Effective Delivery of Polydeoxyribonucleotides for Facial Rejuvenation

Introduction: Polydeoxyribonucleotide or PDRN is a DNA-derived drug consisting of a mixture of double-stranded deoxyribonucleotides with a chain length of 80–2,200 base pairs with a molecular weight ranging from 50-1500 kDa. PDRN is extracted and purified from the sperm cells of Oncorhynchus mykiss, commonly known as salmon trout, which provide high-quality DNA without immunological side effects. Initially intended for tissue regeneration for chronic non healing wounds, its regenerative properties paved its way to be utilized in the field of medical aesthetics and dermatology for skin rejuvenation.

Materials / method: The use of various microneedling instruments, followed by topical application of PDRN or a PDRN cocktail combined with growth factors, peptides, vitamins and non-cross linked hyaluronic acid are performed by clinicians for better skin penetration. Another method is the use of a multi suction injector which aims to combine microneedling with mesotherapy principles.

Results: The device allows advanced solution delivery with a 9-pin multi needle and vacuum function to minimize the leakage of solution, minimize pain and shorten treatment time.

Conclusion: Hence, the delivery of PDRN into the skin using a multi suction injector is more precise and the potential leakage is prevented.

Minimally invasive 980nm laser-assisted lipolysis and skin tightening on lower eyebags of Asian patients

Objectives: The present study aimed to evaluate the effectiveness of minimally invasive 980nm laser assisted lipolysis and skin tightening in lower eyelid blepharoplasty of Asian patients.

Introduction: Patients with mild and moderate degree of eyebags underwent 980nm laser-assisted lipolysis via lower eyelid stab incision between December 2017 to December 2019. Total energy of 1200J -2000J was delivered to both eyebags at 6 to 10 W. They were followed up for at least 6 months.

Materials / method: Evaluation criteria was reviewed by photograph taken pre-operatively and 6-months post-operatively on Global Aesthetic Improvement Scale (GAIS), the patient’s perspective from the questionnaire with the perception of reduction in eyebags size, the average perception of improvement in skin tightening, and the patient overall satisfaction, all with a score of 1-5 (5 being the most noticeable and very satisfied) and complications such as dyspigmentation, hematoma, prolonged edema, skin bump and thermal burn were documented as well.

Results: A total of 178 cases with 137 women and 41 men were included. A total of 166 patients (93.26%) revealed an improvement in GAIS, with the 12 patients (6.74%) complaint no change 6 month postoperatively. Perception of improvement in eye bag protrusion scored 4.39± 0.59, improvement in skin tightening scored 4.42±0.58 and the overall patient’s satisfaction scored 4.59±0.53. The patients average recovered swelling from 4.35±2.3 days. There were 5 patients (2.8%) with dyspigmentation, 3 patients (1.69%) with prolonged edema and 2 patients (1.12%) with skin bump.

Conclusion: Patients with mild to moderate degree of eyebags who resist surgery are good candidates for laser assisted lower eyelid blepharoplasty.

The multi-dimensional anti-aging concept of mandibular margin guided by Oriental aesthetics

Objectives: There are different treatments for mandibular margin relaxation according to its different types. The purpose of this study is to study the different treatment methods for different types of mandibular edge relaxation and aging, and how to combine the best effect.

Introduction: Due to ethnic reasons, Asians have poor bone and muscle development in the lower face, and many people do not have a clear jaw line. With age, sagging and sagging of the lower face is the primary manifestation of many aging people: including but not limited to skin sagging, blurred mandibular edge, double chin, marionette lines and so on.

Materials / method: 50 outpatient patients with anti-aging needs of mandibular margin were collected. According to the specific classification of relaxation, comprehensive treatment including operation, skin and injection was performed according to the specific classification, and the improvement degree and patient satisfaction were observed after surgery.

Results: The aging of mandibular margin can be divided into five types: skeletal type, soft tissue hypertrophy type, sagging type, mixed type and iatrogenic type. For different types, the combined use of surgery, photoelectric instrument, linear lifting, botox and other methods can achieve satisfactory results.

Conclusion: In view of different types of lower face aging, from the perspective of etiology, following the Oriental unique aesthetic evaluation, different types of mandibular edge aging are classified, through different means of addition, reduction, tightening, brightening and elastic, with the help of surgery, instruments, non-invasive injection combined treatment, can achieve a good treatment plan, restore youth beautification and other effects

A comprehensive treatment of focused ultrasound combined with cosmetic injection for facial rejuvenation of lower face

Objectives: 1.The aim of the study was to clearly define effect of focused ultrasound and cosmetic injection.

Introduction: 1.A comprehensive understanding of ultrasound combined with cosmetic injection is more effective than one alone.

