The efficacy of PRP injection on the healing of thyroidectomy wound: a randomized, single blind prospective study

Objectives: The objective of this study is to evaluate the efficacy of PRP on the wound healing of thyroidectomy wound

Introduction: Conventional thyroidectomy causes an obvious scar at neck. The appearance of neck scarring can be disturbing both physically and psychologically. There are many options for reduction and prevent scar formation. Platelet-rich plasma (PRP) has the potential to stimulate and accelerate wound healing process. The cytokines and growth factors released from PRP can play a crucial role in the healing process. However, the use of PRP has not been extensively studied yet in thyroidectomy scars.

Materials / method: A prospective, randomized, controlled trial was performed at Department of Otolaryngology Head and Neck surgery, Ramathibodi hospital, Bangkok, Thailand. 11 patients who met the inclusion criteria that were undergoing thyroidectomy were enrolled. Before closing the skin incision after thyroidectomy was done, the surgical incision sites were divided in midline. One half of the wound was injected with PRP subcutaneously while another side was not injected. Post-operative wound healing was monitored at 1 week 4 months and 6 months

Results: A side of wound injected with PRP showed significantly better healing than the wound side without PRP injection at 6 months post operation, based on VSS (VSS score of -3.09, p value = 0.001, 95% confidence interval -4.76 to -1.42), observer-based POSAS scale (-11.45, p value < 0.001, 95% confidence interval -17.03 to -5.87) and patient-based POSAS scale. (-8.27, P value= 0.03, 95% confidence interval -15.82 to -0.71)

Conclusion: In this study, PRP injection can significantly improve healing of thyroidectomy wound in patients prone to have scar.

PRP for erectile dysfunction: where are we now?

Objectives: Present a summary of recent updates in clinical research regarding the use of PRP for erectile dysfunction

Introduction: PRP has been offered for over 10 years for erectile dysfunction, but clinical evidence has been lacking. Recent updates have seen a increase in the number of clinical studies with interesting results.

Materials / method: Review of the literature and professional clinical experience of using PRP to improve erectile function in patients.

Results: As more clinical studies are published, evidence seems to be increasing to support the use of PRP for erectile dysfunction. However, not all studies are positive and important distinctions are made in the materials and methods.

Conclusion: Increasing evidence suggests that PRP may be an effective treatment for erectile dysfunction, but there are important aspects about the treatment which should be taken into consideration in order to maximize potential results.

Innovative skin aging solutions: the synergistic effects of PRP and PRF with BAP injection technique

Objectives: We observed significant improvements in pigmentation and pore size compared to baseline. Additionally, there were noticeable improvements in nasolabial folds, marionette lines, and acne scars post-treatment. The highest levels of of improvement were observed at week 4, followed by a gradual decrease over time. No serious adverse effects were reported, apart from mild procedural pain (3.5/10 on the pain scale) and transient redness that resolved within 1–2 days.

Introduction: Platelet-Rich Plasma (PRP) has long been recognized as one of the leading solutions for addressing skin aging concerns. Numerous studies have highlighted the exceptional role of PRP in treating wrinkles, enlarged pores, skin hydration, and hair loss. In recent years, Platelet-Rich Fibrin (PRF) has emerged as a promising new agent. However, research on the combined use of PRP and PRF remains limited, and the techniques for utilizing PRF in aesthetic dermatology have not been fully elucidated. Therefore, this study was conducted to explore these areas further.

Materials / method: We conducted a study involving 10 patients who underwent a combination of PRP and PRF treatments using the BAP injection technique specifically for the facial area. Patients were assessed using VISIA imaging at baseline (prior to treatment) and at 2 weeks, 4 weeks, 8 weeks, and 12 weeks post-treatment. Eight parameters were evaluated through VISIA, including spot, UV spot, brown spot, wrinkles, redness, porphyrins, texture, and pores. Additionally, we assessed the improvement in forehead wrinkles, crow’s feet, cheek wrinkles, nasolabial folds, marionette lines, and acne scars.

Results: The brown spots index had a mean baseline value of 68.4% (n=10), peaked at 77.2% (n=10) at week 4, and gradually decreased to 67.9% (n=10) at week 8 and 64.14% (n=7) at week 12. Similarly, the pore index showed a mean baseline value of 30.3% (n=10), steadily increasing to 37.4% (n=10) at both week 4 and week 8, before declining to 32.57% (n=7) at week 12. The highest levels of improvement were observed at week 4, followed by a gradual decrease over time. No serious adverse effects were reported, apart from mild procedural pain (3.5/10), transient redness that resolved within 1-2 days.

Conclusion: The combination of PRP and PRF shows promising potential in improving skin surface conditions, including pigmentation, enlarged pores, and acne scars, as well as addressing other signs of skin aging such as nasolabial folds and marionette lines. The BAP injection technique is a suitable approach for PRF due to its simplicity, minimal pain, and quick application, which prevents PRF from transitioning into a solid state. Further studies are needed to validate the efficacy of the PRP-PRF combination and to optimize PRF injection techniques in aesthetic dermatology.

