Objectives: Non Surgical Hair Regrowth and treatment is always a first priority of the patient with alopecia. However reversal of results with Topical minoxidil/ peptides and other oral medications has been a disappointment in this area. Exosomes becomes as an effective and targeted therapy in hair loss patients and in long term maintenance proves itself to a promising treatment.
In the paper presentation I would be discussing on the the molecular mechanism of action of Exosomes with data available for its use in hairloss patients , along with few review of case studies in context to Indian patients .

Introduction: Exosome based therapy was used with Microneedling in 20 patients with Male Androgenetic Alopecia and Female pattern alopecia which showed significant decrease in hairloss and efficacious improvement in hair texture and in regrowth to quite an extent.

Materials / method: 12 male patients and 8 female patients with different grades of Androgenetic alopecia and female pattern alopecia were treated with micro needling along with Exosomes formulation with 2 sessions at a gap of 3 months. Patients were assessed using Hair Pull test, Global photographic assessment and Physician and patient subjective assessment scale at baseline and 3 months.

Results: Significant reduction in hairfall was seen in all the patients. Hair texture improvement was self noticed by the patients in 9 male patients and 6 female patients. New fine hair regrowth was seen in 17 out of 20 total patients which was seen as early as in 4-6 weeks on average . Subjective scoring by the patients and a blinded Objective scoring was done by a Dermatologist to assess the results in all the patients which showed improvement in most of the cases. There was slight pain and erythema in 3 out of total 20 patients .

Conclusion: Microneedling along with Exosomes formulation is an effective , convenient and easy to perform procedure in comparison to other cell based therapies or mesotherapies. The results were faster as compared to other therapies. It is definitely an efficacious therapy but more trials and studies are needed to validate the same.

Objectives: Evaluating the effectiveness and safety of hair re-growth factor formulation at multiple centres for reduction of hair fall and regrowth of hair.

Introduction: Alopecia is seen in individuals, as young as 20 years of age and progresses in almost 50% of men over 50 years of age and around 50% of women over their lifetime. Current surgical, medical, and aesthetic approaches remain constrained in implementation and effectiveness along with an array of side effects. a bioengineered hair regrowth formulation containing a particular combination of biomimetic growth factors was administered in clinically diagnosed conditions of Androgenetic Alopecia, Alopecia Areata, PCOS-related hair loss, and post-COVID hair loss.

Materials / method: Patients were administered 1 ml of the solution of these growth factors on the scalp through appropriate micro-needling methods. The sessions were spaced out at 3 to 4 weeks and a minimum of 8 sessions of the solution were given to the patients. A hair pull test, global photographic assessment, and video-microscopic assessment tests were performed at the beginning, after the fourth and eighth sessions, and self-assessment through a questionnaire related to satisfaction and side effects was carried out at the beginning and at after the eighth session.

Results: After eight sessions, the hair pull test was positive in only 1/10th of the patients. Beneficial results were noted in the global assessment score after the fourth and the eighth session. After the eighth session, a huge improvement was noted in terminal hair count, vellus hair count, and hair density. Also, with a self-assessment score, a higher satisfaction score was noted. On long-term follow-up as well, no side effects were observed.

Conclusion: The use of the biomimetic peptide formulation is a safe and efficacious option for all forms of Alopecia.
