Comparative study of prenatal MSC-derived exosomes and hyaluronic acid in enhancing skin hydration, reducing erythema, and improving elasticity follow

Objectives: The primary objective of this study is to compare the effects of 120-nanometer prenatal mesenchymal stem cell-derived exosomes with non-crosslinked hyaluronic acid on key dermatological parameters, including transepidermal water loss (TEWL), erythema reduction, melanin content reduction, and skin elasticity improvement. The secondary objective is to assess the overall patient satisfaction and safety profile of both treatments when applied alongside microneedling sessions spaced four weeks apart, with results monitored one month post-treatment.

Introduction: Microneedling has gained popularity for enhancing skin absorption and boosting regenerative processes, often paired with topical treatments like hyaluronic acid (HA) to improve hydration and skin texture. However, prenatal MSC-derived exosomes may offer superior benefits due to their ability to deliver growth factors, cytokines, and miRNAs directly into the skin. This study seeks to investigate whether prenatal MSC-derived exosomes outperform non-crosslinked HA in reducing TEWL, erythema, and melanin levels, while also improving skin elasticity in patients treated with microneedling.

Materials / method: This randomized, controlled study involved 44 patients with visible skin aging signs, divided into two groups (22 per group). Group A received 120-nanometer prenatal MSC-derived exosomes, while Group B received non-crosslinked hyaluronic acid. Both treatments were applied after microneedling (using <1.5mm needles) at four-week intervals for three sessions. TEWL, erythema, melanin, and skin elasticity were measured with a Tewameter, Mexameter, and Cutometer. Results were monitored one month post-treatment. Results: In Group A (exosomes), TEWL decreased by 18.5%, erythema reduced by 25%, melanin levels dropped by 20%, and skin elasticity improved by 22%. In contrast, Group B (HA) showed a 10.3% decrease in TEWL, 15% reduction in erythema, 12% melanin reduction, and 13% improvement in elasticity. Statistically significant differences between the two groups were observed across all parameters (p<0.05), with prenatal MSC-derived exosomes demonstrating superior performance in reducing TEWL, erythema, melanin content, and improving skin elasticity compared to HA.

Conclusion: This study concludes that prenatal MSC-derived exosomes significantly outperform non-crosslinked hyaluronic acid in reducing TEWL, erythema, and melanin levels, while enhancing skin elasticity when combined with microneedling. The exosomes' superior regenerative potential, likely due to their enriched content of bioactive molecules, makes them a more effective treatment for improving skin health. These findings suggest that exosome therapy may offer a more comprehensive skin rejuvenation strategy than HA, particularly for patients seeking advanced anti-aging interventions.

Exploring the facial skin rejuvenation properties of a TGF-β biomimetic miniprotein

Objectives: Our objective has been first to demonstrate in vitro on normal human fibroblasts and at the clinical level the skin's rejuvenating effects of a miniprotein having TGF-β properties.

Introduction: Skin aging, which results from intrinsic and extrinsic factors, is characterized by a rough, uneven and wrinkled appearance at the macroscopic level. At the microscopic level, aging shows lowered keratinocyte turnover, flattened dermal-epidermal junction and reduced collagen fiber density, however, the use of skin biopsies to evaluate ingredient properties on these microscopic changes are too much constringent for panelists and rarely used. The development of performant non-invasive technics is an opportunity to be considered for such evaluation.

Materials / method: In vitro we quantified collagen type I, hyaluronic acid and elastin. At the clinical level, we used non-invasive technics, the Confocal Laser Scanning Microscope which enabled morphological characterization of skin structures (stratum corneum , epidermis, dermal-epidermal junction ..) and high-frequency ultrasonography to quantify the dermal density and thickness. The cutometer assessed the skin biomechanical properties. A double-blind, split-face, randomized, placebo-controlled clinical trial compared the miniprotein versus vehicle on an hemi face design.

Results: In vitro results confirmed the ability of the miniprotein to stimulate collagen type I, hyaluronic acid and elastin, at a level comparable to that induced by the growth factor TGF beta. The clinical data showed that after 28 and 56 days of miniprotein topical application, a significant increase of the DEJ length, the number of edged papillae and the collagen fiber perimeters. B-scan images of face skin showed a significant increase of the dermis density and the dermis thickness. The rejuvenation was also confirmed by an improvement of skin firmness and elasticity.

Conclusion: Altogether, the in vitro and clinical results presented in this paper show that this miniprotein is a potent ingredient to rejuvenate the DEJ and the dermis of mature skins.

Introduction: Regenerative medicine is pivotal in today's aesthetic industry, with patients seeking natural, youthful results. The key question is whether polynucleotides, growth factors, exosomes, and biosomes are essential for skin rejuvenation. Understanding their mechanisms and molecular biology is crucial, as is exploring their role in treating various conditions. Additionally, determining whether a robust exosome containing multiple growth factors is more effective than a targeted subset is vital for optimizing regenerative treatments in aesthetic medicine.

Materials / method: A literature review was conducted to evaluate the knowledge exist today about growth factors, exosomes, and biosomes. This knowledge was combined with clinical studies and practical experience from my clinics.