How to achieve centenarian longevity with gene and advance hybrid therapy in clinical practice

Objectives: The study provides an updated account of genetic factors associated with the extended lifespan and their interactive contributory role with cellular pathways with gene and advance Hybrid therapy in clinical practice including with epigenetics modification for prevention environmental factors which constraints of genetics to either protect or predispose to degenerative diseases.

Introduction: Centenarian longevity is determined by the inherit genes. Some genes promote longevity, including those that encode antioxidant enzymes and heat-shock proteins. Other genes that are associated with serum lipid levels, glycation reaction, repeated inflammation process and immune systems contribute significantly to longevity. Aging is a multifactorial and complex process represented by low energy levels, stress induced loss of homeostasis leading to the risk of diseases and mortality, the functional decline of tissues and organs due to changes in molecular composition and physiology of cells.

Materials / method: The trial in clinical practice to achieve centenarian longevity with gene and advance Hybrid therapy by promoting longevity gene and prevent risk factor that contribute inflammation and degeneration process by gene therapy with stem cell and chorion exosome intramuscular therapy, Metallothionein 1,2,3 (MT1,2,3) intramuscular therapy and deglycation of cell by intravenous high potency antioxidant ,intravenous minerals, including TMS and biofeedback for brain plasticity and regeneration were perform in 5 women age of 90-95 years old.

Results: After 6 months of gene and advance Hybrid therapy protocol with stem cell and chorion exosome intramuscular therapy, Metallothionein 1,2,3 (MT1,2,3) intramuscular therapy and deglycation of cell by intravenous high potency antioxidation molecules,deglycationg molecules including TMS and biofeedback for brain plasticity and regeneration.All of cases were improve in quality of life as increase of energy, improve of cognitive function, good mood, decrease sarcopenia.

Conclusion: The gene and advance Hybrid therapy protocol with em cell and chorion exosome intramuscular therapy, Metallothionein 1,2,3 (MT1,2,3) and deglycation of cell by intravenous high potency antioxidants ,multivitamins ,intravenous minerals, deglycating molecules ,TMS and biofeedback for brain plasticity and regeneration could help to achieve centenarian longevity with good quality of life as increase of energy, improve of cognitive function, good mood, decrease sarcopenia, slow decline of degenerative process.

Exploring the role of probiotic-derived exosome in restoring skin barrier and dermal remodelling in sensitive pigmented skin

Objectives: Probiotic-derived exosomes have emerged as a promising therapeutic innovation
in dermatology, particularly for addressing sensitive pigmented skin. This case report
evaluates the application of Lactobacillus-derived exosomes in ten Indonesian
patients (aged 19-45 years) with Baumann's sensitive-pigmented skin type.

Introduction: The interaction between exosome-delivered components and the skin’s resident microbiome can promote a synergistic effect, enhancing dermal remodeling, reducing inflammation, and improving barrier function.

Materials / method: Ten patients with Fitzpatrick skin type 4-5 (2 males and 8 females, aged 19 to 45
years) with Baumann's sensitive-pigmented skin type, as determined through a self-filled questionnaire, 10 were enrolled in this study. None of the participants had undergone prior facial dermatological treatments. Each patient received a topical application of 2 ml of a product containing Lactobacillus-derived exosomes immediately after undergoing fractional picosecond 755 nm laser sessions.

Results: Objective evaluations demonstrated significant improvements in skin texture and
pigmentation uniformity, while subjective assessments confirmed reduced sensitivity
and increased patient satisfaction. The results underscore the therapeutic potential
of probiotic-derived exosomes in managing complex skin types, providing both
functional and aesthetic benefits.

Conclusion: Combining probiotic-derived exosomes with fractional picosecond laser
technology has proven to be a safe and effective way to care for sensitive pigmented
skin. This approach not only helped restore the skin barrier but also promoted
noticeable dermal remodeling, improving both the skin’s appearance and function.

A comparative study on fractional radiofrequency microneedling with and without platelet-derived exosomes for the management of under eye dark circles

Objectives: To compare fractional radiofrequency (RF) microneedling alone (Group A) versus the same combined with platelet-derived exosomes (Group B) for under‐eye dark circles. Primary outcomes included changes in melanin index, dermal thickness, and vascular visibility. Secondary endpoints assessed safety, adverse events, and patient satisfaction, aiming to elucidate whether exosomes amplify RF‐induced periorbital skin improvements.

Introduction: Under‐eye dark circles result from hyperpigmentation, thin dermis, and prominent vasculature. Fractional RF microneedling induces controlled dermal injury, triggering collagen remodeling and modest pigment reduction. Platelet‐derived exosomes are enriched with growth factors that enhance cellular regeneration and may further reduce melanin synthesis. Combining these modalities could offer synergistic correction of multifactorial infraorbital darkness.

