Objectives: Gluteal augmentation is the cosmetic surgery with the highest demand increase within the last years. Meanwhile, there are disinformation and concerns regarding the surgery’s safety. Objectives: Overcorrection of infragluteal area during circumferential thigh liposuction often resulted in gluteal ptosis and banana roll. The aim of this study was to describe my experience, including techniques and postoperative outcomes, by using various methods: lipoplasty with power-assisted liposuction (PAL) Microaire、buttock lipofilling、skin tightening with Renuvion.

Introduction: Overcorrection of infragluteal area during circumferential thigh liposuction often resulted in gluteal ptosis and banana roll. There are only limited methods were described with no generally accepted techniques. The author described his personal experience on treating infragluteal fold deformity with minimal invasive scar.

Materials / method: In this prospective study PAL (Microaire) was used for fat extraction and to perform reshaping of buttock contouring. Renuvion with 5mm wide and 27mm long hand piece with used for skin tightening. All the collected fat was centrifuged and re-injected into the buttocks to redefine proportionate contour. Postoperatively, photographs were taken.

Results: 10 Patients with age range from 22 years to 48. All patients are required to follow up at least 6 months. All the patient was asked to wear compression garment for at least two month. They returned to work 4 days after surgery and were back to full activity with one week. There is no minor nor major complications reported.

Conclusion: Currently, there is no generally accepted techniques for the correction of gluteal ptosis or banana roll resulted from overcorrection of circumferential thigh liposuction. The method that the author described have given a satisfactory result with minimal invasive scar.

Objectives: 1. Fine facial liposuction with the latest small UAL probe.

Introduction: The history of ultrasound-assisted liposuction(UAL) was begun by a famous Italian plastic surgeon, Michael Zochhi, who introduced the brand-new device in the early 1990s'. Though many advantages of UAL, it has not become popular, cause of unwanted complications like skin necrosis and seroma. Now, with the development of technology, the new fine probes make surgery simpler, easier, and safer without many complications.

Materials / method: The surgery of liposuction would be hard for surgeons by the labor and concentrations that need during the surgery. The ultrasound cavitation devices would be helpful to disrupt the fat tissues more selectively by preserving normal tissues around them. The high-definition lipo-sculpturing surgery would be possible by those devices more effectively for liposuction and fat grafting. Moreover, the latest fine probes will make the small liposuction of the face like jowl, cheek, and zygomatic areas.

Results: Those who enjoyed the fine liposuction of the face reported the subtle changes of the face were very pleasing without loosening of skin. Also, body contouring surgery by the UAL and fat grafting procedures together would be promising for men's high-definition sculpturing and breast fat grafting for women.

Conclusion: It was made possible to suck out small accumulations of superficial fat on the face by the fine probe of UAL, and it can be done under local anesthesia. Liposuction and fat grafting in one surgery would be another advantage of the UAL device with good outcomes.

Objectives: A Novel Procedure: Pelvic Augmentation for Ideal Waist Hip Ratio

Introduction: Waist Hip Ratio (WHR) and Body Mass Index (BMI) was thought to be the main factor of female attractiveness. 0.6 to 0.7 of WHR has been reported as an ideal female attractiveness. Surgical procedures including liposuction and chemical agents altering appetite and absorption of calories are introduced to get better body contouring. However, an attempt at an ideal WHR was insufficient. Because, up to this study, there were no genuine skeletal approaches to achieve the fundamental pelvic bone alteration for the achievement of ideal WHR.

Materials / method: A volumetric CT scanning was performed on pelvic bones, and accurate size and dimension were measured. Using a minimally invasive plate augmentation technique, minimizing soft tissue injury and scars, the Ilyac plate was inserted through the iliac crest and the incision size varied to the implant size. We had patients with Ilyac plates and patients with Ilyac plates and silicon implants.

Results: We could obtain ideal WHR by increasing the pelvic width as much as the patients desired and good cosmetic results. The neurovascular complication was not reported

Conclusion: Pelvic osteoplasty with Ilyac plate and silicon idealizing waist-hip ratio is promising.
