Objectives: Understand MLA and LIOB/LIC effects in skin of color to treat active acne lesions effectively and achieve clearance within a week by inducing photothermal action on sebaceous glands by achieving it's trt i.e60ms.
Use of nanosecond laser to achieve picosecond laser end points and exploring it as a new and effective avenue as a less downtime combination treatment in skin of color.
With the endpoints we mean the purpura and erythema created by picos which settles in 3days without adverse events, the challenge is to achieve same in skin of color without adverse events successfully to target SG

Introduction: As acne is a disease of pilosebaceous units,which is a result of hyper androgen activity resulting in inflammation and then acne. Multiple treatment modalities from nonsurgical, peels to lasers and energy-based devices, and nonablative fractional and nonfractional are available in our armamentarium to improve acne lesions but most of them are very expensive technologies and are either only for acnes or with variable outcomes. 1726nm,1450nm,1540nm ,wavelengths were claimed to improve acnes
Of all the various modalities available, nanosecond 1064 lasers are available for the longest period.

Materials / method: We have achieved picolaser end points with nanosecond lasers to improve acne lesions and skin texture as a monotherapy in early acnes and as a conjunct therapy in treatment resistant or hormonal acnes.
With the advantage of short pulse durations and gaussian beam profile we have treated patients of darker skin type on the highest of energy with varying passes till the end points achieved which in turn created photothermal damage on sebaceous glands making them less active and drying them up keeping the acne type and skin type of patient in mind.

Results: An appreciable improvement in skin texture and acne lesions has been seen, in case of fresh acnes reduction can be seen in a week time as a monotherapy and the beauty was having lesser downtime, and in treatment resistant acnes it worked synergistically to give good outcomes.because of gaussian beam profile short pulses and high amount of energy can be delivered which creates photothermal damage on sebaceous glands which make them dried up less active and hence reduces acne lesion in very short period and because of 10mm spot size no adverse events even in skin of color.

Conclusion: Nanosecond mla toning induced LIOB/LIC can give picosecond laser end points without any adverse events in skin of color with the gaussian beam profile 10mm spot size even on the highest of the energy if the laser Power supply is constant,with the pulse duration of 2-3ns.,which further gives improvement in skin texture and acne lesions as a monotherapy in early acnes and as combination therapy in treatment resistant or hormonal acnes and with the Lowest downtime and offers a new avenue with the same device with the cost affordability specially in countries like india.

Objectives: This study seeks to assess the efficacy and safety of combination and multifocal fractional picosecond Nd:YAG laser in treating acne scars

Introduction: Acne vulgaris ranks among the most prevalent skin conditions, may persisting for years without appropriate intervention. The chronic inflammatory response within the pilosebaceous unit can lead to permanent pigmentary and scars . The multifocal DOE fractional picosecond laser, represents the latest advancement in acne scar treatments. This high-fluence beam columns, inducing Laser Induced Optical Breakdown (LIOB) at uniform depths. We introduce a combination of Microneedle Radio Frequency and Fractional Picosecond laser in the treatment of acne scars at the same session

Materials / method: The study enrolled 30 Asian patients from both genders residing in the tropical zone. Participants underwent six laser treatment sessions at four-week intervals. The regimen included Microneedles Radio Frequency and two steps of Fractional Laser treatments employing fractional DOE handpieces with different focal lengths to target various depths of acne-scarred skin. The Global Acne Scar Severity Scale (GASSS) was utilized for efficacy and safety evaluations at each visit, and at four, eight, and twelve weeks post the final treatment.

Results: All patients exhibited notable improvements in the global improvement scores at weeks 4, 8, and 12, with each follow-up showing enhanced results compared to the baseline. These improvements encompassed enhancements in skin tone, texture, and the depth of acne scars.

Conclusion: The synergistic use of Microneedles Radio Frequency along with a two-step fractional picosecond 1064 nm laser treatment, featuring two distinct focal lengths, is both safe and effective for the treatment of acne scars in South East Asian patients
