Objectives: The roles of EBD in the treatment of acne and rosacea should be considered. I want to introduce some new EBD beneficial for the treatment of acne and rosacea.
Introduction: Although oral medication has been traditionally used in moderate to severe acne, there is an increasing number of patients reluctant to take it for various reasons. Also, there are some problems with medication, such as teratogenicity(isotretinoin) and antibiotic resistance. Some patients cannot take oral medication for different reasons, such as kidney and liver problems. Various devices can be beneficial in the treatment of acne. The energy-based device that can help to treat acne are divided into two.
Materials / method: One is the device that helps to destroy Cutinebacterium Acnes, and the other one is EBD destroys or suppresses the sebaceous glands. Recently IPL notch filter specifically targeting acne came out. Wavelength between 400-600nm targets Cutinebacterium Acnes and 800-1200nm suppresses the sebaceous glands. There is also an RF needle that is intended to target the sebaceous glands directly. We have been using it for acne patients who had many comedones with good results.
Results: Photothermal therapy(PTT) is popular in Korea nowadays for the treatment of acne. It is very similar to photodynamic therapy except that it uses gold nanoparticles instead of the photosensitizer. After applying gold particles to be absorbed into the pores near the sebaceous glands, a laser is irradiated to destroy the sebaceous glands.
In the treatment of rosacea, medication has its limitation in treating the underlying erythema or telangiectasia. EBD treatment reduces the persistent erythema and shortens the period of medication maintenance.
Conclusion: IPL with a short pulse duration(0.5msec) showed a comparable result to PDL in the improvement of erythema in rosacea patients. In addition, a new pulsed-type needle bipolar radiofrequency showed promising results in the treatment of facial erythema associated with acne vulgaris and rosacea.
