Clinical efficacy of combined nasolabial fat liposuction and thread lift for facial rejuvenation: a 3D imaging and patient satisfaction study

Objectives: Nasolabial folds are a common concern across all ages and ethnicities. Causes vary, but in Asian women in their 20s–30s, including Japan, excess nasolabial fat—above the folds—is often a key factor.
This study evaluates nasolabial fat liposuction with a thread lift, a procedure that removes fat and prevents sagging for facial rejuvenation.
We assessed cases treated at our clinic using 3D imaging analysis and patient surveys to determine the efficacy of this combined approach.

Introduction: Nasolabial folds result from age-related factors such as skin laxity, fat redistribution, and skeletal changes, making them a prevalent aesthetic concern.
Various treatment modalities, including dermal fillers, liposuction, and thread lifts, have been explored, each with inherent limitations.
This study investigates the synergistic benefits of combining nasolabial fat liposuction with a thread lift to achieve optimal facial rejuvenation outcomes.

Materials / method: This study included 21 patients, aged 20 to 50, who underwent combined nasolabial fat liposuction and thread lift.
The procedure was performed under intravenous anesthesia using microcannula liposuction, followed by the insertion of polydioxanone (PDO) threads along the
nasolabial folds and midface region.
Treatment outcomes were assessed through visual analysis
(before-and-after photo comparisons), volumetric changes in the nasolabial fat area (measured via specialized equipment), and patient satisfaction surveys, including FACE-Q assessments andｘ.

Results: The combined treatment significantly reduced the depth and visibility of nasolabial folds.
At the three-month follow-up, objective 3D imaging (Life Viz®️) demonstrated an average reduction of 0.50cc in nasolabial fat volume.
The FACE-Q assessment for "nasolabial fold depth (bare face)"
yielded an average score of 3.29, reflecting high patient satisfaction.
Postoperative symptoms such as pain, swelling, warmth, and tightness were noted but resolved completely within three months, with no severe complications reported.

Conclusion: This study highlights the efficacy of nasolabial fat liposuction with a thread lift in reducing nasolabial folds and enhancing facial rejuvenation.
Objective 3D imaging confirmed fat volume reduction, while high patient satisfaction reinforced its effectiveness.
Mild postoperative symptoms resolved within three months, with no severe complications.
These findings suggest this combined approach is a safe, effective option for individuals in their 20s to 50s seeking facial contour refinement.

Reverse vector temple anchoring: a crucial factor in thread lifts

Objectives: • Master the technique of reverse vector temple anchoring for facial thread lifting, including precise thread placement, and tensioning.
• Evaluate the immediate and long-term efficacy of reverse vector temple anchoring in improving facial contour and reducing signs of aging.
• Determine the optimal placement and tensioning of reverse vector temple anchors to maximize lifting and tightening effects.

Introduction: Thread lifting has emerged as a popular cosmetic procedure due to its minimal downtime and safety profile. As thread technology continues to advance, its immediate effects have become increasingly pronounced. However, a standardized procedure technique remains elusive.

Materials / method: To address this need, this study aimed to develop a standardized procedure that would satisfy both surgeons and patients. We analyzed the outcomes of 2,700 patients who underwent facial thread lifting at PangPang Clinic in Seoul between November 1, 2022, and November 30, 2024. By carefully selecting thread designs and types based on individual patient needs, we evaluated the efficacy of a specific technique: reverse vector temple anchoring.

Results: The reverse vector temple anchoring technique proved to be a valuable addition to the facial thread lifting procedure. It not only lifted sagging skin but also served as a guide for accurate placement of other threads. By temporarily fixing sagging skin in the temple area, this technique facilitated immediate lifting effects and potentially stimulated collagen production, leading to longer-lasting results.

Conclusion: The reverse vector temple anchoring technique offers a promising approach to enhance the immediate and long-term outcomes of facial thread lifting. By providing a precise framework for thread placement, this technique can help surgeons achieve more predictable and aesthetically pleasing results. We recommend incorporating this technique into routine facial thread lifting procedures.

Correction of nasolabial fold and marionette line using extension-type cogged threads

Objectives: This study aims to evaluate the efficacy of a novel extension-type cogged thread in addressing lower facial sagging, specifically focusing on the correction of nasolabial folds and marionette lines. By reversing the cog direction of traditional threads, this innovative design seeks to enhance tissue lifting and pushing effects in the lower face.

Introduction: Threads are widely used in cosmetic procedures, with most relying on cogs to anchor tissues and induce contraction for lifting. While effective in the upper face to reverse sagging, the lower face often benefits more from pushing and lifting tissues. To address this, a specially designed extension-type thread was developed, reversing the cog direction of traditional threads, and applied to the lower face.

