Objectives: Examine the capability of an all-purpose FUE device in performing long-hair FUE in Afro-haired patients.
Introduction: Afro-haired patients are classically considered more challenging follicular unit excision (FUE) cases because of the subsurface hair curliness. With this standard, conventional FUE has performed inconsistently, showing wide variations in transection rates between 14 and 80% in shaved donor procedures [1, 2]. Skin thickness and firmness have recently gained attention as another factor contributing to Afro hair FUE difficulty. Materials / method: Patients were males of African descent who desired FUE without shaving the donor.
I utilized the all-purpose FUE device, UGraft Zeus®, with its single punch design called the Intelligent Punch® (Dr. U Devices Inc., Manhattan Beach, CA). The 18 G punch was used.
The mechanism and technique of extraction are described and reported in the video.
Results: The device enabled the extraction of good quality grafts of 2 and 3 bulky curly grafts that were long-hair-shafted 99% of the time. The horizontal graft transection rate was 3-5%. The punch's self-navigating ability, the lack of a need to position a non-existent slot to fit the long hair shaft, the device's exploitation of torque forces (UForce), and its optimal manipulation of compound oscillatory movements vastly simplified the procedure.
Conclusion: Long hair FUE in Afro patients is efficient and consistently successful with low graft transection rates using this device mainly because of its ability to navigate subsurface hair curls, adaptability to changing skin thickness and firmness, as well as its unique long hair harvesting dynamics that are not reliant on punch indents or slots.
