Objectives: This article seeks to present our 3-year clinical experience with
autologous fat transplantation for breast augmentation and describe
our harvesting and fat processing and transfer techniques, along with
review of clinical results of our patients.

Introduction: Autologous fat transplantation has attracted great interest in
breast augmentation for cosmetic purpose. In the present study, we reported
our experience in fat grafting in breast in 100 cases, and some detailed procedure
concerning efficacy and safety of grafting was evaluated.

Materials / method: Fat was harvested using 4 and 4.6 mm cannulas with Low-pressure pump assisted ( −600mmHg )
. After Centrifugation (3,000rpm for 3 minutes). A 14-gauge cannula MAFT gun was used to place the
fat through 3-mm incision on inframammary fold and periaveolar area. The fat was infiltrated into
the breast from deep to superficial subcutaneous plane.

Results: Between July 2018 to April 2021 , 100 patients have undergone
this procedure. The age distribution of the patients ranged from 20-50 years, with a mean of 31.6 years. Grafted fat volume has ranged from 280 to 360 mL(average, 322 mL) per breast per session. All women had a significant improvement in their breast size and shape postoperatively, and the breasts were soft and natural in appearance.

Conclusion: Liposuction and autologous fat transplantation is a suitable
approach for augmentation mammaplasty.

Objectives: Implant surgeries and autologous fat grafting are two major methods of breast augmentation in current plastic surgery field. As for the aesthetic results, implant surgeries may have problems of unnatural cleavage or obvious upper pole cleft, which are so called “fake breast” appearance in Asian society. On the other hand, autologous fat grafting may have problems of insufficient enhancement in breast sizes and unsatisfied projection. In this short talk, we hope audiences to learn the method of “hybrid breast augmentation” and apply in right situation.

Introduction: In Asian, breast hypoplasia is a popular cosmetic problem for female customers. However, Asian type of extreme skinny body figure may cause plastic surgeons more difficulty in creating both natural and beautiful appearance of big breasts. We reported our experiences by combining the concept of implant surgeries and autologous fat grafting in augmentation mammoplasty for Asian females to overcome the problem of “fake breast” appearance, and we called it “hybrid breast augmentation”.

Materials / method: A retrospective clinical chart review examined all “hybrid breast augmentation” patients who had received breast implant placement and autologous fat grafting simultaneously in the same operation. Surgical approach, implant size and selection was recorded. Fat grafting areas and volume were also reviewed. Follow-up was regularly done with photography taken for comparison.

Results: In all of our “hybrid breast augmentation” patients, breast implants were inserted either from trans-axillary incision or inframammary fold incision with endoscopic dual plane dissection. Autologous fat grafting was injected subcutaneously at medial and superior part of breasts in the same operation. No complications such as neurovascular injury, nodulation, severe capsular contracture were recorded. Only one case of secondary breast fat grafting with previous implant surgery had focal infection episode was noted. During the long-term follow up period, most patients were satisfied.

Conclusion: “Hybrid breast augmentation” concept offers plastic surgeons a good reliable approach to achieve the best augmentation mammoplasty results. Breast implants can give good enlargement and projection of breasts. Autologous fat grafting can make a more natural cleavage and smooth upper pole slope with good soft tissue coverage. This technique can provide long-term aesthetic appearance and patients’ satisfaction.
