Latrogenic alopecia post aesthetic procedures: Trichoscopic categorization and a novel clinical treatment approach

Objectives: The primary objective of this study is to categorize iatrogenic alopecia induced by non-surgical aesthetic procedures based on advanced trichoscopic findings. By delineating specific trichoscopic features, such as variation in follicular ostia, perifollicular discoloration, hair shaft caliber changes, and the presence of dystrophic hairs or cadaverized hairs, we aim to correlate these features with tailored therapeutic strategies. This categorization will aid in precise diagnosis and optimization of treatment approaches, addressing the pathophysiological nuances of each alopecia subtype.

Introduction: Iatrogenic alopecia following non-surgical aesthetic procedures represents a complex and often underappreciated clinical challenge. This study aims to establish a novel categorization system for iatrogenic alopecia, utilizing detailed trichoscopic analysis. Trichoscopy, with its ability to reveal subtle changes in follicular structure, perifollicular patterns, and hair shaft integrity, offers a precise diagnostic tool for differentiating types of alopecia. This categorization not only enhances diagnostic precision but also guides the implementation of targeted therapeutic protocols.

Materials / method: In this retrospective study, we analyzed 28 cases of iatrogenic alopecia subsequent to non-surgical aesthetic interventions. Trichoscopic evaluations were conducted with a focus on identifying distinct features such as: (1) Follicular Miniaturization with decreased hair shaft diameter and reduced terminal
hair ratio, (2) Perifollicular Hyperpigmentation or the presence of Peripilar Signs, indicative of inflammatory processes, (3) Ruptured Hair Shafts with trichorrhexis nodosa-like appearances and flame hairs, and (4) Yellow Dots with perifollicular fibrosis suggestive of chemical panniculitis

Results: four distinct: (1) Reduced Follicular Density and Absence of Terminal Anagen Follicles (21%), often caused by vascular compromise leading to ischemia and increased telogen effluvium; (2) Brittle and Miniaturized Hair with Peripilar Sign (11%), primarily due to chronic inflammation and chemical irritation from mesotherapy or other injectable treatments; (3) Ruptured Hair Shafts with Perifollicular Inflammation (25%), resulting from structural damage due to physical or chemical trauma; and (4) Yellow Dots with Perifollicular Fibrosis (14%), typically following panniculitis from fat-dissolving.

Conclusion: This study presents a novel trichoscopic categorization of iatrogenic alopecia resulting from non-surgical aesthetic procedures, linking each category to its underlying cause and appropriate therapeutic strategy. By using advanced trichoscopic criteria and understanding the etiological mechanisms, clinicians can more accurately diagnose and treat iatrogenic alopecia, resulting in better patient outcomes. This approach represents a significant advancement in the management of alopecia, offering a precision medicine framework tailored to the unique needs of each patient.

Prosthetic hair injection: A novel treatment for androgenetic alopecia - a case series study

Objectives: A retrospective analysis of the efficacy and safety of prosthetic hair restoration in 20 woman who underwent this procedure on our clinic.

Introduction: Alopecia is a common medical condition affecting both sexes. Androgenetic alopecia is one of the most common conditions and is a stressful experience for both sexes, but substantially more distressing for women. Women with androgenetic alopecia possess a more negative body image and a pattern of less adaptive functioning. Although this is a verry common condition we still lack a non surgical effective treatment and Surgical Restoration remains the only reliable method to restore hair in women. Therefore there is a need for an additional non-surgical treatment option.

Materials / method: 20 patients (ages 45-68) underwent prosthetic hair injection. All patients underwent a test procedure in which 48 hairs were implanted to make sure there is no adverse reaction and 6 weeks later underwent full treatment. Patients underwent between 1-3 treatment sessions with prosthetic hair fibers ( by Hairstetics LTD). Hair implants is made of two biocompatible materials: nitinol and nylon and are preloaded on sterile disposable cartridges 12 hair fibers on each. All patients were injected with 768-1152 at each treatment session. Side effects and patient satisfaction were monitoed.

Results: All patients that were chosen to undergo this treatment had good clinical improvement and over 80% mentioned that it improved their self confidence and body image. Side effects were mostly mild and included focal erythema and papular lesions (in 4 patients overall). No patients needed an extraction of the implants.

Conclusion: Prosthetic hair injection is a safe and effective procedure mainly for woman with stable androgenetic alopecia- since there is a need for a reliable minimally invasive procedure .It can serve as an additional tool for patients which are looking for an office procedure rather a surgical one. Patient selection is crucial for positive outcome.

Hair transplant with micrografting technology

Objectives: Very high success rate of transplanted hair when combining with autologous Micrografting technology specifically in the early 48 hours of hair transplant

Introduction: Many people around the world suffer from Androgenetic Alopecia (AGA). There are well-known and approved medications such as Minoxidil and Finasteride, in addition to hair transplant surgery. Recently the use of micrografting technology showed promising results when used for patients with specific ddconditions. There are constraints when it comes to the benefits of using this technology.

