Gluteal sculpting: Minimally invasive collagen biostimulators and threadlifting

Objectives: 1. Revisit the anatomy of the gluteal region.
2. Learn about minimally invasive options for gluteal sculpting, gluteal projection repositioning, and gluteal augmentation, including collagen biostimulators and threadlifting.
3. Learn about my personal experience and live videos of various techniques involving gluteal sculpting.
4. Appreciate the pros and cons comparing surgical and non-surgical methods.

Introduction: Buttock ptosis is a cosmetically and psychologically distressing aesthetic phenomenon. In addition to a disrupted gluteal contour, new onset striae (a form of scarring associated with stretched dermis) from elastolysis of the mid-dermis and decreased collagen can cause further grievance. More commonly noted in the female population - especially those facing middle age weight fluctuations, postpartum women, and individuals who have come on and off certain medications such as steroids, not everyone is prepared to make the surgical commitment of using implants and autologous fat grafting.

Materials / method: A subdermal implant with 160mg of PLLA and 40mg of CMC, reconstituted with 9mL of sterile water and 1mL of lignocaine was used. A range of 40-60mL of the above was injected into the superficial fat layer above the gluteus maximus on multiple cases, with the maximum projection point of the buttock repositioned to the superolateral quadarant to lift the ptosis. Skin laxity in the infragluteal folds was corrected, striae diminished. 20 [P(LA/CL)HA] threads optionally added to the infra-gluteal fold for further enhance transverse support.

Results: Before/after clinical photos available. Gluteal projection restored, ptosis reversed, patient's self-esteem returned.

Conclusion: PLLA + CMC subdermal implant and [P(LA/CL)HA] threads are effective both individually based on their own merits, and as part of a combination therapy, in cases of appearance of fat eradication, gluteal sculpting and contour reshaping; aiding the process of rejuvenation, restoration and self-esteem enhancement.

Art of nonsurgical correction for selected mons pubis ptosis, an innovative technique by P(LA/CL) threads: A pilot study

Objectives: Regarding impacts of mons pubis ptosis on the patients; including psychosocial discomfort and embarrassment, and also safety. Efficacy and longevity of P(LA/CL) threads in correction of ptosis in different parts of body, we claim our innovative method with appropriate P(LA/CL) threads might be effective procedure with acceptable safety for selected volunteers with mons pubis ptosis.

Introduction: Pubic ptosis may be associated with weight gain, weight loss, and aging. It affects both men and women. Understanding of the anatomy of the superficial fascial system has improved results significantly and decreased complications of trunk and lower body lift. Laxity of the entire lower trunk could be treated in one stage nonsurgical methods for selected patients. Alterations in volume, dimensions, and projection of soft tissue in the pubic area can bring psychosocial discomfort and embarrassment to patients. In some cases, the deformity can be seen even under clothing.

Materials / method: we conducted a pilot study with 10 volunteers with grade 1 or 2 ptosis of mons pubis. from 2 Discrete entry points (3 to 5 cm apart) blow umbilicus on the drown line between iliac spines ,10 branches of P(LA/CL) thread inserted in subcutaneous tissue towards pubis area in an innovative fanning technique.
Photos were taken before and 6th month of doing procedure. By comparing photos, the patients rated improvement based on a 5-point Likert scale with options of 1 (very dissatisfied), 2 (dissatisfied), 3 (moderately satisfied), 4 (satisfied), and 5 (very satisfied), surgeons rated on GAIS.

Results: Regarding primitive data of our pilot study, we found acceptable to strong agreement in improvement of mons pubis ptosis between patients, surgeons. We didn’t find any major complication in the pilot study. The study will be continued up to 25 volunteers.

Conclusion: With a low rate of side effects and positive results of improvement, our simple innovative technique with appropriate P(LA/CL) threads might be considered as an alternative to correct ptosis of the mons pubis for selected volunteers.

Solid and liquid threads: Friends or enemies?

Objectives: The topic concerns personal experience in using two popular procedures for full face 1. Thread lifting 2. Liquid PLLA - which has recently been called Liquid threads by trend.
During the treatment of patients mainly 40+, two procedures in combination gave very good results in terms of bio stimulation and tissue reposition.
The combinations worked especially well in patients who had not previously undergone any aesthetic procedures.
In case of combinations 1st Liquid PLLA was injected and month later Solid multidirectional threads for tissue lifting.

