27.8 % after KTP laser vs 30.9 % after PDL). No significant differences in efficacy between both lasers were observed. Pain level was 2.7/10 for the KTP laser and 3.8/10 for the PDL treated sides, revealing no significant difference. The KTP laser caused significantly less post-treatment swelling, purpura, and crusts. No severe adverse events were observed for either laser. Both lasers indicated high patient satisfaction.

Conclusion: Both 532 nm KTP laser and 595 nm PDL are effective techniques for treating PWS. Due to comparable efficacy and safety profiles, lower downtime, and more stable technology, KTP laser may serve as a potential substitute to conventional PDL treatment. Further prospective clinical trials are warranted to assess long-term effects and adverse events of the 532 nm KTP laser.

Quantum mechanics principle and other physics principles involved in light and laser therapy

Objectives: Laser and light therapy have revolutionized the field of cosmetic medicine with their ability to safely and effectively treat a range of dermatological conditions. However, the theoretical principles underlying these modalities are often overlooked. In this article, we explore the key quantum mechanics principles and some physics principles involved in laser and light therapy.

As a team of doctors who are quantum mechanics enthusiasts, we discuss the concept of wave particle duality, whereby a photon can exist in two states simultaneously, allowing for its emission and absorption during laser

Introduction: In quantum mechanics, light is modeled as both a wave and a particle, known as a photon. When light interacts with matter, the behaviour of the photons is governed by the laws of qunatum mechanics.

Intense pulsed light and laser are technology that use spectrum of wavelengths to selectively traget certain chromophores in the skin, such as melanin or haemoglobin. This process is used for a variety of cosmetic and medical applications including hair removal, skin rejuvenation and treatment of vascular and pigmented lesions.

The interaction of laser and light in intense pulsed light with matter

Materials / method: Keywords including "wave-particle duality", "Pauli exclusion principle", "superposition", "spontaneous emission", "stimulated emission", "photo reaction", "Intense Pulsed Light", "Laser" were searched in the OVID, PubMed, and MEDLINE databases for relevant studies published on light and laser treatments. Some papers were further reviewed in order to fit the mechanism of principles involved in light and laser treatments. Clinical photos of daily practice were also included to allow better understanding how these reactions related to clinical practice.

Results: Different quantum mechanics principles (Wave-particle duality and Pauli exclusion principles) and other physics principles (Superposition, Spontaneous emission and Stimulated emission, Photochemical reaction, Photothermal reaction, Photomechanical reaction, photoionization) were explained with evidence in this presentation. By understanding these fundamental principles physicians can treat their patients better.

Conclusion: this presentation has highlighted the crucial role of quantum mechanics principles and some other physics principles in laser and light therapy. Quantum mechanics provides the theoretical framework necessary for understanding the behavior of photons and their interaction with tissues during these therapies. The principles of particle-wave duality, pauli exclusion principle and superposition are fundamental in explaining the mechanisms behind laser-induced tissue heating, selective photothermolysis, and the emission and absorption of photons.

Efficacy and safety of non-insulated microneedling radiofrequency (MNRF) alone or in combination treatments in the management of acne scars and trauma

Objectives: Efficacy and safety of non-insulated microneedling radiofrequency (MNRF) alone or in combination treatments in the management of acne scars and traumatic scars - Retrospective analysis of 50 Indian patients
We have been using multiple modalities over the years to treat scars at SkinSpace Clinic. This retrospective analysis is performed to establish efficacy and safety of Microneedle Radio Frequency Treatment.

Introduction: Presence of scars over face or other body areas following acne, trauma or other factors is highly disturbing to individuals. There is a significant loss of self esteem associated with presence of scars. Patients across the world seek betterment in appearance of scars. Managing scars is challenging owing to fibrosis, lack of blood supply to scar tissue and high patient expectations. Multiple modalities are available in the treatment of scars and understanding objective benefits of different modalities can largely help physicians to produce better outcomes.

Materials / method: Specifically designed assessment and treatment protocols were utilized at SkinSpace Clinic.
Demographic data of patients is collected and scar assessment was done using classification,Type of scars, location and age of scars.All patients received MNRF treatment using noninsulated micro needles once every 4 weeks for average 6-8 sessions.Few patients also received combination treatments like QSNDYAG laser and PRP. Physician observations and patient satisfaction (high, average, low) were recorded.Improvement in the scar depth, surface roughness and pigmentation was recorded by the physician.

Results: MNRF treatments were well tolerated by all patients. Scars depth reduced significantly in 50 % patients and skin surface texture improved in 80 % patients. It was observed that better results with same protocols were obtained in young scars, scars over cheeks, boxed and rolling scars. Improvement was slightly better when microneedling was combined with PRP injections and QSNDYAG laser based on patient characteristics. Over all 80% patients were highly satisfied after the treatment based on their personal feedback. Pain, PIH and acne like eruption were noted in a few patients.

