Combined 1444-nm fiber laser and ultrasound for multi-layer facial and neck tightening

Objetivos: To present the rationale, technique, clinical outcomes, and safety profile of combined 1444-nm fiber laser and ultrasound treatment for multi-layer tightening of the lower face and neck. Participants will learn patient selection, treatment design, anatomical targets, expected durability, and management of procedure-related complications. 이 문장은 뜻이: Introducción: Non-surgical facial rejuvenation devices often show limited durability because they primarily target a single anatomical layer. Ultrasound-based devices mainly affect the superficial musculoaponeurotic system (SMAS), whereas laser-based approaches are more effective in the dermis and subcutaneous tissue. A multi-layer treatment strategy may therefore provide more durable and clinically meaningful lifting. Materiales / método: This retrospective clinical analysis included 893 patients treated over a 9-year period. The mean patient age was 42.3 years. The treatment area extended from below the zygomatic arch to the cervical angle of the neck. The technique was designed to target multiple tissue planes simultaneously: dermal collagen remodeling and selective subcutaneous fat contraction using a 1444-nm fiber laser, together with SMAS tightening using ultrasound energy. Clinical outcomes, durability, retreatment patterns, and complications were assessed. The mean follow-up period was 39.2 months. Resultados: The combined multi-layer approach produced clinically significant lifting and tightening of the midface, lower face, jawline, and upper neck. Most patients maintained visible improvement for approximately 2 years, and commonly returned for repeat treatment within that interval. Only 3 patients required a touch-up procedure at 1 year because of insufficient clinical effect. Temporary facial nerve weakness occurred in 2 cases, with no permanent deficit reported. Conclusión: Combined 1444-nm fiber laser and ultrasound treatment is an effective multi-layer facial and neck tightening strategy with durable results and an acceptable safety profile. In this series of 893 patients with a mean follow-up of 39.2 months, the procedure demonstrated sustained improvement for 2 to 3 years in most patients, a very low early touch-up rate, and limited transient complications. This approach may serve as an effective bridge between conventional energy-based rejuvenation and surgical facelift in selected patients.