Introduction: In recent years, liquid PLLA has been introduced into the practice of aesthetic medicine even more intensively due to its qualities as safety, biostimulation and lifting effect. Based on this, many doctors offer patients so-called liquid threads, i.e. injections of liquid PLLA and persuade them from threads with fixators. Although in personal practice, one should not make enemies of these two procedures, since it is obvious that they both have different categories of patients and, moreover, if they are combined, then in combination, especially in 40+ patients they give impeccable results

Materials / method: The presentation shows an example of 20 patients of different ages in three categories: Ptosis degree 1,2,3. In patients with ptosis degree 1, only liquid PLLA injections were used, since there was no particular need for tissue repositioning. In the remaining degrees, liquid PLLA injections were initially made and only then, after a month, tissue repositioning was done with barbed threads. In both procedures, the material was injected using a fan technique in the mental and middle third of the face, correcting the contour of the lower jaw and the volume in the middle third of the face.

Results: Results showed, in patients with ptosis 1st injection of PLLA collagen stimulator was enough to receive natural and satisfying results. In ptosis 2 and 3 with excess skin and needs of tissue reposition combination of PLLA collagen stimulator and than barbed threads gave us stronger fixation of the threads after their reposition and natural results. Previously injected PLLA at the top of 1 month gave enough fibrosis and collagen stimulation which allowed the threads to better adhere to previously treated tissues and lift sagging areas. Patients reported improved skin elasticity and tone.

Conclusion: In conclusion, we can say that both methods and products are effective not only in a single treatment but also in combination. Considering that a bit complicated patients with ptosis of 2 and 3 degrees, loss of volume and low quality of tissues, always need such a combination, we can freely say that these two methods, procedures should be friends rather than competitors.

Evaluating the efficacy of P(LA/CL) barbed threads in achieving static and dynamic facial symmetry: A comprehensive study on 20 patients

Objectives: The primary objective of this study was to determine the effectiveness of P(LA/CL) barbed threads in achieving static and dynamic facial symmetry. Secondary objectives included evaluating the degree of improvement in midface, lower face, and upper face asymmetries, as well as the overall satisfaction of patients as rated by independent physicians using the Likert scale.

Introduction: Asymmetry in facial features, both static and dynamic, presents a significant challenge in aesthetic medicine. The use of threads, specifically the P(LA/CL) thread with barbs USP 2/0, EP 3, 80mm from the Aptos brand, has been gaining traction as a minimally invasive method to address facial asymmetry. This study aims to evaluate the efficacy of this technique in improving symmetry in a sample of 20 patients, including 16 women and 4 men, aged between 32 and 54 years. The assessment was conducted using Vectra, ImageJ, and Catometry, alongside photography reviewed by two independent physicians u

Materials / method: A total of 20 patients were included in the study, with 12 presenting with midface asymmetry, 6 with lower face asymmetry, and 2 with upper face asymmetry. The procedure involved the insertion of P(LA/CL) threads with barbs USP 2/0, EP 3, 80mm from Aptos. Vectra 3D imaging, ImageJ analysis, and Catometry were used to assess pre- and post-treatment symmetry. Photography was conducted at baseline and at the 6-month follow-up. Two independent physicians evaluated the photographic results using the Likert scale, where a score of 1 represented minimal improvement and 5 represented significant impro

Results: The results demonstrated a significant improvement in facial symmetry. The R0, R5, and R6 parameters showed statistically significant enhancement, with R0 improving by an average of 22.7%, R5 by 18.3%, and R6 by 16.9%. The Likert scale results indicated that 85% of the patients had notable improvements in symmetry, particularly in the midface region. The Vectra and ImageJ analyses corroborated these findings, showing a measurable lift and alignment in the treated areas. The Catometry results were consistent with the photographic analysis, further confirming the effectiveness of the treatment.

Conclusion: The use of P(LA/CL) threads with barbs from Aptos provides a significant improvement in both static and dynamic facial symmetry in patients aged 32 to 54. The procedure was particularly effective in treating midface asymmetry, with substantial improvements observed in lower and upper face regions as well. The results, as evaluated by both objective imaging techniques and independent physician assessment, support the efficacy of this minimally invasive approach in achieving facial symmetry without exaggeration. Future studies with larger sample sizes and longer follow-up periods are recommended

Comparison of two eyebrow lift methods in a same patient

Objectives: The aim of this work was to compare the efficacy of two non invasive methods in the upper third to evaluate which one has the greatest effect in repositioning the eyebrow and elevation of its tip.

Introduction: The look is much more than what we see, it transmits sensations, anger, tiredness, sadness, joy, security, empathy. Women between 45 and 55 years old usually begin to consult the possibility of a blepharoplasty, however there are alternative methods which are non invasive, effective, safe, and with almost zero down time.

