New trends in breast augmentation: hybrid (composite) breast augmentation and breast implant conversion with fat grafting

Objectives: We will describe the new indications, types, and timing of augmentation together with the recommended technique in these different indications, specifically fat processing via the vibration technique using PAL (MicroAire) technology and decanting.

Introduction: The use of fat grafting in breast surgery has become a well-established method with several indications. Although the roots of autologous fat grafting have hypoplasia indications, there are new, aesthetic, and revisional indications emerging that include conversions (e.g., breast implant replacement with fat) and hybrid/composite augmentation (e.g., implant enhancement to modify volume and/or shape). These options have indications for both simultaneous and delayed approaches.

Conclusion: This evolution in indications is particularly important given the advent of troubling conditions such as breast implant-associated anaplastic large cell lymphoma, breast implant illness, and capsular contracture. But changing times and tastes, including patient’s desires to remove implants altogether, call for surgeons to also include these procedures as part of their armamentarium.

Current state of fat grafting the breast: practice and evidence

Objectives: In this webinar we will review the current common practice of fat grafting the breast with an informal survey including 5 surgeons, describing some practical cases, and we will summarize the existing evidence regarding procedural variations, and safety.

Introduction: The role of fat grafting the breast has evolved in the recent past gaining many nwe applications within both, reconstructive and aesthetic surgery. Inictially mainly used as and adjunct, it has grown to encompass a variety of primary indications including complete reconstructions and augmentations.

Results: The most common indication was correction of tuberous breasts. Harvesting was mainly performed with hand hel siringe with 3mm cannula, followed by centrifugation with Coleman’s procotol and reinjection with 1.2-2mm cannulas.
However, there is yet no high level evidence to support a universal protocol of fat grafting the breast.
Complication rate is low and although there is still some contradictory information regarding its oncological safety, most authors consider it a safe practice with no interference in recurrence nor indication of biopsies.

Conclusion: Autologous fat transfer is an accepte procedure yielding good outcome in a variety of reconstructive and aesthetic indications. No level I or II data exist to support a consensus recommentadion for clinical practice. Overall it has proven to be safe with low morbidity and no increase of breast malignancy risk. Nevertheless, more human studies with long term follow up are required.
