Objectives: Open-label split face clinical trial assessing the safety and efficacy of Adipose Stem Cell Derived Exosomes vs PRP post RF microneedling in the cosmetic improvement of facial photoaged skin

Introduction: Exosomes and PRP are increasingly used in aesthetic medicine

Materials / method: 15 subjects FP I-IV between ages 44-68 years old were treated. Subjects received 3 RF microneedling treatments to the full face spaced at 4-week intervals.
Post treatment, PRP was applied to the right side and exosomes to the left side of the face. All subjects Followed for 6 months post final treatment. 4 biopsies to the right & left infra-auricular area at: Baseline; After the final treatment (Tx 3); at 3-month follow-up and : 6-month follow-up

Investigator and subject assessments were undertaken regarding overall skin appearance, wrinkling, dyschromia, erythema, and textural quality.

Results: Exosomes and PRP indicated comparable histological results. 
Exosomes showed slightly better results than PRP in Investigator visual assessments

Conclusion: Exosomes are less invasive as they do not require blood draw. Exosomes can result in equal or improved efficacy than PRP. 

PRP is harvested from the patient's own blood, reflecting their age, whereas exosomes are harvested from healthy donors aged 18 to 25 years, potentially offering more robust regenerative properties

Objectives: The future of aesthetic medicine is regenerative medicine using a conservative approach. Actually we have many millenial patients ' requests to restore the jawline area, to correct the tear trough deformity . As the demanding patients are becoming younger, we need to use a conservative approach rather than filling with an important quantity of fillers leading to the classical pillow faces and heavy jawline because of the filler migration following the gravity.

Introduction: Tear trough correction with HA fillers is a challenging procedure because of the high probability of short and long term complications. The aim of the study is to show the efficiency of a new autologous natural approach using platelet rich plasma clot for a full face rejuvenation and contouring.

Materials / method: use the platelet rich plasma tubes with a separating gel as a medical device containing an antithrombine allowing the formation of a clot. I mix the prp with the fibrin clot with a ratio 9/1 then i use a 27 G needle and inject deeply on the periosteum in the mandibular angle.Then I correct the tear trough deformity using a premixed prp medical device tube with 40 mg of non cross linked Hyaluronic Acid .in Some cases, I use also the premixed prp tube with HA for lips rejuvenation.

Results: The result is an immediate jawline definition , an improvement of the skin texture and a glowing effect on the skin thanks to the growth factors contained in the prp.
No short of long term side effects have been noticed.

Conclusion: The future of Aesthetic medicine is conservative and regenerative medicine allowing to use autologous growth factors stimulating the fibroblasts to produce more collagen elastin fibres and endogenous hyaluronic acid. As the social media influence is impacting the young generations demanding always more aesthetic procedures, our role as cosmetic doctor is to direct the patients towards safe autologous regenerative and conservative approaches.