Materials / method: Micrografting achieves autologous cell suspension by mechanical fragmentation of subcutaneous and adipose tissue from the occipital area. The use of the safe micrografting technology in the first 48 hr of transplantation surgery can give us high success rate of transplanted hair

Results: High success rate of transplanted hair
Decrease or Stop hair loss for Androgenic alopecia
Thickening of the hair
Regrowing of hair fibers from the viable hair grafts

Conclusion: After treating over 2700 androgenic alopecia patients in my clinic using autologous micrografting technology and by applying a specific treatment protocol for each case; depending of the remaining viable follicles, or with hair transplantation surgey;
Promising results were achieved.
Furthermore, the use of micrografting technology is a treatment for all parts of the head with minimum to null recovery time.

Hair restoration with combination techniques

Objectives: This study aimed to confirm the combination of Follicular Unit Extraction (FUE) and nanofat to enhance the density results in high grade baldness patients

Introduction: Male Androgenetic Alopecia (MAGA) has been one of the most common reasons for consultation. Shaven FUE has been an attractive treatment option for patterns with severe AGA. FUE hair transplant is becoming most wanted with demand for higher density and coverage of baldness. It has evolved mainly with the aim of making the results look more natural with no scar. In high grade of baldness,scalp donor has limit to fulfill the required number of follicles for Norwood grade V and VI baldness. Nanofat can use combine with FUE for stimulate hair growth and prevent shock loss

Materials / method: Fifty male patients with poor density AGA in Hamilton Norwood Grade V-VI were enrolled in the research. All participants underwent one nanofat session and two FUE hair transplantation sessions. Nanofats were inject all scalp areas one month before the first session of FUE hair transplantation. The second session of FUE hair transplants were done after first session 6-9 months. All patients were general conditions. Assessment were evaluated with photography.

Results: Combination is a good result answer to cover higher grades of baldness. We harvested an average of 2500± grafts per session.Recipient density ranged from 40 FUE/cm2 to 50 FUE/cm2.Nanofat can effectively lead to natural density and appearance in one session. Nanofat helps enhance hair growth. The combination of FUE hair transplant and nanofat can improve high grade baldness that a large number of grafts. It can use combine with shaven FUE to cover up frontal and crown area balding.

Conclusion: According to the outcomes, nanofat and shaven FUE is satisfactory treatment for patients with high grade baldness. Nanofat contains abundant stromal vascular fraction cells and adipose-derived stem cells. Adiposed derived stem cells were still richly present in nanofat. It contains angiogenic growth factors and other molegules that can promote the growth of new blood vessels, which are essential for the nourishment of hair follicles. It provides high success rates for hair transplantation. Combined treatment with nanofat and and FUE is more effective than alone.

Retrospective assessment of nutritional factors and non-surgical multimodal therapy in non-cicatricial alopecia

Objectives: The objective of this study is to examine the biochemical, nutritional, and lifestyle contributors to hair loss and evaluate the role of multimodal therapy in managing non-cicatricial alopecia. Specifically,
it aims to:
-Identify metabolic and nutritional deficiencies—such as elevated triglycerides, cholesterol, and deficiencies in Vitamin D3, Vitamin B12, and iron—potentially linked to hair loss, with a focus on gender differences.
-Assess the efficacy of an integrative treatment approach that combines autologous multi-spin PRP, topical minoxidil, oral finasteride, and lifestyle adjustments

Introduction: This study investigates hair loss as a condition influenced by various factors, including genetics, nutrition, and lifestyle, aiming to evaluate both biochemical assessments and the efficacy of multimodal therapy.

Materials / method: Laboratory Analysis:Blood samples from 594 patients were analyzed after an 8-hour fast to assess triglycerides, cholesterol, Vitamin D3, Vitamin B12, and iron levels, exploring metabolic and nutritional contributors to hair loss.
Multimodal Therapy: A subset of 350 patients were selected, classified by Norwood-Hamilton and Ludwig scales, received eight sessions of therapy including PRP, topical minoxidil, and oral finasteride along with nutritional deficiencies corrections (if) required, along with life style modifications to target non-cicatricial alopecia.

Results: Laboratory Findings: High levels of triglycerides and cholesterol were prevalent among male patients, indicating a possible link to hair loss. Vitamin D3, Vitamin B12, and iron deficiencies were common, especially among males, underscoring nutritional factors in hair loss.

Therapy Outcomes: Noticeable improvements in hair density and diameter were achieved in patients with lower-grade alopecia. Advanced cases (Grades 6-7 in males and Grade 3 in females) showed moderate improvement, reflecting mixed patient satisfaction.

Conclusion: Laboratory screening for metabolic and nutritional deficiencies in patients with hair loss is valuable. The effectiveness of multimodal therapy reinforces the importance of a comprehensive approach, combining multiple spinning PRP, pharmacologic, and lifestyle modifications to manage non-cicatricial alopecia.

