“There is no single disease which causes more psychic trauma, more maladjustment between parents and children, more general insecurity and feelings of inferiority and greater sums of psychic suffering than does acne vulgaris.” was showed by Professor Sulzberger in 1948.

Acne vulgaris is one of the most leading cause for visits to a dermatologist., and are a lot of myths about acne, including the connection acne - diet.

Introduction: Acne pathogenesis is related to several key factors: excess sebum production by the sebaceous glands, follicular occlusion, hyperproliferation of Propionobacterium acnes (P. acnes) – now is Cutibacterium acnes (C. acnes) bacteria, and inflammation.

The crossover study by Fulton (who denied the connection between acne and chocolate) was methodologically flawed by comparing chocolate and sweet vegetable oil bars that had the same glycemic index (GI). In addition, the placebo bars had higher content of partially hydrogenated vegetable fat, which may contribute to inflammation.

Materials / method: The frequent consumption of carbohydrates - the "Western diet" - which means a high glycemic index, may repeatedly expose adolescents to acute hyperinsulinemia. Acute hyperinsulinemia influences follicular epithelial growth and keratinization, as well as sebaceous secretion mediated by bovine androgens. A “Western” diet is typically rich in high glycaemic index (HGI) foods and is considered to cause chronic hyperinsulinaemia.

Results: The pathogenesis of age-related diseases of civilization such as obesity, T2DM, metabolic syndrome, cancer, neurodegenerative diseases and early aging have all been related to persistently increased activation of the nutrient-sensitive kinase mechanistic target of rapamycin complex 1 (mTORC1).
Acne represent the mirror of exaggerated insulinotropic Western nutrition and a clinical and epidemiological indicator of appropriate or inappropriate human nutrition.

Conclusion: Acne is a polymorphic disease with noninflammatory and inflammatory aspects and a wide spectrum of severity. The pattern of disease, relative severity and distribution, and the patient’s social circumstances will influence management. Before treatment is undertaken, education about acne, its pathogenesis and likely outcome, as well as requirement for ongoing treatment, is required. Commonly held beliefs surrounding acne need to be rationally discussed with both the patient and their family.

Introduction: The pathophysiology of acne vulgaris depends on active sebaceous glands, implying that selective destruction of sebaceous glands could be an effective treatment.
Photothermal therapy (PTT) refers to efforts to use electromagnetic radiation for the treatment of various medical conditions. This approach is an extension of photodynamic therapy, in which a photosensitizer is excited with specific band light. This activation brings the sensitizer to an excited state where it then releases vibrational energy (heat), which is what kills the targeted cells.

Materials / method: After cleansing the skin, light-absorbing gold microparticles were sonophoresis for 10 minutes. The remaining light-absorbing gold microparticles on the skin surface were then wiped off with gauze. We treated acne using a 1064 nm long pulse Nd: YAG laser with 30-33 J/cm2, 30 milliseconds duration.

Results: After PTT with light-absorbing gold microparticles, acne lesions and sebum secretion decreased.

Conclusion: PTT with light-absorbing gold microparticles is effective in treating acne. The advantage of PTT with light-absorbing gold microparticles is non-pharmacological treatment, does not require sun protection for 48 hours after treatment, the main target of treatment is sebaceous gland, so the duration of remission is long.

Objectives: Participants will gain a deeper insight into the available evidence for the treatment of acne, learn more on optimizing acne treatment and will be able to treat acne patients in accordance with current EU guidelines.

Introduction: Acne is one of the most frequent skin diseases. Many treatment options are available for the treatment of acne. But which treatment is the best for which acne type? What are the current treatment recommendations? For how long should the treatments be given and what recommendations are reasonable?

Materials / method: The European Acne Guidelines are based on a systematic literature search and a structured consensus process (S3 Level).

Results: For comedonal acne, topical retinoids, azelaic acid or BPO are recommended. For mild to moderate papulopustular acne, fix combination treatments with adapalene and BPO or BPD and Clindamycin get the highest strength of recommendation. For severe papulopustular acne and conglabate acne, systemic isotretinoin received the highest strength of recommendation.

Conclusion: Treatment selection needs to be oriented along patients needs.

Objectives: We are seeing an increase of demanding for other kind of options to treat, control or repair the acne vulgaris lesions, beyond medication.
In this lecture we revue the different kinds of modalities available as the state of the art in this pathology.

Introduction: Acne vulgaris is one of the most common dermatological problems among people worldwide. Although is more prevalent between young’s is not rare that it steel remains in adulthood. Several medications are available to control the problem, as the well-known peroxide benzoyl, antibiotics and retinoids, but some conditions as pregnancy, liver disease, antibiotic résistance are a concern and could be a contraindication to their administration. Also some patients do not wont to take pharmacological medications and refuse all kinds of systemic pills.

Materials / method: This presentation proposes to evaluate other acne vulgaris treatment options beyond medication.

Results: We will speak about treatment of acne inflammatory lesions, comedo lesions and scar lesions by different techniques and devices as chemical peelings, LEDs and PDT, LASERs (ablative, non-ablative, full beam or fractionated, YAG), RF microneedling, bio stimulation by PRP (platelet rich plasma) and dermal fillers, alone or in combination protocols.

Conclusion: When discussing the available treatment modalities for acne vulgaris in their different forms, clinicians should take into account the desired outcome, tolerability of therapy, adverse events as much as patient preferences, psychosocial factors and cost.
In this complex and multifactorial disease, we should keep in mind all the options with the final goal of treating acne and preventing scarring.

Objectives: To understand the real reasons behind the emergence of Resistant Acne

Introduction: Resistant Acne can be a therapeutic nightmare for Clinicians in practice . Understanding the reasons behind the resistance and then managing the outcome are equally important.

Materials / method: A review of the reasons of Resistant Acne and its prevalence has been done with special emphasis on trying to understand the multitude of factors involved in them . Suggestions on treating such cases have also been reviewed.

Results: A vast majority of Resistant Acne ,but not all , are because of drug resistance . Other causes can be Hormonal , Genetics, Tobacco use , Gram negative Folliculitis etc .

Conclusion: Resistant Acne is a dilemma that needs proper deciphering to understand the aetiology and therefore treatment accordingly. Over usage of Antibiotics both topical and oral has played a major part but may not be the only factor.
