Objectives: As we get older, we can see many aging signs with contour change of the lower face. So, we need to treat the improper lower face contour with small chin, prejowl sulcus, groove and furrow induced by volume loss with static tight retaining ligaments and dynamic wrinkles and lines. In the practical treatment, today, we usually need combination procedures of filler, toxin, thread, skin booster and EBD, and so on. Especially, I'm gonna talk about a combination treatment of PDO volumizing threads & HA filler for the rejuvenation of perioral region including the mandible border line.

Introduction: HA (Hyaluroic Acid) filler injection is one of the most widely used aesthetic technique for minimally invasive plastic surgery. And PDO volumizing thread lifting has gained popularity among patients who seek more aesthetic improvement of the facial contour. I will explain how to use PDO volumizing threads with HA fillers with related anatomy in the presentation

Materials / method: PDO volumizing threads are multi-filament threads without having to fix them because they don’t have barbs. These days, volumizing PDO multifilament threads are in fashion to make augmentation effect by filling the space.
Looking at some types of PDO volumizing threads, there are cylindrical mesh type, spring, broom, twisted stick type, and so on. You can insert the thread first & then inject the HA filler around the thread. At this time, the volumizing thread can act as supporting stand for the filler.

Results: PDO volumizing threads show strong physical support with filling effect to act as the structure frame. And this structure offers not only the mechanical strength against tissue pressure, but also gives the attachment scaffold for cell regeneration & other biochemical factors. Good volumizing threads also have proper tension with soft elasticity to retain their original shape even when they enter the tough tissue. Injected HA fillers after the thread insertion make better outer shape and contour with smooth surface.

Conclusion: The role of PDO volumizing thread is to augment the tight and firm tissue of the perioral region including the mandible border. And volumizing threads have effective tensile strength, so they can provide the structural frame & maintain its shape without being flattened against tissue pressure. So, they can make the best results in the combination treatment with HA fillers for the improvement of lower face aging and contour.

Objectives: Moderate, natural-looked rejuvenation with minimum invasive procedure is one of the options in our daily practice. For the attainment of this purpose, how to combine with threads, fillers and devices?

Introduction: Recently, various kinds of non-surgical treatments for sagging face has come into our aesthetic medical market.
Now, we usually combine devices, injectables and threads in daily practice. About energy-based devices such as HIFU, it’s painful and patients sometime disappoint with the results. About filler such as hyaluronic acid, we sometime create strange shaped glamorous appearance as a result, if we inject too much volume. And about thread lift, it’s invasive treatment and efficacy last for only a several months.

Materials / method: I tried to find less invasive technique to get moderate result by combination with threads, fillers and HIFU.

Results: Less invasive, minimum number of non-cogged short threads, a few syringes of fillers and partial treatment of HIFU is suitable for some patients who require the moderate results.

Conclusion: Many aesthetic practicians believe that patients always desire excellent results by using non-surgical procedures. However, some patients, especially Japanese are very conservative, therefore they sometime wish to change appearances as “nobody will ever know”. It means natural-looked rejuvenation.
I’ll talk about my pearls of combination lift with non-cogged threads, filler and HIFU, focus on minimal invasive technique.
