The coronavirus pandemic (COVID-19) has served as a call-to-arms in preparing practices for the next disaster whether it is another infectious disease or a flood, hurricane, earthquake, a sustained power outage, or something else. A group of predominantly core aesthetic physicians discussed the various aspects of their office procedures that warrant consideration in a proactive approach to the next pandemic/disaster-related event. This guide does not set a standard of practice but contains recommendations that may avoid some of the "lessons learned" with the COVID-19 pandemic. In this paper, the board-certified core aesthetic physicians classified these recommendations into four generalized areas: Practice Management; Supplies and Inventory; Office Staffing Considerations and Protocols; and Patient Management Strategies. Proactive strategies are provided in each of these categories that, if implemented, may alleviate the processes involved with an efficient office closure and reopening process including, in the case of COVID-19, methods to reduce the risk of transmission to doctors, staff, and patients. Ethylene oxide (EtO) is a known carcinogen and mutagen associated with increased incidence of breast and blood cancers. The largest medical sterilization facility in Michigan had been assessed by the U.S. Environmental Protection Agency as imposing an additional cancer risk greater than one in one thousand in nearby neighborhoods. This prompted the Michigan Department of Environmental Quality (now referred to as the Department of Environment, Great Lakes, and Energy) to conduct an air quality modeling study of the ambient EtO impacts of the sterilization facility, followed by 24 h Summa canister sampling and TO-15 analysis in two phases. Inverse modeling of the measured 24 h EtO concentrations during the second phase yielded estimates of 594 lbs/year for the facility's total emissions of EtO and 0.247 µg/m for the urban background concentration. The inverse-modeled emissions are similar to reported emissions by the facility operator based on indoor air measurements and simple mass balance assumptions, while the inferred background concentration agrees with estimates from other field investigations. The estimated peak 24 h exposure to EtO caused by the sterilization facility in nearby neighborhoods was 1.83 μg/m above the background level, corresponding to an additional cancer risk of approximately one in one hundred, if assumed to represent annual mean exposure. read more

Non-surgical fat reduction procedures have gained in popularity over the past few years and remain in great demand. The process results in accumulation of breakdown products, lipid droplets, that are slowly absorbed over a period of months. This paper outlines the physiological process whereby lipid droplets are absorbed through a process of autophagy (lipophagy) involving a repackaging of these droplets to smaller sizes so that macrophages can then cope with digestion of these very large particles. Furthermore, a fat compartment is described within the dermis surrounding the tail of the hair follicle, which is attracting much attention due to its unique phenotype, function, and connection to the deeper subcutaneous fat compartment. This provides an entry route for direct signaling to the subcutaneous fat. Related to these novel concepts, peptides can be designed in liposomal delivery systems to target lipid droplet breakdown via the hair follicle entry route. This concept is elucidated in this paper. Non-ablative fractional laser procedures have become increasingly popular since their introduction in 2004. The fractional 1,927 nm thulium laser is a non-ablative device that penetrates up to 300 μm in the skin and the 1,550 nm erbium:glass laser penetrates up to 1,400 μm. These procedures are considered minimally invasive with a high safety profile; therefore, infectious complications are exceedingly rare. However, we report five recent cases of bacterial infection with both gram-positive and gram-negative organisms following treatment with the fractional 1550/1927 nm laser approximately 1 day to 1 week post-procedure. One patient had a rapidly progressing pustular eruption with symptoms of sepsis. These patients were seen immediately, cultures were obtained and empiric antibiotic therapy was initiated. They recovered without long-term complications. Rapid-onset bacterial infections following non-ablative laser resurfacing with the 1550/1927 nm laser have not been previously reported in the literature. The infections can progress quickly and lead to serious sequelae, including systemic illness and severe scarring, if not identified and appropriately treated. We present these cases to highlight the importance of close surveillance and when appropriate, rapid intervention, following non-ablative fractional procedures, especially when patients present with atypical symptoms and signs. read more

A new modality, the Portrait plasma skin regeneration (PSR(3)) system, allows precise and rapid treatment of photo-damaged skin, with controlled thermal injury and modification. Radio frequency (RF) energy converts nitrogen gas into plasma within the handpiece. Rapid heating of the skin occurs as the plasma rapidly gives up energy to the skin. This energy transfer is not chromophore dependent. The gold standard, carbon dioxide (CO(2)) laser resurfacing, has decreased in popularity due to high morbidity and downtime. There is demand for a technology that can provide the degree of improvement obtained with resurfacing without the complications associated with its use. This study evaluated the PSR(3) technology in full facial procedures. A two-site prospective study evaluated safety and efficacy for a single pass treatment of the full face using the Portrait PSR(3) system. Improvement in skin texture, tone, fine lines, dyschromia, and rhytides were assessed. Two-millimeter punch biopsy specimens were taken pre- and 90 days post-treatment. Follow-up was performed at days 2, 5, 7, 30, and 90 post-treatment to monitor recovery, improvement, and any subsequent sequelae. Patients developed erythema and edema shortly after treatment, with no immediate epidermal loss or charring. Epidermal loss occurred in the subsequent 24-48 h followed by epidermal recovery in approximately 7 days. Histological investigation showed regenerative epidermal and dermal architecture. The Rhytec Portrait PSR(3) system provides an attractive alternative to standard lasers that is well tolerated by patients, stimulates collagen remodeling, and provides excellent clinical outcomes. read more
