What happens beneath the skin after threads insertion?

Objectives: There can be a difference in effectiveness of thread lifting depending on thread number, diameter, the presence of barbs, barb types, needle types, the location of threads, and patients’ condition. There is no well-designed study regarding efficacy difference according to those factors. I will discuss the histologic and biomolecular differences among the various absorbable threads and the infection that physicians can experience during the process of thread lifting.

Introduction: Antiaging has become a very popular aspect in the field of dermatology. Conventional nonsurgical procedures including botulinum toxin and filler injections have their limitations in improving deep wrinkles and decreased tissue laxity. Recent esthetic procedures have focused more on safety, ease of procedure, effectiveness and lower risk for side effects. Thus, thread lifting has become a common trend.

Materials / method: Dorsal skin of rat was divided into five different compartments and four different PDO threads and monofilament poly-lactic acid (PLA) thread were inserted. Tissue samples were obtained at week 1, 2, and 12 after the procedure for histopathologic review and real-time PCR for quantification of collagen.

Results: Multiple PDO filaments produced more collagen at 2 weeks. Single-stranded PLA thread insertion resulted in more Col1α1 levels than the double PDO thread and also showed the most Col1α3 production at week 2. The amount of collagen showed a sharp decline at week 12. Histologic evaluation showed retained threads surrounded by fibrous capsule-like structure at week 12.

Conclusion: There was more collagen production in multiple stranded PDO threads compared to a single strand and increasing number of threads leads to more collagen synthesis. However, things like scars at the insertion site, resistence during the procedure, and the infection should be considred in selecting the types of threads.

From PDO to P(LA/CL), P(LA/GL): is it a paradigm shift in threading?

Objectives: Comparisons between thread ingredients. Best thread for skin stimulation effect.

Introduction: Recently, thread lifting methods are more widely applied in dermatology and aesthetic surgery areas. The main idea of thread lifting was always fighting against soft tissues sagging – Anti ptosis. But there is not enough just reposit the tissues mechanically for total face rejuvenation, it is required to influence the skin structure and quality as well. Different manufacturers using different type of thread composition. Unfortunately, chemical composition of the offered threads in the market (PDO, PDS, PGA, PLA, CL, Polypropylene, Silicone) is not always provided with the required effect. As

Materials / method: Since 2016, we apply P(LA-CL) threads with hyaluronic acid additionally in the thread composition (thread formula looks as P(LA-CL)HA). We have performed procedures for more than 100 patients. During this period some patients underwent the procedure twice or more. Mainly, threads were injected in the middle and lower face thirds per 5 threads per each side. Diameter of the thread is 2/0 with multi-directional barbs.
We have also performed histological researches in rats comparing P(LA-CL) threads and P(LA-CL) threads with hyaluronic acid addition in the thread composition. The research lasted

Results: Histological researches of P(LA-CL) threads and P(LA-CL)HA comparison have provided with the following results in favour of threads with hyaluronic acid:
- Decrease of inflammatory process and joint capsule around the thread
- Decrease of elastin around the thread from the first week of implantation
- Increase of I and III type of collagen for 55% and 20% accordingly

This research and the results in patients have showed the application of thread with hyaluronic acid hastens rehabilitation process after the procedure, excludes risk of subcutaneous scars appearance, increases development of ela

Conclusion: By our practice P(LA-CL)HA thread composition is the best for skin stimulation effect we can get from thread lifting procedures.

Thread insertion for optimal face lift: modifying vector designs to achieve great results in Asian patients

Objectives: Thread Lifting has been becoming popular among Asian patients. Due to the face shape is different the insertions points of the Thread is modifying. We use the entry points at the temporal hairline. We insert the thread upwards into the temporal SMAS or the interfascial plane above the deep temporal fascia. On the downward side we insert the thread along the superficial fat compartment to lift the nasolabial fold , the Marionette line and the jowl.

Materials / method: We did 12 cases with this approach. We insert the thread upwards into the temporal SMAS or the interfascial plane above the deep temporal fascia. On the downward side we insert the thread along the superficial fat compartment to lift the nasolabial fold , the Marionette line and the jowl.

Results: The results of the thread lift is good without widening the shape of the face. The malar prominence stays the same. We has a case of minimal bruising.

Conclusion: Thread lifting with the good insertion points of the vectors will get the maximum lifting effect of the face