Materials / method: 1.The combined effect of focused ultrasound and cosmetic injection were observed in 126 Chinese.

Results: 1.Focused ultrasound includes two parts of face lift and face firming, Through different levels of action to achieve tightening and lifting, improve the contour curve, stimulate collagen regeneration and other effects.
2.We routinely recommend that patients use botox in combination.
3.If patients with mandibular Angle defects need to use hyaluronic acid to further reshape mandibular Angle, including the use of regenerative products, which will improve the elevation of the lower face. If there is a problem of oral pouch, we will also recommend patients to use small protein lines for treatment.

Conclusion: 1.The author confirmed that lower facial combined focused ultrasound and cosmetic injection has higher clinical satisfaction rate, longer maintenance time and better firming and enhancing effect.

Changing skin cells behavior – the future of aesthetic medicine

Introduction: Regenerative medicine holds a prominent position in the current landscape of the aesthetic industry. Presently, patients visiting our clinics express a strong desire for outcomes that appear natural, with a recurring sentiment of wanting to regain a youthful natural appearance.
The inquiry arises: Can polynucleotide, growth factors, exosomes, and biosomes be regarded as pivotal elements in achieving rejuvenated skin? understanding the mechanism of action and the molecular biology underlying these components becomes imperative.

Materials / method: A literature review was conducted to evaluate the knowledge exist today about growth factors, exosomes, and biosomes. This knowledge was combined with clinical studies and practical experience from my clinics.

Results: Polynucleotides play a crucial role in creating an optimal microenvironment for cell organelles, leading to their potentiation, while growth factors excel in stimulating specific cellular functions. Exosomes, offering a more potent stimulation, present a challenge in isolating specific subsets of growth factors due to their heterogeneity, making it unpredictable to anticipate outcomes.

Looking towards the future, biosomes emerge as a promising evolution of exosomes, providing the flexibility to deliver a chosen subset of growth factors in a completely synthetic manner.

Conclusion: • Employ a diverse range of products for skin regeneration, exercising judiciousness by comprehending the purpose behind each application or injection. Ensure a meticulous match between the chosen product and the specific indication for optimal results.

• The future of regenerative aesthetic medicine might lie in biosomes, offering a combination of a targeted subset of growth factors and robust natural stimulation. This innovation enables the simultaneous delivery of both precise and powerful regenerative stimuli.

Placental peptides treat sensitive skin syndrome through immune regulatory mechanism

Objectives: To evaluate the safety and efficacy of different doses of placental peptide through a BALB/c mouse model of atopic dermatitis induced by oxazolone (OXA).

Introduction: In recent years, placental extract (PE) extracted from mammalian tissues has been found to have various positive effects on the skin. According to "Guidelines for the Diagnosis and Treatment of Sensitive Skin (SS) in China" 2024 edition, SS is a syndrome in which multiple subjective and objective symptoms appear after receiving minor external stimuli and is more common in the face. Currently research suggests that the occurrence of SS is a complex process involving the skin barrier, nerves, blood vessels and immune inflammation. Therefore, PE may become a method for treating SS.

Materials / method: Sensitize the abdominal skin of mice with 3% OXA one week in advance, and then continuously stimulate the back skin and right ear of mice with 0.5% OXA for three weeks, while administering medication treatment. Evaluate the inhibitory effect of placental peptides on atopic dermatitis by evaluating mouse ear thickness, ear swelling rate, back skin inflammation score, as well as ear tissue weighing, skin pathological staining, and changes in serum and skin inflammatory factors at the experimental endpoint.

Results: 1. The animals in Group L applied placental peptide 50 μL, 100 μL and 200 μL were well tolerated and did not experience any adverse reactions; The animals in the dexamethasone group showed moderate weight loss, indicating a certain level of toxicity. 2. The therapeutic effect of the test substance in a BALB/c mouse model of atopic dermatitis: placental peptide 50 μ L,100 μ L and 200 μ L showed a certain dose-dependent inhibition of delayed hypersensitivity in mice, and volume of 200μL of placental peptide is equivalent to a concentration of 1mg/mL of dexamethasone.

Conclusion: Placental peptides inhibit inflammation through immune regulation which could be a new therapy for SS. Although animal experiments have validated the effectiveness and safety of placental peptides, clinical trials are needed to demonstrate its therapeutic effects in SS.

Clinical management of overfilling syndrome

Objectives: The author effectively evaluates the beauty seeker before surgery, using the the "future" aesthetic evaluation method, with which guide clinical operations.Through injection,the Facial aging problem in the middle and lower parts has been solved in multiple dimensions and aspects,with a total of over 200 clinical cases. Introduction: With age, Orientals have a gradual change in facial features, such as bone resorption, enlarged ligamentous spaces, skin laxity, and accumulation of superficial fat pads that move inward and downward. These changes directly lead to a series of superimposed aging phenomena in the middle and lower parts of the face. For more than 20 years, we have been battling "aging" by applying a lot of HA or PCL or fat fillers to the face to achieve a youthful state. In recent years, we have identified a number of complications that we call "overfilling syndrome".