Injectable platelet-rich fibrin and its results in female pattern hair loss

Objectives: Female pattern hair loss (FPHL) is a common form of non-scarring hair and often a source of significant psychological distress. Autologous platelet aggregates have been proposed as a potential therapy to treat FPHL, either alone or in combination treatment. In this study, we aimed to evaluate the application of injectable platelet-rich fibrin (i-PRF) as a novel hair loss solution, with a review of literature data.

Introduction: Various trials have been conducted on the management of female pattern hair loss, but the outcomes often seem to be limited. Platelet-rich plasma (PRP) has a well-established role as adjunctive therapy in androgenetic alopecia but there are many limitations of it. In an attempt to overcome the shortcomings of PRP, liquid platelet-rich fibrin (PRF) is guaranteeing a prolonged release of growth factors and cytokines and a three-dimensional scaffold structure for hair cells regrowth.

Materials / method: 11 patients were enrolled in this scalp trial underwent 4-monthly intradermal injections of i-PRF. Under all aseptic precautions, the tubes were centrifuged at 800 rpm for four minutes. The i-PRF obtained was injected in deep dermal area using point by point and nappage technique. Patients have completed total of 4 sessions, at an interval of one month. Maintenance sessions were proposed 4 times per year. Mean hair count and hair thickness, global assessment by the investigators, subject self-assessment and adverse effects were assessed.

Results: The mean number of hair follicles containing hairs per unit area improved at 3- and 6-months follow-up (p < .001), and all participants had a negative hair pull test. Hair volume and thickness, and patient-reported outcome scores also improved at follow-up (p < .001). Adverse effects were minor and self-limited. A series of three i-PRF injection sessions were effective for the treatment of FPHL, as shown by improved hair analysis parameters (in both V-scalp points and F-scalp points) and patient self-assessment scores.

Conclusion: Injectable platelet rich fibrin (i-PRF) seems to be an effective therapy and safe autologous biomaterial for androgenetic alopecia . To ascertain and potentiate our findings, large scale studies are required.

Systematic approach to diagnosing and treating hair loss: The role of trichoscopy, PRP, and laser therapy

Objectives: In this presentation, I will provide physicians with a systematic approach to diagnosing and treating common hair disorders, based on my extensive experience as a practitioner, trainer, and advisor in hair conditions. The role of trichoscopy in diagnostics will be highlighted. Several case studies will be presented, where patients were treated with platelet-rich plasma (PRP) and/or Er:YAG laser therapy. PRP and LASER are two modalities that have garnered significant interest from both patients and hair specialists in recent years. For this reason alone, they deserve our attention.

Introduction: Globally, hundreds of millions of men and women are affected by hair loss. Only a minority seek medical help; many choose not to start medical treatment or discontinue it prematurely. Patients often search for solutions online, falling prey to quack remedies that promise miracle cures. Physicians who aim to treat these patients must be prepared to become experts in hair loss. By following a systematic approach—comprising rigorous diagnostics and a selection of scientifically proven treatments —they can guide patients through the complex pathology of various hair conditions.

Materials / method: Using a variety of case studies, diagnostic pathways and biostimulatory treatments will be presented. Trichoscopy allows for the rapid and accurate identification of pathognomonic features of conditions like androgenetic alopecia. The use of artificial intelligence (AI) further provides an accurate overview of the patient’s current hair status. Treatments with PRP and Class 4 Er:YAG laser will be described in detail. The rationale behind therapy selection, the development of specific treatment protocols, and the methods of follow-up and evaluation will be discussed.

Results: The results of these treatments will be thoroughly discussed, supported by standardized before-and-after photographs.

Conclusion: The days when patients had to helplessly watch their hair gradually deteriorate are definitively behind us. For many patients, hair preservation is within reach. Physicians interested in treating these patients must become proficient in the complexities of hair loss, its various entities, and the appropriate treatments. Accurate diagnosis is crucial, and several treatment options have been scientifically researched, although the search for the optimal combination of therapies continues to evolve.

Each stage is under a mono and a mix therapies
Starting with “ minoxidil, exozomes, svf , hair dust cells& natural hair implant “ in more advanced casses when hair fue implant is excluded because of the donor area or scars we combine it with svf of biocompatible fibres.
The groups are watched under studies for 4 years.
Following the evolution, the success rate the outcoming results, and the longitivity of the results .

Results: - in fibrotic scars svf & exozomes created new microcirculation. After hair dust cells, new hair follcilles are noted and the installed of implanted hair is lore successful .
- grade 1,2,3 of hamilton mostly be restored with minim fue implant, more svf & exozomes sessions .
- svf & hair dust can prevent for a while progressing of alopecia in first grades especially front lines.
- combining svf and micro hair dust with fue requires a smaller donor area to be implanted to achieve a suitable result.Less follicles than fue as a mono therapy.

Conclusion: Svf therapy mixed with natural autologous blood exozomes and hair dus cells are modern therapies that may prevent follicle loose and create new follicles especially hamilton 1,2,3
In hamilton 3 it can boost the regeneration by using fue natural implant but the results are observed more density seen in the treated area.
