Plasma radiofrequency-assisted microsuction for the treatment of overfilled face syndrome induced by diverse causes in Asian patients

Objectives: 1.Categorize the inducement of facial overfilled syndrome（FOS).
2.Introduce a new treatment method of FOS.
3.Evaluate the clinical outcomes and safety of the study device and surgical procedure.

Introduction: In Asia, facial overfilled syndrome (FOS) can arise from iatrogenic causes involving the excessive use of filling materials, as well as physiological causes such as adipose tissue hypertrophy type, age-related type, and lacuna type. This study aimed to demonstrate the safety and effectiveness of plasma radiofrequency-assisted microsuction (PRFAMS) for improving the appearance of FOS induced by various causes.

Materials / method: PRFAMS was performed on 84 anatomical regions of 37 female patients (including 12 with physiological causes, 6 with hyaluronic acid causes, and 19 with fat causes), aged between 20 and 50 years (mean, 35.9 years), who had FOS. Demographic and surgical data were collected retrospectively. Preoperative and postoperative photographs were taken, and satisfaction interviews were conducted at least six months after surgery.

Results: All patients underwent the operation successfully under local or intravenous anesthesia. Only four anatomic regions showed noticeable asymmetry post-surgery requiring a secondary operation. Additionally, five patients with longer edema periods demonstrated substantial improvement after more than five weeks. No cases of skin necrosis, thermal injury or other serious complications related to the study device or procedure were reported. Ultimately, all patients expressed satisfaction with their outcomes.

Conclusion: The PRFAMS technique is a safe and effective method for treating FOS induced by various causes,while minimizing complications and ensuring high patient satisfaction.

A new medial epicanthoplasty: Upper medial epicanthoplasty - For Asian patients

Objectives: A new medial epicanthoplasty for asians

Introduction: The folds of skin, orbicularis oculi muscle, and mongolian folds that extend from the upper eyelid to the lower eyelid are known as the mongolian folds and are unique to Asians. This fold is said to be present in 70% of the Japanese population. The presence of these folds gives the eyes a rounded and gentle appearance, but to a greater degree, they make the two eyes appear further apart or, in the case of blepharoplasty, hide the inner part of the double fold.

Materials / method: For this reason, medial epicanthoplasty to release the mongolian folds is often performed. Conventional medial epicanthoplasty is a method to release the Mongolian fold by pulling the lower edge of the Mongolian fold inward and shortening the distance between the internal eye angles. Therefore, not indicated when the inner canthal distance is short. We devised a new method of creating a small triangular flap at the inner eye angle to release the Mongolian fold without changing the distance between the inner eye angles.

Results: The advantage of this method is that it does not change the inner canthal distance.

Conclusion: By the wide surgical field, removal of the medial canthus fibrous band (MCFB), which has been suggested to correlate with the severity of the Mongolian fold, is easier than with conventional medial epicanthoplasty. Therefore, it is believed that it is possible to reliably release the mongolian folds and prevent retroversion.

Safe and effective orbital fat reposition technique for lower lid rejuvenation

Objectives: This study was performed to introduce a novel approach for transconjunctival fat repositioning using a bi-directional surgical needle. The advantages of this technique compared to conventional internal fixation (IF) and external percutaneous suture (EPS) techniques were discussed, and the effectiveness of this method in achieving satisfactory results with decreased downtime for patients undergoing tear trough deformity correction was also evaluated.

Introduction: Lower eyelid contour changes result from prolapse of orbital fat and descent of cheek tissues, leading to accentuation of the orbital rim and tear trough groove. In such cases, it is advantageous to reposition the fat over the orbital rim, rather than removing it. For fixation of fat pedicle, the IF and EPS techniques are used, which have their own advantages and disadvantages. The present study aimed to introduce a novel approach of IF technique using a bi-directional surgical needle for specializing a transconjunctival fat repositioning and to compare its outcomes with conventional methods.

Materials / method: Patients with primary tear trough deformity who underwent transconjunctival fat repositioning by IF were reviewed retrospectively. The new IF technique using devised bi-directional surgical needle for specializing a transconjunctival fat repositioning was adopted. Post-operative complications and aesthetic outcomes were assessed using a four-scale grading system.