Materials / method: Two methods double needle (A) were performed on the upper face of a nowadays 56 year old female patient. In January 2022, conventional thread with spikes (B) applied to the upper third of the face, placing 5 threads per side. Opening of the visual field and elevation of the entire eyebrow (eyebrow and tail of the eyebrow) were observed. In April 2023, the Light lift b method was applied, and it was assessed both immediately after application and after a month of application

Results: The elevation of the eyebrow with the method A was three times more than the elevation with the method B

Conclusion: The method A resulted in greater traction and consequently greater elevation of the eyebrow and, in particular, of the tail of the eyebrow. This method is an option to postpone or supplant blepharoplasty due to its downtime and its excellent results.

Midface rejuvenation: Combining surgery and threads

Objectives: This study was performed to evaluate the safety and efficacy of integrating midface thread lifting as adjuvant procedure combined with lower blepharoplasty. The advantages of this novel approach compared to the conventional method of combining lower blepharoplasty with midface lift surgery were discussed, and the outcomes of this technique in terms of addressing periorbital aging were examined.

Introduction: Combining lower blepharoplasty with midface lift surgery provides a comprehensive approach to address periorbital aging due to soft tissue descent and deflation. Traditional facial aging surgeries pose risks and involve longer recovery times, so exploring new techniques with lower risks and shorter downtime is essential. This study integrated midface thread lifting with lower blepharoplasty, and its safety and efficacy were evaluated using two outcome measurement scales.

Materials / method: Retrospective analysis from record of six consecutive patients (mean age 56.7 years) who underwent Lower blepharoplasty and midface thread lift. All six patients had orbicularis suspension and one had additional canthopexy. Two patients out of six underwent orbital fat repositioning, while orbital fat was simply removed from the remaining four patients. Focused on individuals with baggy lower eyelids and descent and deflated malar fat pads, outcome measures included the Allergan Infraorbital Hollows Scale and Allergan® midface volume deficit scale, assessed at baseline and three months.

Results: All patients showed aesthetic improvements, with mean photo numeric scale scores improving by at least one grade. Although the combined procedure had a longer mean operation time than only lower blepharoplasty, there were no cases of ectropion, scleral show greater than 1 mm, dry eye, or vision loss, patient satisfaction was high.

Conclusion: Midface thread lifting procedure plays a complementary role by optimizing environment for, lower blepharoplasty by lifting of the descent mid cheek, supporting OOM suspension, prevention of lower lid malposition by support and buttress. As conventional midface lift surgery combined with lower blepharoplasty can paradoxically cause lid malposition by scar contracture especially due to hematoma or traumatic dissection, this combined technique presents a promising, minimally invasive option for comprehensive lower lid-midface rejuvenation with natural-looking results and reduced downtime.

Comparison of PLA/PCL+HA thread lifting versus surgical mini-lift for midface: A retrospective study

Objectives: The aim of this study was to compare mid face lifting between a traditional mini facelift (extended minimal access cranial suspension lift) vs. PLA/PCL +HA thread lifting.

Introduction: Midface ageing is a primary conern of people referring to an aesthetic practitioner nowadays.
More women and men from all ages and backgrounds are considering facial rejuvenation.
New minimally invasive methods are introduced, that provide a safer, economically better but similar result to surgical face lifting without the downtime and complications of undergoing a surgery.

Materials / method: 40 patients were used for this study:

-20 patients underwent the traditional extended minimal access cranial suspension lift (X-MACS)

-20 patients underwent midface lifting using PLA/PCL +Ha threads

All patients were treated by the same doctor to eliminate the surgeon’s experience factor in the limitations of this study and evaluated at a 3-month interval.

Results: With the X-MACS technique, 1 patient had a minimal complication of wound site infection and delay in closure. Expectations vs results tend to an average rate of 3.75.
In comparison, PLA/PCL +HA thread lifting group had 2 patients with a minimal complication of local bruising. Overall this group had higher satisfaction rates compared to surgical lift (4.5) and a lower downtime, minimal morbidity. Overall, patients satisfaction was particularly increased due to the HA effect on texture and quality of skin.

Conclusion: PLA/PCL +HA thread lifting methods provide fast, safe and satisfactory results for mid face lifting and preferred compared to other traditional methods and should be employed in cases that surgical intervention can be postponed to a later age, since as we prove the results are comparable to a surgical mini facelift.

Facial paresis treatment using combination of various threads

Objectives: Patients with facial paresis suffer by limited tissue function and noticeable facial asymmetry. We use combination of threads as a treatment. The outcomes demonstrate enhanced facial symmetry, improved support for limited tissue function, and a subjectively elevated quality of life for patients.

