Objectives: 9 soft tissue layers in dorsal hand were identified using sonography for the first time. After injection of HA filler, the changes of Thickness, Roughness, and Hydration on dorsal hand were evaluated.
Introduction: Using high resolution sonography, soft tissue thickness can be measured accurately to assess volumizing effects. Also the author tried to refined more detail soft tissue layers in sonography.
Materials / method: In this prospective study, 1cc of monophasic stabilized hyaluronic acid(mS-HA) filler was injected utilizing subdermal scrapping fanning injection technique(ssFIT) into the dorsal superficial lamina(DSL) and dorsal intermediate lamina(DIL) for 20 patients, and the results were measured utilizing sonographic evaluation of soft tissue thickness, topographic computer analysis(TCA) of skin roughness, stratum corneum hydration (SCH) at 1-week,1,2,3,6-months.
Results: The hand appearance and skin roughness improved in all patients. Soft tissue thickness in sonography was increased by 4.52mm(immediately-post-treatment), 5.52mm(1week), 4.89mm(1month), 4.25mm(2m), 4.08mm(3m), and 3.86mm(6m) from the pre-treatment baseline of 3.20mm. Roughness evaluated by TCA in 50(10) magnification dermascope photos, was reduced by 15.39(16.17)% at 1-month post-treatment, 21.5(18.12)% at 2-months, 22.7(23.91)% at 3-months, 27.16(38.12)% at 6-months, indicating improvements in fine wrinkles after the procedure. SCH on hand dorsum slightly improved during the study.
Conclusion: Detailed subdivided 9-layers of hand dorsum in sonography were established in this study for the first time. Soft tissue thickness was increased more than 32% after the treatment, and placed HA materials was confirmed mainly in DSL. The hand appearance and skin roughness improved in all patients. Veins and tendons became less apparent after the single injection showing volumizing effects lasting more than 6 months. All patients stated that their skin became more moisturized with increased SCH, youthful and smooth during the follow-up period after only one session.