Materials / method: For "overfilling syndrome", comprehensive treatment such as laser, botulinum toxin, lytic enzymes, and glucocorticoids is used to restore the thinness of the patient's face according to different causes.

Results: With this complete aesthetic evaluation system and comprehensive treatment plan, we have successfully achieved remarkable results in 200+ beauty seekers.

Conclusion: Overfill syndrome is a kind of excessive medical treatment under the deformity of aesthetics, in the process of treatment, we not only have to solve the patient's bloated face in practice, but also to establish a correct aesthetic for them.

Aesthetic and therapeutic approaches for the Asian women's perioral rejuvenation

Objectives: Due to different races and skeletal anatomy systems of Far-East Asians, they usually have flat bones and cultural differences in aesthetics: Asians are more restrained and reserved. Therefore, rejuvenation of Asians requires shaping of the three-dimensional perioral contours. Gender and natural transition with subunits around the face including nasolabial folds, marionette lines, chin and cheeks. This lecture aims to study comprehensive treatment methods for perioral rejuvenation in Far-East Asian women.

Materials / method: Collected 60 women in the outpatient clinic. Based on the facial structure and cultural aesthetics of Asian, perioral aging and other characteristics, combined with Asian perioral aesthetic assessment, a comprehensive evaluation was conducted. Non-surgical comprehensive treatments such as injections, skin lasers, and thread lifts were combined. After surgery Observe the patient's degree of improvement and satisfaction.

5 tips for a quick fix nose job

Objectives: What characteristics make the nose attractive and appealing
• The Golden Ratio is 1:1.618. For instance, if the width of a nose is 1, the length of the perfect nose will be 1.618.

Introduction: Technical considerations…
• Nasal augmentation is the primary cause of nasal tissue necrosis and the second cause of visual loss after the glabella region.
• The communication between the external and internal carotid vascular network increases the danger of embolization in a retrograde flow towards the central retinal artery behind the eye.
• Vessel constriction or occlusion can occur when the blood vessels have too much pressure or occlusion within the artery resulting in alar or tip necrosis, ocular ischemia or blindness.
• The sides of the nose is a no-go zone for a full finger breadth

Materials / method: Needle or cannula???
Theoretically speaking, using a small blunt cannula (e.g., 27-gauge) to perform non-surgical rhinoplasty may decrease the risk of vascular occlusion. However, reports of tissue ischemia and blindness have been reported even with canula use.
• A technical advantage of using the cannula is apparent when injecting large flat areas (e.g., for dorsum augmentation), which can help decrease local tissue trauma and increase efficiency by minimizing the number of injection points.
• A needle injection technique using a small caliber needle (27/30-gauge) ensures precision

Results: 5 key points to treat for the perfect nose …
• Radix - to improve the depressed nasal bridge

•Non- surgical rhinoplasty treatments can be quite a gratifying experience for both, the patients and the physicians.
•Post procedure care and instructions is essential for better outcomes

Unraveling The Supreme Male Calves

Objectives: Previous research has shown that soft tissue augmentation poses challenges for plastic surgeons due to the body's natural defenses against foreign materials, despite the fact that silicone rubber implants are widely used. Recent research indicates that alternative procedures such as liposuction and gastrocnemius muscle resection are viable options. To date, there is a scarcity of current research addressing the aesthetic ideal criteria for male calves, with constant discussion about the best procedures to achieve such goals. This study intends to conduct a systematic review to fill this gap.

Introduction: Legs are significant in the cultural construct of beauty across various societies. Furthermore, they are an important component of personal attractiveness and secondary gender characteristics in both men and women. Beyond medical reasons, many people, including athletes, seek calf reconstruction to improve their appearance. Women place a high value on the ankle region, whereas men in many cultures associate well-developed calf muscles with attractiveness and masculinity. There are differences in the supreme criteria for calves between women and men.

Materials / method: A comprehensive literature search for a systematic review was conducted from April 2024 utilizing electronic databases including PubMed, Medline, Embase, ProQuest, Science Direct, Cochrane, and Scopus. Key terms included "Calves," "Legs," "Male," "Ideal," "Aesthetic," and "Measurement." This study examined prospective and retrospective research on calf measurements in males, establishing objective ideal standards. Limited literature on male calf aesthetics places the highest evidence level at level 3. The risk of bias was evaluated using the CASP tool for cohort studies.