Results: In regenerative aesthetic medicine, integrating polynucleotides, growth factors, exosomes, and biosomes is crucial for optimal results. Each has unique advantages and drawbacks, requiring a deep understanding to tailor treatments to individual patients. Polynucleotides enhance the cellular microenvironment, while growth factors stimulate specific functions. Exosomes offer potent stimulation but are unpredictable due to their heterogeneity. Biosomes, an evolution of exosomes, allow synthetic delivery of specific growth factors, enabling precise treatments, such as selectively using TGF-BETA 3.

Conclusion: • Employ a diverse range of products for skin regeneration, exercising judiciousness by comprehending the purpose behind each application or injection. Ensure a meticulous match between the chosen product and the specific indication for optimal results.
• The future of regenerative aesthetic medicine might lie in biosomes, offering a combination of a targeted subset of growth factors and robust natural stimulation. This innovation enables the simultaneous delivery of both precise and powerful regenerative stimuli.

Advanced wound care post-surgical harvested skin: A network meta-analysis of randomized controlled trials

Objectives: This network meta-analysis aimed to compare different conventional dressing options based on re-epithelialization time and side effects for split-thickness skin graft donor sites.

Introduction: Over 160,000 skin grafts are conducted annually in approximately one out of every three burn hospitalizations, since they are primarily simple and quick. This network meta-analysis aimed to compare different conventional dressing options based on re-epithelialization time and side effects for split-thickness skin graft donor sites.

Materials / method: A thorough systematic review and network meta-analysis was conducted in accordance with PRISMA guidelines. A comprehensive search was performed through Medline, Cochrane, and Embase databases, up to the year 2023 without time limitation.

Results: This study performed a network meta-analysis of 25 unique randomized controlled trials published between 1983 up to 2022. We found that Alginate is the most frequently used, followed by Paraffin gauze. Nonetheless, Povidone-Iodine was the most effective (90.4%), followed by hydrocolloid (87.9%). Petrolatum (73.5%) is associated with faster re-epithelization than alginate (40.6%) and paraffin (18.1%). Cotton gauze ranked as the worst modality for skin-graft donor-site dressing (3.9%).

Conclusion: This study serves as a guide for the selection of proper dressing for patients undergoing partial-thickness skin-graft harvest. Even though povidone-iodine was the most effective, we recommend further research comparing other factors for selecting dressings, such as wound and patient characteristics, availability, and costs of dressings.

Different faces of PRP therapy: Treatment complications after BoNX injections - Brow ptosis, edema, face asymmetry

Objectives: 1. Different faces of PRP-therapy. Not only effective anti-age treatment, but one of the best treatments of common complications after botulinum toxin injections ( BoNT) in comparison with traditional physiotherapy methods.

2. Different types of PRP / different biostimulation effects (the important role of platelet count , white blood cell (WBC) content, red blood cell (RBC) contamination, and PGF concentration)

3. Common complications after BoNX injections ( brow ptosis, edema, face asymmetry after full-face injections)
4. PPP ( pure plated plasma with high red blood cells ( RBC)

Introduction: PRP therapy have been used for more than 30 years, resulting in considerable interest in the potential of autologous PRP in regenerative medicine.
White blood cell (WBC) content, red blood cell (RBC) contamination, and PGF concentrations play the main role in the bio stimulation effects of PRP-therapy. But it’s not only anti-aging and regenerative treatment. In our research we want to notice the perfect outcomes of PPP therapy treatment ( pure plated plasma with high concentration RBC) complications after botulinum toxin injection ( brow ptosis, face asymmetry after full-face BoNX )

Materials / method: We divided 36 patients with complications after full-face botulinotoxin injections for two groups.
All patient’s were female, medium age 30-40, without any chronic diseases. All complications were mild and common after BoNX infection ( brow ptosis, face edema and lip/face asymmetry after low face treatment). First group was treated with standard treatment ( RF/physiotherapy for improving micro circulation and vit B6,B12 injections). Second group was treated only with PPP fraction injection , 2-3 session in 5 days.

Results: We achieved incredible results in second group. Only after few sessions ( 2-3 session of pure plated plasma (PPP-plasma) we notice rapid improvement-microcirculation, returning muscle construction and activation lymphatic system . In the first group the same results we achieved only after 6-8 sessions of skin toning and heating treatments ( RF, micro toning, vit B 6,12 intradermal injections).
Very important the method of PPP -fraction preparation. In our research we use RegenLab blue tubes with lower level of centrifugation ( 1500/5 min)

Conclusion: Botulinum toxin is the most popular type of aesthetic procedure that is gaining popularity due to this relatively fast effect and recovery time. Although the overall side effects profile of such procedure is low. But popular techniques BoNX full-face injections often can cause edema and face asymmetry. One of the fastest and effective treatment is 2-3 session of PPP-therapy ( pure fraction PRP for muscle bio stimulation, improving vessels microcirculation and lymph flow). We achieved rapid and incredible results without any recovery time and high patients satisfaction .

