Objectives: The aim of the network ResoVerneuil is to improve global health, management and quality of care in HS patients

Introduction: ResoVerneuil is a French French-speaking network created in 2015 and dedicated to any medical or paramedical practionner interested in the field of hidradenitis suppurativa (HS).

Materials / method: • Improving care pathway and management of HS patients
• Develop research in the field of HS
• Favor information and experience sharing, and dissemination of knwoledge
• Breaking prejudices conveyed by HS by raising awareness of health care providers, general public and decision-makers

Results: The network has near 200 members, with either public or liberal activities, including dermatologists and surgeons.

Conclusion: ResoVerneuil offers a town-hospital network as close as possible to the patients. It also supports clinical research by facilitating patients recruitment.

Objectives: To improve the knowledge about the principles, the indications and the techniques of surgical treatment of hidradenitis suppurativa (HS)

Introduction: HS remains poorly recognized by practionners adn as such may be treated unefficiently. Surgery is one of the major treatment option and deseves to be performed by aware practitionners

Materials / method: Presentation of a 12 year experience of surgery for HS, higlighted by a literature review

Results: participants to this session will have practical data to choose how and when operate the patients and how to develop a surgical activity in the field of HS

Objectives: Understand the main features of the disease, the challenges with the treatments and how to choose between antibiotics and biotherapies.

Introduction: As HS is both an infectious and auto-inflammatory disease, treatments can be both antibiotics and anti-cytokines antibodies, mainly anti-TNF-alpha. Surgery is also an important option which will be discussed in another session.

Materials / method: Review of the literature ans experience of RESOVERNEUIL in France (french network for HS).

Results: The main protocol is the association of rifampicin / clindamycin. Many protocols have derived now from this historical one: rifampicin / ofloxacin; rifampicin / moxifloxacin / metronidazole in induction phase, and then cotrimoxazole in maintenance phase. For biologics, the anti-TNF adalumimab in the only one to have an agreement in Europe. But many new molecules are under investigation: anti-IL-17 (secukinumab), anti-IL-23 (guselkumab), anti-complement fractions (anti-C5a). The field of treatments is now increasing every year.

Conclusion: The best option depends on the main clinical presentation: purulent or rather inflammatory phase. In a same patient, treatment can change depending on a particular phase of the disease. It remains anyway a multidisciplinary approach.

Objectives: To evaluate the effectiveness, safety and tolerability of intralesional photodynamic therapy (I-PDT) in the management of HS. Also, to assess the effect of this technique on the different areas treated

Introduction: Hidradenitis suppurativa (HS) is a chronic, inflammatory, recurrent skin condition which has no cure nowadays . Photodynamic therapy (PDT) has been used for more than ten years with different results because of the depth of the disease. Intralesional PDT, could resolve this limitation.

Materials / method: Review of our experience in more than 60 patients with HS treated with intralesional PDT. This technique consists of the use of a photosensitizer gel (5-aminolevulinic acid 2%) introduced inside the fistulas or abscesses to treat to be incubated for two hours. Later a 630nm laser light irradiates the lessions. Finally patients are reevaluated with different HS scales to identify the response of the technique.

Results: In case series of 38 patients 29 achieved complete response , and 18 of them needed only one session and 8 patients had persistence of lessions. Difference between basal and final Hidradenitis Severity Score showed a significant reduction of 24.5 points (p<0.001). Dermatology Life Quality Index scores reached a reduction of 10 points (p<0.001)

Conclusion: Intralesional-PDT could be an excellent local treatment for HS lesions. It has achieved high rate of long-term remission and without relevant complications.