Results: A total of 32 patients was evaluated with a median follow-up of 90 days. In the analysis of aesthetic outcomes, all patients were grade 0 (no deformity) or I (mild deformity) post-operatively with scale scores improving by at least one grade. Procedure had a shorter mean operation time than conventional IF technique and shorter downtime than EPS techniques. 2 out of 30 patients underwent secondary operation due to under-correction, after which both had satisfactory results. There were no cases of ectropion, scleral show greater than 1 mm, dry eye, or vision loss. Patient satisfaction was high.

Conclusion: Our technique using bi-directional surgical needle for specializing a transconjunctival fat repositioning might possess advantages of the conventional IF and EPS techniques, showing lower complication rates and aesthetically satisfactory outcomes with decreased down time, and could be a safe and reliable method of transconjunctival fat repositioning.

Harmonization of face via varied genioplasty techniques: Insights from our practice

Objectives: To study the aesthetic outcome of various techniques of Genioplasty such as sliding Osseous Genioplasty and Alloplastic chin augmentation via Implants like High Density Poly-ethylene (HDPE), Poly-ether-ether-ketone (PEEK), Silicon (Si) and Hydroxyapatite (HA) implants.

Introduction: Chin is the pivotal feature in harmonizing the overall facial appearance. 1st Genioplasty dates back to 1934 when Aufricht utilized nasal hump for chin augmentation. Over the years, various techniques of Genioplasty have been revolutionized through innovations in digital imaging and 3D virtual surgical simulations that has enhanced surgical precision and predictability. We present our institutional techniques of Genioplasty via Osseous and Alloplastic Implants such as High Density Poly-ethylene, Poly-ether-ether-ketone, Silicon and Hydroxyapatite implants.

Materials / method: The study was conducted over a period of 20 months from August’23 to March’25 at Sir Ganga Ram Hospital, New Delhi, India. Pre-operatively, a 3D virtual simulation of expected post operative outcome was generated. Of 15 patients who desired chin augmentation for Aesthetic purpose, 4 underwent sliding osseous genioplasty and remaining chose chin implants. The choice of implants included High Density Poly-ethylene, Poly-ether-ether-ketone, Silicon and Hydroxyapatite implants in 4, 3, 3 and 1 patient respectively. Patients were evaluated for complications 6 months after surgery.

Results: Of 15 cases, 9 were males and 6 were females, in the age group of 18 to 35 years. All Genioplasty techniques were done using intra-oral approach. Maximum horizontal and vertical projection achieved was of 8mm and 7 mm in patients of Silicon and High Density Poly-ethylene chin implant respectively. There was no major complication noted like Neurosensory deficit, extrusion, or wound dehiscence. All patients were satisfied with their aesthetic outcomes except one patient who had slight asymmetry due to implant migration 1 month postoperatively.

Conclusion: The described Genioplasty techniques, be it Osseous or via Alloplastic Implants achieve good outcomes with high patient satisfaction. Pre operative 3D virtual simulation of results is important to have predictable results. Deep knowledge of surgical technique is fundamental to minimize complications. Caution is required to avoid potential under-correction or over-correction.

Deep plane facelift for Asian patients

Objectives: In Japan today, thread lifts seem to be all the rage for improving sagging facelines, but there is no disagreement that incisional facelift surgery is still necessary as a fundamental treatment.

Introduction: However, there are various surgical procedures under the umbrella term incisional facelift, and in Japan, simple procedures such as skin resection only, SMAS tacking, and triangular flap method are still commonly performed.

Materials / method: Japanese people have highly developed retaining ligaments, and it is thought that facelift surgery is less effective due to their skeletal and skin characteristics, and I have seen many cases where good results could not be obtained with traditional simple facelift surgery.

Results: For Japanese people, who are less likely to see results, I have obtained good results by performing deep plane facelifts that focus on releasing the retaining ligaments, suctioning the superficial fat compartment, and resecting/lifting the buccal fat pad for redraping and repositioning.

Conclusion: I would like to share my experience and ideas regarding deep plane facelifts for Japanese people.