Exosomes in dermatology and aesthetic medicine: An update

Objectives: This study aims to provide a comprehensive overview of exosomes' emerging role in dermatology and aesthetic medicine, highlighting their potential therapeutic applications.

Introduction: Exosomes are nanoscale vesicles derived from cells, recognized in regenerative medicine for their capability to transport diverse biomolecules such as proteins, nucleic acids, and lipids. These vesicles are key mediators of intercellular. communication, impacting critical cellular processes including proliferation, differentiation, migration, and cell death.

Materials / method: Leveraging our extensive experience in exosome-based treatments for over 2000 patients, this study synthesizes recent research to outline the mechanisms and potential applications of exosomes in various dermatological and aesthetic procedures.

Results: Exosomes show promise as therapeutic agents in a broad spectrum of dermatological and aesthetic interventions, including wound healing, scar management, skin aging reversal, facial rejuvenation, pigmentation issues, psoriasis, atopic
dermatitis, vitiligo, lupus, sclerosis, and hair growth. Their multifaceted
mechanisms offer versatile solutions for numerous dermatological conditions and aesthetic improvements.

Conclusion: The growing research on exosomes underscores their transformative potential in dermatology and aesthetic medicine. Harnessing exosomes' regenerative properties could revolutionize treatment approaches for various skin conditions and aesthetic. needs. Further research is encouraged to fully unlock their therapeutic
potential and innovate clinical applications.

The efficacy and safety of ASC-exosomes in androgenetic alopecia based on the translational study including preclinical and clinical study

Objectives: 1. To understand that exosomes derived from adipose stem cells (ASCE) can be a paradigm shift in regenerative aesthetics & therapeutics

Introduction: Exosomes are nano-sized (30 - 200 nm in diameter) lipid bilayered vesicles secreted by most cell types. Adipose Stem Cell-derived Exosomes (ASCE) are emerging as a promising cell-free therapy for the treatment of hair loss and scalp rejuvenation. ASCE has been tested for in vitro, in vivo, & clinical studies to prove its efficacy for hair loss. To understand that adipose stem cell exosome (ASCE) can be a paradigm shift in regenerative aesthetics & therapeutics. To know that ASCE can make scalp rejuvenation and hair loss prevention.

Materials / method: ASC-Exosomes were isolated from the serum-free conditioned media by sequential filtration and characterized as recommended by the International Society for Extracellular Vesicles (ISEV). The effects of ASC-exosomes were demonstrated from a variety of in vivo, in vitro, & clinical studies against dermal papilla cells and human hair follicles, in order to show longer hairs and better hair bulb shape. In our clinical study, 29 subjects were treated with microneedling procedure with topical ASCE treatment for 6 months.

Results: ASCE labeled with green fluorescence was easily up-taken by human dermal papilla cells in vitro. ASCE could reduce by up to 20% the level of cellular aging of human dermal papilla cells from UVB irradiation in vitro. ASCE could effectively increase by 10-20 % the length and thickness of human hairs derived from ex vivo human hair follicles for 8 days, which was superior to minoxidil. ASCE-containing formulation showed strong improvement in clinical cases of androgenic alopecia. The ASCE-containing formulation showed the maintenance of the shape of human hair follicles from an ex-vivo study.

Conclusion: ASCE may serve as next-generation technology in terms of regeneration and anti-inflammation for scalp and hair.

Study of the effect of the combined drug PN and HA on the regeneration of collagen fibers of the skin in an experiment

Objectives: To study the therapeutic effect of the polynucleotides and hyaluronic acid combination on the healing of skin wounds in laboratory animals.

Introduction: The scientifically justified use of drugs containing polydeoxyribonucleotide (PN /PDRN) to stimulate various regeneration links has been confirmed by repeated publications in medical literature. The pharmacological properties of PN and hyaluronic acid (HA) make it clear that, when combined, these drugs will synergistically stimulate skin healing. However, there is no information about the study of the therapeutic effect of the PN/HA combination on the healing of skin wounds yet.

Materials / method: The process of collagen fiber (CF) regeneration under the influence of a combination of polynucleotides and hyaluronic acid was studied on an experimental model of a scalp wound in 17 white mongrel rats. When analyzing the results obtained: macro- and microscopic indicators, morphometric indicators of collagen fibers in the regeneration area, the dynamics of skin CF restoration were assessed and compared in two groups: the study group, where the combined drug PN/HA was used, and in the comparison group, where a 0.9% NaCl solution was used.

Results: In PN/HA group, the dynamics of maturation of CF was ahead of that of the comparison group by an average of 7 days. In PN/HA group, restoration of skin appendages was noted in the regeneration zone by the 21rst day, a week earlier than the comparison group. During all periods of observation, CF in the area of the scalped wound were formed with an advantage in diameter and accelerated maturation, ahead of the comparison group by 7 days. Scar hypertrophy was not detected in the PN/HA group.

Conclusion: Earlier epithelialization of the wound and maturation of the collagen fiber, the normotrophic nature of scarring in the PN/HA group indicates more favorable conditions for regeneration under the influence of the combined drug PN/HA.
