Objectives: To evaluate the efficacy of a multifaceted treatment regimen combining with exosomes (Rose Stem Cells-Exosomes, RSCEs) with fractional lasers, RF microneedling, and subcision techniques in the management of acne scars.

Introduction: Acne scarring is a challenging condition that affects both the physical appearance and psychological well-being of patients. Traditional treatments have offered limited success, prompting the exploration of innovative approaches.

Materials / method: The study employed a combination of exosomes application, fractional laser therapy, RF microneedling, and subcision in a cohort of patients with varying degrees of acne scarring. Treatment efficacy was evaluated through comparative before-and-after analyses, considering scar depth, skin texture, and patient satisfaction.

Results: Significant improvements were observed in scar appearance, skin texture, and overall patient satisfaction. The combined treatment regimen was found to be more effective than traditional methods, with minimal side effects and reduced recovery time. The findings suggest that the synergistic effects of exosomes and minimally invasive procedures can significantly enhance the outcomes of acne scar treatments. This approach not only accelerates the healing process but also addresses the aesthetic and functional concerns associated with acne scarring.

Conclusion: The study demonstrates the potential of combining exosomes with advanced dermatological techniques to improve acne scar treatment outcomes. Further research is recommended to optimize treatment protocols and evaluate long-term effects.

Objectives: The needle-less syringe is revolutionary. It is very fast and has functions that cannot be done with manual injection. In particular, it can produce groundbreaking and good results in the treatment of acne scars. I would like to do a biopsy and show photos of the tissue reaction and discuss it together.

Introduction: There are several ways to treat acne scar, including fractional laser, micro needling, and medications. The author would like to introduce a needle-free injection method using micro-droplets, which is dramatically more effective than existing methods.

Materials / method: The efficacy and safety of using a needle-free injector for the treatment of acne scars has been demonstrated. Through cases presentation and analysis, this study aims to demonstrate the benefits of this advanced technology, which provides fast (10 injections per second), accurate and consistent injections and can control the depth and speed of the dermal layer.
The authors also investigated the potential of needle-free injector to promote collagen production and tissue remodeling, especially with regard to the treatment of acne scars, through injection of various agents such CaHA,PDLLA,PLLA.

Results: Active acne and acne scat were treated well after CAHA injection using needle free injector

Conclusion: In conclusion, the needle-free injection method utilizing CaHA micro-droplets presents a promising advancement in the treatment of acne scars. Through this study, the efficacy and safety of this innovative approach have been demonstrated, showcasing its superiority over conventional methods.

The rapid, precise, and consistent injections provided by the needle-free injector offer significant advantages in acne scar management. Moreover, the ability to control depth and speed within the dermal layer enhances treatment accuracy and efficacy.

Objectives: This work aims to assess the impact of sequential multimodal laser therapy on acne scar geometrics and texture.

Introduction: Scarring is one of the most prevalent long-term complications of acne vulgaris and has cosmetic, psychological and social burdens. Contemporary management programs integrate multiple modalities to best address the multiple factors underlying their development and persistence.

Materials / method: This is a prospective open-label trial of sixteen (n=16) patients. Adult patients with Fitzpatrick skin type II-IV and presenting facial acne scars (combination of box, rolling and icepick scars) were recruited and underwent three combination fractional ablative (CO2) and nonablative (1570 nm) laser treatment sessions at two-month intervals. Scar microtopography was assessed at baseline and 6 months after the last treatment.

Results: Patient cohort included 9 men and 7 women, average age of 29.3±7.9 years. Mean pretreatment scar volume was 5.7±5.2 mm3 and mean affected area was 165.6±134.0 mm2. Overall scarring intensity was moderate in 9 and severe in 7 patients. Six months after treatment sessions, scarring intensity improved in all patients, with all severe cases graded as moderate and all moderate cases graded as mild. Mean scar volume was 3.1±3.0 mm3 and the mean affected area was 94.0±80.1 mm2, translating to 47.0±7.9% and 43.2±8.6% reductions from baseline, respectively. Patients were highly satisfied.

Conclusion: Multimodal CO2 and 1570-nm laser treatment improved the surface profilometry of patients with atrophic facial acne scars. Customization of both treatment intervals and laser settings to cosmetic regions, scar profiles and skin phototypes may further enhance treatment outcomes and expand its applicability to additional skin deformities.

Introduction: Keloids are one of the most dreaded sequelae of acne. They can be symptomatic with pruritus and pain, and can cause significant disfigurement, leading to psychological distress and impacting the quality of life. They are notoriously difficult to treat, as most treatment modalities have limited effectiveness and have adverse reactions. Moreover, they have a tendency to recur, leading to frustration for both patients and clinicians. Research into more effective treatments is therefore essential to improve outcomes for this challenging condition.

Materials / method: This was a hospital based prospective, double blind, randomized controlled trial of 20 patients with chest keloids. They were randomly assigned to receive fractional laser assisted drug (LADD) delivery with either triamcinolone 10mg/ml solution or 50mg/ml 5 fluorouracil (5fu) solution. Treatments and monitoring of outcome measures were done monthly for 6 months. Outcome measures were lesion size, Vancouver Scar Scale (VSS) score, Patient and Observer Scar Assesment Scale (POSAS) scale and photography.
Adverse events were noted.

Results: Both treatment arms reduced the keloids’ overall surface area. There was no statistical difference between the two groups. There was more pain and erythema noted in the 5fu while telangiectasia and pruritus were more prominent in the triamcinolone group

Conclusion: Laser assisted drug delivery offers a potential alternative treatment for keloids by improving drug penetration and efficacy. Triamcinolone and 5fu are equally effective in reducing lesion size.
