Introduction: Horizontal neck wrinkles are difficult to treat due to thin skin, repeated movement, and reduced dermal support. This study introduces a novel approach using a dermal filler composed of acellular dermal matrix (ADM) processed via supercritical decellularization. The method aims to safely restore volume and stimulate collagen without the complications of synthetic fillers.
Materials / method: Twenty patients with moderate-to-severe neck wrinkles received 2–3 sessions of 8% ADM filler injection using either a 30G needle or needle-free jet injector at 4-week intervals. No anesthesia or pretreatment was required. Clinical photos, 3D imaging, and GAIS were used to evaluate outcomes. Histological analysis was conducted on biopsy samples from consenting subjects.
Results: Significant improvement in wrinkle depth, skin texture, and elasticity was observed after 1–2 months. GAIS scores averaged 4.3/5. No Tyndall effect, nodule formation, or delayed reactions were reported. Histology showed increased fibroblast activity and neocollagenesis. The ADM filler demonstrated excellent tissue integration with no phase separation during injection.
Conclusion: Supercritical ADM fillers provide a safe and biologically active solution for neck rejuvenation. Unlike HA or CaHA, ADM delivers immediate structural support and collagen-rich dermal integration. This technique offers a reproducible and non-bulky approach to treating delicate neck folds, with both clinical and histological benefits confirmed.
