Objectives: The correction of deformities of the lower eyelid such as huge skin excess, senile eyelid, malar pouches, festoons, is one of the aesthetic surgeries which requires more precision and sensitivity on part of the surgeon. This involves the "gaze" of the person, which must remain unchanged.

Introduction: Such correction requires special attitudes of surgical technique, in order to avoid or reduce the risk of complications such as "sclera show", which is the most frequent.

Materials / method: The technique is characterized by a lateral resection and suspension of the orbicularis muscle and skin resection.

Results: It has been used for the correction of complex eyelid deformities , avoids the possibility of ectropium, corrects sclera show and improves the medial face flaccidity.

Conclusion: The Myo-extended Inferior Blepharoplasty is a standard and reproducible surgical procedure with objective parameters of reference during the execution .

Objectives: The aim of Tranconjunctival Blepharoplasty (TB) is to allow physicians to treat inferior lid’s fat herniation avoiding an obvious cutaneous scar.

Introduction: the TB was born at the end of 1900. The usefulness of this surgical procedure is growing up in the time demonstrating a very wide field of applications in the correction of many defects of the lower lid

Materials / method: The incision is performed in the conjunctival groove. the possibilities for use are varied also in association with surface technique of skin contraction to increase the range of treatments to be proposed.

Results: The effectiveness and quality of the results makes it a timeless technique that would be almost always worthwhile to use except in special indications.

Objectives: Browptosis correction

Introduction: Surgical correction of superciliary ptosis has always been a challenge for plastic surgeons for decades. Several techniques are described in the literature with their pros and cons.
We present a new technique for correction of superciliary ptosis without videoendoscopy supportand with minimal incisions and detachment in the frontal subcutaneous plane.
Technical and instrumental details will be presented for its accomplishment. We started to perform this procedure in January 2016. Since then, it has been our first choice for the treatment of brow

Materials / method: we selected 27 patients with brow ptosis and submitted to correction by this technique, associated or not with blepharoplasty

Results: the aesthetic results have been satisfactory both from the surgeon's point of view and from the patients. In some patients (2) there was a loss of the level of the brow without however compromising the final result

Conclusion: is a valid and safe technique alternative for correction of browptosis

Introduction: Introduction

During blepharoplasty, harvesting of Reidy Adamson flap interrupts most of the motor innervations coming from the zygomatic branch of the facial nerve. The conservation of the functionality of the orbicularis oculi muscle, maintained in patients underwent inferior blepharoplasty, suggested to investigate the role of the innervation coming from the medial branch of the zygomatic and buccal nerve of the 7th cranial nerve. Aim of the study: compare, trough the electroneurography, the residual functionality of the orbicularis oculi muscle after transcutaneous blepharoplasty.

Materials / method: The study enrolled 5 candidates (10 eyes) to transcutaneous blepharoplasty with stabilization of the inferior eyelid with anchorage of the orbicularis oculi muscle to the periosteum of the upper lateral border of the orbit according to Reidy Adamson technique. In each patient bilateral muscle functionality was evaluated with electroneurography preoperatively and three months after surgery. Followed parameters: Compound Muscle Action Potential (CMAP) as expression of quantity of muscular fibres activated by electric stimulation of the facial nerve. Results were statistically analysed.

Results: The middle age of the patients was 62 years (range 55-69), with an average follow-up of 5 months (from 3 to 6 months). 18 measurements were collected; one patient was excluded from this study because missed the postoperative electroneurography. No modifications of CMAP wideness were observed postoperatively. No statistically significant differences were observed between pre and post-operative collected data.

Conclusion: Basing on our results, the medial innervation of inferior orbicularis oculi guarantees a satisfactory recovery of the muscular functionality including stability of the inferior eyelid.

Objectives: The Author shows some clinical cases of his experience about rejuvenation of the face, using the lipofilling as an ancillary procedure, as well as alternative surgery.

Introduction: During the last two decades, lipofilling has increasingly indicated by plastic surgeons for aesthetic treatments. Thanks to the improvements of the technique regarding harvesting, proceeding and injecting the adipose tissue, today it is very often used in facial rejuvenation, associated or not to other procedures

Materials / method: In presence of severe skin laxity, the face lift is considered by the Author the treatment of choice, but the association of lipofilling certainly improves our results : we can correct the depletion of the adipose component in different areas of the face and improve skin tone and texture. Lipofilling is used as complementary procedure with face lift in high-risk patients, such as heavy smokers or diabetics, and an alternative procedure in cases of moderate skin laxity , particularly if associated with reduction of the adipose compartment.

Results: In the cases treated with the association of lipofilling to the face lift, the results have always been encouraging both for the volumization achieved and for the overall improvement of the face. Even using lipofilling as a single procedure we have achieved good results but we have limited the indication only to patients with moderate excess skin.

Conclusion: Even if the facelift is the main surgical treatment for facial rejuvenation in presence of severe skin and tissue laxity, lipofilling must be taken into consideration as an ancillary procedure, as well as, in selected cases, as alternative surgery.

Objectives: triple anchoring facelift

Introduction: This type of facelift is based on 3 principles:
1 estimate of final cutaneous excision before the procedure
2 triple subcutaneous and percutaneous anchorage minimizing the operative follow-up and perpetuating the results
3 absence of cutaneous traction

Materials / method: In most cases, the procedure is performed under pure local anesthesia, which allows complete approaches without any difficulty; only very fat faces or particularly anxious patients will be operated on with additional anesthesia.
The undermining is generally quite limited, allowing to reach the musculo aponeurotic structures (SMAS and platysma) repositionned in a traditional way (first anchorage). If the patient presents structures to be restored remotely (malar sagging, platysmal cords), their displacement can be ensured by permanent threads.

Results: The second anchorage concerns the subcutaneous structures close to the final scar blocked by fast resorption separated points avoiding any traction on the scar. The third anchorage is provided by percutaneous points of nylon ensuring the cutaneous adhesion on all the detached areas: there is no more risk of hematoma, the bruises are often absent and the edema minimized.

Conclusion: Patients can resume their socioprofessional activities as of the tenth day.
