Comparative evaluation of mechanical projection vs biostimulatory remodeling in superficial midface lifting

Objetivos: Midface aging in Asian patients is characterized not only by volume depletion but also by progressive ligamentous attenuation, superficial fat compartment descent, and alteration of malar structural support. Unlike Caucasian anatomy, Asian midface morphology often presents with broader zygomatic projection, relatively thicker dermis, and distinct soft tissue biomechanics. These anatomical characteristics may influence lifting vectors, tissue responsiveness, and long term remodeling outcomes. However, comparative data evaluating mechanical versus regenerative driven lifting strategie specifical Introducción: This study proposes a comparative evaluation of two fundamentally different lifting paradigms: immediate structural projection using high elastic modulus (G′) (HA) fillers and progressive biostimulatory remodeling using (PCL) microspheres suspended in (CMC). Within an anti-aging context, these approaches represent mechanical repositioning versus regenerative tissue reinforcement. The innovation of this work lies in directly comparing viscoelastic structural elevation and collagen-mediated remodeling within standardized superficial midface vector in Asian cohorts. Materiales / método: In this prospective preliminary investigation, ≥20 Asian patients presenting with early to moderate midface descent underwent superficial lateral midface lifting along standardized zygomatic and lateral midface vectors. Participants were allocated to either high G′ HA filler (mechanical projection group) or PCL with CMC (biostimulatory group). Assessments were performed at baseline, immediately post-treatment, and at 2-month follow-up. Quantitative lifting magnitude and vector displacement were analyzed using three-dimensional imaging (QuantifiCare system). Resultados: Immediate post treatment analysis demonstrated comparable lifting magnitude between groups, with greater early projection observed in the high G′ HA cohort, consistent with its higher elastic modulus and structural dominance. However, at 2 months, the PCL with CMC group exhibited sustained vector maintenance and increased dermal–subcutaneous thickness on ultrasound. Progressive enhancement of fibrous septae architecture suggested active neocollagenesis following gradual CMC resorption. Conclusión: In conclusion, understanding the balance between mechanical projection and biologically mediated remodeling is essential in designing personalized anti-aging strategies for Asian patients. The differentiation between structural lift and regenerative lift may redefine treatment algorithms in midface rejuvenation. Larger studies with extended follow up are warranted to further validate long-term biomechanical and histologic outcomes.