Materials / method: From February to December 2024, bidirectional sculpted cogged threads (4 cm, PLACL material) were inserted into 20 patients to treat nasolabial folds and marionette lines. Photographs taken pre-procedure and two weeks post-procedure were analyzed and categorized as Good, Moderate, or No Change.

Results: For nasolabial folds, 13 patients showed good results, 4 moderate, and 3 no change. For marionette lines, 17 patients showed good results, and 3 moderate.

Conclusion: The specially designed extension-type cogged thread provided excellent aesthetic outcomes, particularly for lower facial sagging, and enhanced the efficacy of thread lifting technology, suggesting a significant advancement in minimally invasive cosmetic procedures.

Threads vs fillers: the battle for a perfect facelift

Introduction: Discussion of a crucial part of modern aesthetics,the question for a youthful and radiant appearance.As we navigate the vast variety of non surgical cosmetic treatments available two most popular options stand out. Threads and fillers
Both promise to revitalise our looks but which one is the best choice?
We will delve into the world of threads and fillers exploring their differences , benefits and limitations.
We are discussing the benefits and limitations of fillers and threads in anti-aging medicine.Discussing the role of customisation and patient selection.

Materials / method: We enrolled 30 subjects aging between 30 to 60 years.We treated 10 subjects with fillers to give lifting and facial rejuvenation effect.We treated 10 subjects with threads for face lifting and sagging treatment.
We treated 10 subjects with a combination of threads and fillers to achieve the maximum benefit of both the treatments.
We used three to 4 ml of fillers on 10 of each patient. We used a box of 10 threads for 10 of each patient.We used a combination of a box of threads having 10 threads and combined it with 2 mls of fillers on each of 10 patients.

Results: Results of only fillers
Patients experienced a reduction in lines and hollowness. Results lasted for a year. 4 subjects didn’t get a lift. 1 subject complained about the puffiness. 5 out of 10 patients were satisfied.
Result of only threads
10 subjects experienced a reduction in lines , hollowness and sagging with an improved contour..10 patients saw an improved quality of skin.
Results of combination therapy
9 out of 10 patients were satisfied with the results.10 patients saw an improvement in quality of skin.All the patients saw a reduction in sagging and hollowness of their face.

Conclusion: Two treatments when combined together according to the requirements of patients are better than one .We should avoid wrong selection of patients.
We should avoid giving false hopes and over promising results to the patients.Products which are not designed to deliver specific outcomes should not be over used.Threads and fillers when used according to the need of the patient deliver a comprehensive and harmonious result.
We should customise the treatment according to the patient’s need and choice.

Nonsurgical Rejuvenation of the Periorbital Region Using PDO threads, Dermal fillers and Biostimulators

Objectives: 1. Evaluate improvements in skin quality, volume restoration, and tear trough deformities.

2. Assessing the longevity of results and patient satisfaction.

3. Classifying patients using Hirmand's tear trough grading system to tailor treatments.

Introduction: One of the earliest areas to exhibit signs of aging is the periorbital region due to volume depletion, skin laxity and decrease in collagen. The common concerns are tear trough deformities, hollowness, fine lines, and skin thinning. While blepharoplasty, a surgical option, offer long lasting results, some patients prefer nonsurgical treatments with minimal downtime. This study
evaluates the efficacy and safety of PDO threads, hyaluronic acid dermal fillers, and biostimulators for periorbital rejuvenation, incorporating Hirmands's tear trough classification for treatment selection.

Materials / method: A prospective study was conducted with nine adults 9 (3 in each group) between 30 and 80 years of age with periorbital aging concerns, with no history of aesthetic treatment in this region for the past three months were included after their informed consent. Treatment was tailored based on Hirmand's tear trough grading classification. Grade 1 and 2 hyaluronic acid fillers for volume restoration, grade 2 and 3 HA fillers, biostimulators (poly-D L-Lactic Acid), grade 3 hyaluronic acid fillers, biostimulators and PDO threads. Standardized photography and Lemperly wrinkle scale were used.

Results: Hirmand's Grade 1 classification patients showed the best response to fillers and an immediate Lemperle 2 point reduction. Grades 2 and 3 achieved an immediate 1.5 point reduction with more improvement in 2 weeks.

Conclusion: Nonsurgical periorbital rejuvenation using PDO threads, dermal fillers, and biostimulators is a safe and effective alternative to surgery. Combination therapies yield superior results by addressing volume loss, skin laxity, and collagen depletion. Further studies with larger sample sizes and longer follow-up periods are recommended to refine treatment protocols and improve outcomes.