Materials / method: Twenty‐four adults (age 27–54) were randomized into two groups of 12. Each underwent three sessions of fractional RF microneedling (2 MHz, 40–50 mJ, 0.8–1.2 mm depth) at 4‐week intervals. Group B additionally received topical platelet‐derived exosomes post‐needling. Outcomes (melanin index by colorimetry, dermal thickness via OCT, vascular scores on dermoscopy) were measured at baseline and 12 weeks. Data were analyzed by repeated‐measures ANOVA (p<0.05).

Results: Group A showed a 14.8% drop in melanin index vs. 21.3% for Group B (p=0.022). Dermal thickness rose by 7.3% in Group A and 12.9% in Group B (p=0.039). Vascular scores declined by 0.6 (A) and 1.2 (B, p=0.031). Patient satisfaction was higher with exosomes (8.1±1.2) than RF alone (7.0±1.4, p=0.045). Mild erythema and edema occurred in both groups, resolving within 48 hours without complications.

Conclusion: Fractional RF microneedling significantly lessened under‐eye dark circles, and platelet‐derived exosomes enhanced clinical efficacy by further reducing pigmentation, boosting dermal thickness, and lowering vascular prominence. Higher patient satisfaction and negligible adverse events support the synergy of biologic augmentation with RF for periorbital rejuvenation.

Exosomes and nano fat transfer in the treatment of melasma: a good alternative

Objectives: 1. Explain the scientific principles behind exosomes and nano fat transfer in the treatment of melasma, including their mechanisms of action and biological benefits.

Introduction: The combination of exosomes with nano fat transfer offers a synergistic effect by enhancing cellular communication, accelerating skin repair, and providing long-lasting improvements in skin tone and texture. While exosomes deliver specific regenerative signals, nano fat acts as a biological scaffold that supports prolonged tissue regeneration. This combination not only addresses pigmentation but also improves overall skin health, making it an ideal treatment for patients with melasma who seek long-term solutions beyond conventional depigmenting treatments.

Materials / method: The use of exosomes combined with autologous fat and platelet-rich plasma involves extracting approximately 10 cc of fat and mixing it with 1 cc of exosomes and 1 cc of platelet-rich plasma, followed by the injection of this combined solution.

The procedure is performed under local anesthesia.

A 28-gauge cannula is used to introduce the fat combined with exosomes and PRP.

The fat extraction technique is crucial to avoid damaging the adipocytes.

The fat must be centrifuged at low revolutions to obtain a purer fat.

Results: The results were very significant in 70% of patients, showing improvement not only in melasma but also in skin quality, tone, and texture. We perform this type of procedure when we have already achieved a 50% improvement in melasma with other treatments, allowing us to achieve even better results with this approach.

Conclusion: We conclude that this procedure is one of the therapies that can provide the greatest benefits in the treatment of melasma, utilizing regenerative medicine to improve this chronic condition, which often requires combined therapies.

Melasma is not a condition that should be treated with a single procedure, but we can enhance its improvement with modern medicine, especially regenerative medicine.

From fibroblast activation to tissue remodeling: the science of skin renewal

Introduction: Regenerative aesthetics has evolved significantly, integrating multi-modal strategies that harness biostimulation, energy-based interventions, and biologically active topicals to optimize skin quality and function. PLLA biostimulators are increasingly recognized for their role in fibroblast activation, macrophage modulation, and collagen induction. NAFL induces controlled thermal micro-injury, stimulating dermal remodeling without compromising the epidermal barrier. Topical formulations containing growth factors, exosomes, and biosomes act at a cellular level to enhance regenerative processes

Materials / method: A thorough literature review and clinical case analysis were performed to evaluate the regenerative potential of PLLA biostimulators, NAFL, HIFU, and topical biologics. Patient cases were assessed based on treatment outcomes, while published histological data and genetic markers of regeneration were reviewed to provide additional insight. The evaluation focused on collagen synthesis, dermal remodeling, and overall improvements in skin quality.

Results: •PLLA upregulate key regenerative genes such as HGF, DKK1, and ELN.
•NAFL treatments significantly improved skin texture, elasticity, and fine lines, with histological and genetic evidence supporting its regenerative capacity.
•Topical and injectable growth factors, exosomes, and biosomes accelerated post-procedure recovery, modulated inflammatory pathways, and amplified cellular communication for enhanced skin renewal.
• Combination approaches demonstrated superior clinical outcomes compared to monotherapies.

Conclusion: A multi-layered regenerative approach combining PLLA biostimulators, NAFL, HIFU, and topical biologics optimizes collagen synthesis, dermal remodeling, and tissue repair, offering a scientifically validated, non-surgical solution for comprehensive skin rejuvenation. Future research should focus on refining treatment protocols to further enhance patient-specific regenerative strategies.
