Objectives: Severe inflammation and injury can lead to disfiguring scarring, which can cause cosmetic, functional, and even psychological impact;
moreover, treatment of scars remains a persisting challenge. Scar classification is paramount in determining an appropriate treatment
strategy. They can be classified into hypertrophic, keloid, or atrophic scars. Tissue repair and regeneration after damage is not
completely understood, and current therapies to support this process are limited.
Introduction: Today, exosomes are one of the most highly researched topics in regenerative medicine. Exosomes are cell-derived nanoscale vesicles
carrying various biomolecules, including proteins, nucleic acids, and lipids, with the capability to communicate with adjacent or distant
cells. The exosome can modulate essential cellular processes, such as proliferation, differentiation, migration, and cell death.
Therefore, exosomes can be therapeutic options in regenerative medicine and aesthetics, such as wound healing and scars.
Materials / method: In normal conditions, the wound healing process is associated with cell migration and proliferation, extracellular matrix remodeling,
angiogenesis, and re-epithelialization. Several risk factors, chronic inflammation, and some diseases lead to a deficient wound healing,
producing a scar that can finish with a pathological fibrosis. Exosomes have shown paracrine beneficial effects, regulating
inflammation, modifying the fibroblast activation and production of collagen, and promoting neovascularization and
re-epithelialization.
Results: Our experience of exosomes dermatology and aesthetics clinical applications, more than 1500 scar patients underwent treatment with
exosome in our clinic, this presentation aims to provide a comprehensive summary of the most recent research findings regarding the
mechanisms and applications of exosomes in scar treatment and the best administration route of the exosome combined with different
therapeutic modalities.