Conclusion: MNRF is highly effective, well tolerated and safe treatment modality for acne scars and traumatic scars in Indian Patients. Results can be bettered with correct use of combination treatments in select group of patients.

Concomitant application of ultrasound energy and endogenous enzymes in facial contouring

Objectives: 1. To outline the best practices on the integrated treatment approach of ultrasound technology and endogenous enzymes in facial contouring
2. To outline the mechanism of action of ultrasound in lipolysis
3. To outline the mechanism of action of lipase, collagenase and hyaluronidase enzymes in lipolysis
4. To outline the value of objective evaluation of results through standardise 3D photography

Introduction: The optimisation of facial contours by non-surgical means often involves a multimodal approach, which in many cases, requires the reduction of lower face volume including the jowls and submental area. The concomitant use of ultrasound technology with the injections of endogenous enzymes, produced by genetic engineering, offers a well-tolerated, minimally invasive approach to refinement of the jawline. The ultrasound system is known to destabilise the integrity of the adipocyte membrane whereas the enzymes merely enhance lipolysis of the triglycerides within the adipocyte.

Materials / method: Thirteen patients were treated simultaneously with Ultrasound Technology and injected with a combination of the endogenous enzymes Lipase, Collagenase and Hyaluronidase, to treat excess volume in the lower face and submental area. Each patient received two to four treatments, spaced two weeks apart. A 3D imaging system was used for standardised photography and the objective assessment of lower facial volume, in each case.

Results: All patients who received the combined treatment of ultrasound and endogenous enzymes were shown objectively to have positive results in terms of volume reduction and the improvement of jawline contours. Furthermore, patient satisfaction was high. Most importantly, the combined treatment approach was not associated with any negative treatment emergent outcomes.

Conclusion: The concomitant application of ultrasound energy and endogenous enzyme showed a synergistic effect in terms of volume reduction and improvement in facial contours.
The combination treatment is both safe and effective.

Revolutionizing lip aesthetics: the "Lip Trio" protocol - A comprehensive approach to lip rejuvenation

Objectives: The main objective of this presentation is to introduce the novel "Lip Trio" protocol, aiming to provide attendees with:

2. Insights into the comparative advantages of "Lip Trio" over existing lip treatment modalities.
3. Demonstrated results of the "Lip Trio" treatment through case studies and client testimonials.

Introduction: Lip aesthetics have been at the forefront of facial rejuvenation for decades. However, the constantly evolving expectations of clients call for more comprehensive treatments that offer holistic rejuvenation rather than isolated interventions. The "Lip Trio" protocol is designed to address this gap by integrating laser technology, micro-needle radio frequency, and skin booster treatments, providing a well-rounded approach to lip aesthetics.The protocol involves three sessions, spaced one month apart. The three stages of the protocol are performed in the same session.

Materials / method: A retrospective study of 30 female patients (25-50 years old)
The "Lip Trio" protocol is implemented in three successive stages:
1. *Laser Treatment*: Utilizing slow thermal non ablative er:yag laser, this stage performs thermal stimulation of the tissue, and biosynthesis of fibroblasts.
2. **Micro-Needle Radio Frequency**: Enhances lip texture and stimulates collagen production.
3. **Skin Booster**: Instead of traditional injections, the skin booster is applied over the lips, utilizing the micro-channels created by the micro-needle RF; designed to restore the lips' natural hue and suppleness

Results: Improvement was measured in a follow-up visit a month after the final treatment; by patients' self satisfaction questionnaire scoring using the Likert Scale assessed improvement
Over a sample size of [30 patients], the following outcomes were observed post-treatment:
1. Improvement in lip texture in [87%] of the patients.
2. Enhanced lip color and suppleness observed in [85%] of the patients.
3. Improvement in Lip volume [93%] of the patients.
4. Overall Improvement [90%] of the patients.
Pain was graded using VAS Pain Scale
80% graded the pain between 0-2, 10% graded 3, 10% graded 4

Conclusion: The "Lip Trio" protocol presents a groundbreaking approach to lip aesthetics, bridging the gap between client expectations and tangible results. Its three-pronged approach ensures a comprehensive treatment outcome, making it a potential game-changer in the realm of aesthetic medicine. Lisbon and Oporto experience with subdermal laser for treatment of skin laxity

Objectives: The Authors located in the 2 Portygal´s major cities (Lisbon and Oporto) using this new tecnhique - 1470 nm Diode Sub-dermal laser, compareted their experiences in Face and neck Laxity, in terms of skin quality, thightening, non-surgical lifting effect.