Results: Seven cohort studies, among 322 initially identified, met criteria. They uniformly found calf aesthetics follow the golden ratio, with ankle to gastrocnemius muscle distance matching knee to its prominent point. Data on adult males included mean calf circumference (35.8 ± 2.4 cm), medial head length (19.1 ± 1.3 cm), and distance to medial malleolus (22 ± 1.7 cm). The ideal size value was found to be larger in males than in females. Implants offer significant size change but higher risks. Liporeshaping can increase perimeter by 2.8 cm.

Conclusion: In conclusion, our systematic review underscores the importance of understanding and defining ideal calf aesthetics in males. Our findings underscore the significance of legs in cultural constructs of beauty and personal attractiveness. Despite challenges in soft tissue augmentation, alternative procedures like liposuction offer promising options. The cultural significance of legs and the pursuit of personal attractiveness drive the demand for calf reconstruction. Notable gender disparities in aesthetic preferences highlight the need for tailored approaches.

Introduction: In the aesthetic industry one of the most demanded procedures can be considered thread lifting procedures, which gained high popularity among aesthetic doctors and plastic surgeons. Materials / method: So called ‘’nose job’’ is one of the most popular and most complicated procedure. Increased satisfaction with minimal scarring and downtime in liposuction

Objectives: This presentation focuses mainly on liposuction with the least amount of scarring possible and the shortest term of downtime.
The number of scars can be minimized by using multiple shaped cannulas.
Scar release on the day of surgery can reduce downtime.
These can have a positive impact on patient satisfaction.

Introduction: In recent years, the patient population for liposuction has expanded to include not only obese people, but also slender people seeking further beauty.
For slender individuals in particular, scarring can reduce satisfaction.
We believe that more discreet scars and minimal downtime will lead to a further increase in awareness of liposuction.

Materials / method: We report a technique for abdominal liposuction in which the approach is made only from the umbilicus rather than from the traditional visible position.
Although this technique had been used in the past, it had the disadvantage of inadequate liposuction.
The use of multiple cannulas has been used to eliminate this disadvantage.
The range of indications for the technique will also be presented.
We will introduce a device to minimize downtime by releasing excess anesthetic solution by releasing the wound on the day of liposuction.

Results: The use of multiple cannulas allows liposuction with minimal scarring.
In addition, patients can lead their lives without worrying about scars.
This is especially beneficial for modeling clients.
On the other hand, there is no indication for this technique in high-definition liposuction or liposuction for obese individuals.
Drainage of anesthetic solution reduces downtime.

Conclusion: Patient satisfaction can be increased through more discreet scar placement and shorter downtime.
They will make people aware of liposuction as an accessible procedure.

Blurring lines between Scarring Hair loss; FPHL with histologic evidence

Objectives: To reinforce that every FPHL with seborrhoea needs to be evaluated for FAPD

Introduction: FAPD is a newly reported entity. However, we report 10 cases of FPHL with evidence of miniaturization in trichoscopy but histology suggestive of fibrosis and peripilar &
inflammation.

Materials / method: We enrolled 10 women with complaints of hair thinning for trichoscopy and biopsy. Clinical photographs, Trichoscopy pictures and Histologic pictures were documented in all cases.

Results: Results showed that all women had evidence of miniaturised hairs in trichoscopy consistent with FPHL. Of them, 7/10 ladies showed evidence of fibrosis and peri follicular infiltrates along with periadnexal infiltrates.

Conclusion: The outcome of FPHL with early treatment interventions vary from that of scarring hair loss. Hence it is crucial to diagnose the component of scarring in seemingly naive FPHL cases.

The study of Adipose derived stem cell exosome LncRNA assist wound closure's mechanism

Objectives: The aim was to determine the role of ADSC exosome MircroRNA in healing skin wounds.

Introduction: Skin wound healing is a complex and well-orchestrated process that occurs in response to various forms of injury such as surgical incisions, thermal burns, chronic ulcers, microbial infection and ultraviolet radiation. However, disruptions in this intricate process including diabetes, venous insufficiency, and pressure ulcers can lead to chronic non-healing wounds. The refractory wounds pose substantial challenges for both patients and the society in general. Therefore, exploring novel markers and therapeutic strategies for wound healing are crucial to better understanding.

Materials / method: ADSCs were cultured under conditions to obtain Exo. And the effect and mechanism of Exo on wound healing were investigated in vitro and in vivo.

Results: We found that Exo promoted human dermal fibroblasts (HDF) proliferation, apoptosis and migration, as well as rat skin wound healing. The lncRNA derived from Exo has been proved to improve wound healing.

Conclusion: Consequently, we confirmed the effect of active TGF-β on wound healing via the lncRNA pathway. In the end, we concluded that ADSC exosomes promote skin wound healing by regulating the lncRNA /TGFBR1 pathway.
