Skin tightening using microneedle radiofrequency

Objectives: To understand skin tightening using micro-needle radio-frequency

Introduction: Bipolar microneedle radiofrequency is a novel method that allows non-thermal penetration of the epidermis followed by radiofrequency coagulation at selected depths of the dermis that are surrounded by a zone of non-coagulative volumetric heating.

Materials / method: Each patient was treated with bipolar radiofrequency device for one treatment. All patients were treated with local anesthesia prior to treatment. Patients were examined a week and 3-12 months after laser treatment. A image analyzer system was used to objectively evaluate each patient.

Results: All patients tolerated the treatments. Immediate pinpoint bleeding was observed in all patients. Ecchymosis was observed in one patient but it disappeared within two weeks. Scar, Hypopigmentation or hyperpigmentation was not observed in any patients. Mild to moderate improvement was seen in every patient.

Conclusion: A bipolar microneedle radiofrequency is effective for skin tightening with minimum complications in Asians.

Safety and efficacy of combining monopolar RF with MFU with visualization for lifting and tightening of the face and upper neck

Objectives: To evaluate the safety and efficacy of combining treatment of MRF with MFU-V for lifting and tightening of the face and upper neck.

Introduction: Over the past decade, 2 major modalities for noninvasive skin tightening have emerged: monopolar capacitive-coupled radiofrequency (MRF) and microfocused ultrasound with visualization (MFU-V). MRF achieves volumetric heating of tissue that generates dermal temperatures between 65 and 75⁰C while maintaining epidermal temperatures below 40⁰C. By contrast, MFU-V uses focused ultrasound to create multiple microthermal zones of coagulation at 65⁰C, approximately 1 mm3 in size at speciﬁc tissue depths, while bypassing the epidermis entirely.

Materials / method: Eleven subjects with mild to moderate skin laxity with fitzpatrick skin type III-V received combining treatment of MRF and MFU-V for lifting and tightening of the face and upper neck with interval 1-10 weeks. Improvement was evaluated by blinded dermatologist observer using post treatment photographs compared to baseline after 1-6 months after post last treatment .

Results: All 11 subjects (100%) showed improvement of tightening in submental and jawline area. Ten patients achieved excellent response (76–100%), One patient achieved good response (51-75%). Following treatment there were reported 10 adverse events in 6 patients describe as numbness (2 events), edema (3 events), pain (5 events). All events resolve after 30 days without sequele.

Conclusion: The synergistic effects of the combined therapy MRF and MFU-V in mild to moderate skin laxity of the upperneck and midlower face is safe and effective.

Different employments of ultrasound to different aging morphotypes

Objectives: 1. Knowing differences in aging in Asian skin and facial shapes.
2. Knowing different effects after different energy setting in different skin thickness and facial shapes.
3. Limitation of HIFU

Introduction: Employments of focused ultrasound greatly change and improve our non-surgical aesthetic treatment practices, especially in Asians. Our skin are thicker and delayed aging. We have thicker and more compact dermis which resists formation of fine wrinkles and dynamic wrinkles. But the downside of thicker skin will be sagging which resists to many lifting procedures which might work well in Caucasians. Asians have wider and rounder face, but flatter midface. When we doing procedures to correct deficits, the facial structure differences will make the treatment different from that in Caucasians.

Materials / method: The depth and thickness of SMAS also varies in different areas of mid face. It ranges from 1.7 to 4.3 mm over cheek area in mid face. Sometimes SMAS is very deep, especially in lower face. It was up to 6 to 6.5mm in depth.

Results: A new classification is proposed to show how I evaluate and treat patients in my practice.

Conclusion: The layered anti-aging approach model to minimize risk, to reduce downtime and, most importantly, to maximize anti-aging effects was proposed in my lectures in IMCAS 4 years ago. Making very good use of these anti-aging methods, depending on the demands of patients and evaluations by dermatologists, we will offer the best of mono or combined therapy for our patients. Focused ultrasound is just only one of the important devices to tighten deeper part of skin.

Comparison of energy devices for skin tightening and how to avoid the complication risks

Objectives: To provide an understanding of the different energy based devices as an assistance in selecting which device for which purpose.

Introduction: There is a plethora of energy based devices on the market and each claims therapeutic benefits. In my experience all EBD's do work. The significant issue is
"How they work" and "to what extent".
By reviewing the physics and the physiological effects of different devices we can better understand the choices we need to make.

Materials / method: I will review Radio-Frequency, Microwave, Laser, LED, Electro magnetic fields, and compare and contrast the methods of energy delivery and efficiencies.

Results: Potential risk of complications are attendant on each device and this too is explored to try to mitigate the risk profile.

Skin tightening - Newest perspectives in Europe

Objectives: Selection of the best skin tightening tactics based on many years of experience using leading hardware technologies

Introduction: What skin tightening methods do you use? Why? Are there any better alternatives?

Materials / method: Analysis of the effectiveness of the use of fractional lasers (erbium and CO2), radio wave methods (invasive and non-invasive), phenol - more than a hundred patients in each direction. 7 year observation period.

Results: Found the best combination.

Conclusion: Best technology = best result. The right choice is based on practical experience.