Introduction: Skin Laxity occurs naturally with age progression, causing in many cases, disconfort , social impairment, and is a major cause of patient concern. It has many causes and tend to worsen in age. There are many medical approches to skin laxidy, from surgical Lifting in most severe cases, to non surgical medical treatment in litgh and mild cases. One of the most recent treatments is minical invasive sub-dermal laser specially 1470nm Diode Laser whitch cause skin retraction and stimualtion of new colagen fibers.

Materials / method: The authors used several scales to measure skin laxity in face and neck. Excluding severe laxity cases, whitch were referenced to plastic surgery face lifting.
The light and mild cases were submited to 1 or 2 session of minimal invasive Diode 1470 nm Sub-Dermal Laser, for skin thightening and redensification, followed by lymphatic drainage massage. The authors applied a local anesthesia, and a fiber using a fanning technique for aplying the laser treatments.

Results: Both in face and neck, skin laxidy improved most significantly in ligth and mild cases. The results wore more notable after 3/4 months after the last treatment and lasted for at least one year (depending on the previous laxidity status).
There were mininal side effects and general improvement of skin quality and thightening were notable in all patients.

Conclusion: The Diode 1470nm Laser permited a significant improvement with minimal/no downtime in skin quality, redensification and thightening, allowing a minimal invasive non-surgical aproach to skin laxidy both in face and neck.

Treatment of benign pigmented lesions with a picosecond laser using novel handpieces with 730 nm and 532 nm wavelengths

Objectives: This study assessed treatment with a picosecond laser, using a novel 730-nm picosecond titanium sapphire laser handpiece that specifically targets melanin and melanocytes and a fractional 532-nm handpiece with a ring of diffuse fluence emitted around each of the microbeams. The 532-nm wavelength targets more shallow/epidermal lesions, while the 730-nm wavelength targets deeper/dermal lesions.

Introduction: Q-switched (QS) nanosecond lasers are commonly used to treat benign pigmented lesions including dyschromia, but there is increased risk of post inflammatory hyperpigmentation (PIH) and scarring when treating skin of color.

Materials / method: Healthy adults with mild to severe benign pigmented lesions received up to four picosecond laser treatments, at 6-week intervals, to the face with the 532-nm handpiece and/or the 730-nm handpiece for both face and off-face treatments. Investigator assessment of pigment improvement was done at the 3-month follow-up post final treatment, using a 5-Point Pigment Improvement Scale (0=0% No improvement to 4=75-100% Excellent response). Subjects reported satisfaction with treatment outcome at the 3-month follow-up, using a 5-point Likert scale.

Results: 92 subjects with Skin Types II-V underwent treatments to the face and/or body for both epidermal and mixed epidermal and dermal pigmentation. Full-face treatments were performed with the 532-nm and focal treatments of face and body were performed with the 730-nm handpiece .80% responded positively to the treatment with the majority (79%) showing “Good” to Excellent” improvement. Edema, erythema, whitening and darkening of pigment resolved within several days. The overall rate for PIH and rebound melasma was 8% and resolved with 4% hydroquinone cream. No scarring was reported.

Conclusion: A series of 3-4 picosecond laser treatments, with the 532-nm handpiece, and focal treatments with the 730-nm handpiece to the face and body resulted in significant pigment improvement and high patient satisfaction with treatment outcome. PIH and rebound melasma rate was low in dark-skinned subjects. Longer-term follow-up in a larger study cohort is warranted.

Introduction: Microfocused ultrasound has become a popular modality for non-invasive facial rejuvenation. While linear applicators have been widely used for lower facial treatments, their application in the mid-face region has been limited due to anatomical challenges. In recent years, advancements in technology have introduced pen-like applicators, providing new opportunities for targeting mid-facial structures with increased precision and patient comfort.

Materials / method: The presented technique involves the use of microfocused ultrasound with pen-like applicators to target the nasolabial fat pad and malar eminence in the mid-face region. The application parameters include maximum energies of 0.5 to 0.7J, an optimized number of shots (600 to 800 per side), and precise anatomical localization. Pain levels and any adverse effects were recorded and analyzed.

Results: The results of this mid-facial rejuvenation technique with pen-like applicators were found to be consistent and natural-looking. Patient reported a noticeable reduction in nasolabial and palpebromalar grooves, resulting in a lighter, rested, and youthful appearance. Moreover, she expressed satisfaction with the outcomes and reported a decreased need for facial fillers in this area following the treatment. The technique demonstrated a high safety profile, with no adverse effects, such as pain and erythema, reported in this case.

Conclusion: The use of microfocused ultrasound with pen-like applicators represents a significant advancement in mid-facial rejuvenation. The presented technique, targeting the nasolabial fat pad and malar eminence, showed consistent and natural-looking results, providing patients with a refreshed appearance. This innovative approach allows for increased customization and patient comfort, potentially reducing the reliance on facial fillers in the mid-face region.