Introduction: Facial paresis is often caused by oncological diseases that require surgical intervention. However, this can result in damage to the facial nerve, leading to limited tissue function and visible signs of facial asymmetry.

Materials / method: We have focused on using thread lifting as a minimally invasive intervention for patients with facial paresis. This treatment is performed under local anesthesia, which is beneficial for patients who have undergone multiple surgeries under general anesthesia in the past. Thread lifting is considered a safe and effective method, utilizing various types of threads, absorbable and non-absorbable, specifically for these cases.

Results: Barbed threads are used to lift tissue in ptosis and restore lost volume in the cheekbone area. We can also rebuild the disappeared nasolabial fold, helping to achieve facial symmetry.

Conclusion: The use of absorbable and non-absorbable threads in the treatment of facial paresis can yield valuable results. This minimally invasive method offers a less traumatic procedure under local anesthesia, with a relatively fast recovery time. The results show improved facial symmetry, support for limited tissue function, and a subjectively higher quality of life for patients.

The North African beauty standards achieved with P(LA/CL) threads

Objectives: The beauty standards and ethnic features influencing the patients demands.
The indications for the P(LA/CP) thread methods depend on the age of the patients and the degree of ptosis
The importance of associating thread methods to fillers, toxins and EBD devices to perform a nonsurgical facelift.

Introduction: Beauty standards have always been extremely prevalent throughout human history , and today they strongly influence everyday communication, the media, and the business world.

Materials / method: It’s a retrospective study held from november 2021 to June 2024 including 234 P(LA/CP) thread procedures. For each patient, we have registered the age, the area treated with threads and the thread method used and the filler or toxin injection or the EBD device associated.

Results: 95% of the patients were women, only 5% men.
70% of the patients are aged from 35 to 55 years old and their main demand is face rejuvenation and natural looking face lift.
19% of the patients are aged from 56 to 72 years old and we performed for them mini invasive facelift with p(LA/CP) associated to filler, toxin injections and EBD devices.
11% of the patients age from 23 to 34 years old and they were performed for them eyebrow lift, nose remodeling and jawline contouring with P(LA/CP) threads

Conclusion: So, as always, the aesthetic doctor has a crucial role: understating the needs and wants of the patients, of all ages, of all ethnics and opt for the best treatment.
With thread methods associated to fillers, toxins and EDB devices we can perform a non surgical facelift with a short recovery time and long lasting results.

Scalp threading: New treatment option for adrogenetic alopecia

Objectives: Scalp threading is a novel treatment that stimulates the hair follicle cells proliferation, extends the anagen phase of hair growth and upregulates the growth factors. Threads are inserted in the scalp for the purpose of prolonged hair follicle stimulation.

A single application of threads results in significant hair growth.
And combining scalp threading with additional complimentary treatments, optimizes the treatment results in hair regrowth.

Introduction: Cochrane review reports that the efficacy of finasteride and minoxidil for AGA varies between 40% and 60%, which implies that a significant number of patients go bald despite therapy. The efficacy of the conventional therapies with respect to new hair growth is unsatisfactory.

Scalp threading, growth factors and peptides, DFCM with MPF, mesenchymal stem cells from autologous micrografts, PRP etc. that act on stem cells and signaling pathways and regenerate and reactivate the hair follicle, are the future in hair regrowth strategies.

Materials / method: Various techniques of thread applications in the scalp and adequate combination of different threads were used to achieve maximum hair regrowth.

Patients with early stage AGA were treated with only scalp threading.
For patients in advanced stages of androgenetic alopecia, different additional treatments were implemented, including growth factors mesotherapy,
mesenchymal stem cells from autologous micrografts, DFCM with MPF, PRP, minoxidil etc.

The evaluation of the hair regrowth results included before-after photos, before-after trichoscopy images and patient's satisfaction survey.

Results: The before-after photos and trichoscopy images, shows significant improvement in the hair regrowth. The photos show improvement from 30% , 50% up to 70% after three and six months and nine month accordingly. The results are superior when combining treatment methods, with best results achieved with scalp threading and mesotherapy with growth factors/multi peptide factors.

The patient's satisfactory survey shows that 60% were extremely satisfied, 20% very satisfied, 15% satisfied, 5% not satisfied.

Conclusion: Considering its long-lasting and stimulation potential and results achieved, scalp threading can be considered to be a good treatment option for patients with alopecia androgenetica.

Single application of threads, results in hair growth comparable to that achieved with consecutive applications of minoxidil. Hair count increases from average number of 65 hair follicular units to 93 hair follicular units per cm2.

Combining scalp threading with different treatment methods allows for optimal hair regrowth and high patient's satisfaction rate.
